BackgroundDual-task, performing a balance task and a cognitive task simultaneously, has been used as sport-related concussion assessment tool. However, the effect of dual task on balance performance remains controversial. Also, the fluctuation of hormone across menstrual cycle on balance and dual-task performance has not been fully examined.ObjectiveTo examine (1) the effect of dual task on balance performance, (2) the interaction of female sex hormonal level, balance, and dual-task performance.DesignA cohort study.SettingA laboratory of concussion research and services.ParticipantsA group of 49 healthy female college-aged students (age = 21.6 ± 2.0 years).Main outcome measuresThe Stability Evaluation Test (SET) protocol, Balance error scoring system (BESS) test and postural sway velocity (deg/sec) by using VSR SPORT force plate. Three auditory mathematics questions were given for each condition of the SET test. Two single-task conditions were provided, including balance only and math only. Balance test and math questions were given simultaneously during the dual-task condition.ResultsThere was no significantly statistically difference in BESS or sway velocity between single and dual task. However, the performance of math calculation is better in dual-task comparing to the single task (math only), with an 8%–12% higher rate of accuracy, but it didn't reach a statistical significance. The rate of accuracy during the dual task didn't vary among menstrual phases.ConclusionsBalance performance was maintained under dual-task conditions and fluctuation of female sex hormones across the menstrual cycle may not affect working memory and executive function. These findings are potentially clinically applicable to detecting balance deficits and cognitive alterations in female concussed individuals. Future studies are needed to investigate the underlying mechanism of balance maintenance and attention dividend under dual-task conditions.

We report a study that investigated the neuroanatomical correlates of executive functions in dual-task performance with functional magnetic resonance imaging. Participants performed an auditory and a visual three-choice reaction task either separately as single tasks or concurrently as dual tasks. In the dual-task condition, two stimuli were presented in rapid succession to ensure interference between the component tasks (psychological refractory period). The behavioral data showed considerable performance decrements in the dual-task compared to the single-task condition. Dual-task-related activation was detected with two different neuroimaging methods. First, we determined dual-task-related activation according to the method of cognitive subtraction. For that purpose, activation in the dual-task was compared directly with activation in the single-task conditions. This analysis revealed that cortical areas along the inferior frontal sulcus (IFS), the middle frontal gyrus (MFG), and the intraparietal sulcus (IPS) are involved in dual-task performance. The results of the subtraction method were validated with the method of parametric manipulation. For this purpose, a second dual-task condition was introduced, where the difficulty of the dual-task coordination was increased compared with the first dual-task condition. As expected, behavioral dual-task performance decreased with increased dual-task difficulty. Furthermore, the increased dual-task difficulty led to an increase of activation in those cortical regions that proved to be dual-task related with the subtraction method, that is, the IFS, the MFG, and the IPS. These results support the conclusion that dorsolateral prefrontal and superior parietal cortices are involved in the coordination of concurrent and interfering task processing.

Abstract Objective This study aimed to investigate the effects of dual-task gait training using a treadmill on gait ability, dual-task interference, and fall efficacy in people with stroke. Methods Patients with chronic stroke (N = 34) were recruited and randomly allocated to the experimental or control group. Both groups underwent gait training on a treadmill and a cognitive task. In the experimental group, gait training was conducted in conjunction with the cognitive task, whereas in the control group, the training and the cognitive task were conducted separately. Each intervention was provided for 60 minutes, twice a week, for a period of 6 weeks for both groups. The primary outcomes were as follows: gait parameters (speed, stride, variability, and cadence) under single-task and dual-task conditions, correct response rate (CRR) under single-task and dual-task conditions, and dual-task cost (DTC) in gait parameters and CRR. The secondary outcome was the fall efficacy scale. Results Dual-task gait training using a treadmill improved all gait parameters in the dual-task condition, speed, stride, and variability in the single-task condition, and CRR in both conditions. Difference between the groups was observed in speed, stride, and variability in the dual-task condition. Furthermore, dual-task gait training on a treadmill improved DTC in speed, variability, and cadence along with that in CRR, indicating true improvement of DTC, which led to significant improvement in DTC in speed and variability compared with single-task training. Conclusions Dual-task gait treadmill training was more effective in improving gait ability in dual-task training and DTI than single-task training involving gait and cognitive task separately in people with chronic stroke.

Prior research has suggested that measurements of brain functioning and performance on dual tasks (tasks which require simultaneous performance) are promising candidate predictors of fall risk among older adults. However, no prior study has investigated whether brain function measurements during dual task performance could improve prediction of fall risks and whether the type of subtasks used in the dual task paradigm affects the strength of the association between fall characteristics and dual task performance. In this study, 31 cognitively normal, community-dwelling older adults provided a self-reported fall profile (number of falls and fear of falling), completed a gait dual task (spell a word backward while walking on a GaitRite mat), and completed a supine dual task (rhythmic finger tapping with one hand while completing the AX continuous performance task (AX-CPT) with the other hand) during functional magnetic resonance imaging (fMRI). Gait performance, AX-CPT reaction time and accuracy, finger tapping cadence, and brain functioning in finger-tapping-related and AX-CPT-related brain regions all showed declines in the dual task condition compared to the single task condition. Dual-task gait, AX-CPT and finger tapping performance, and brain functioning were all independent predictors of fall profile. No particular measurement domain stood out as being the most strongly associated measure with fall variables. Fall characteristics are determined by multiple factors; brain functioning, motor task, and cognitive task performance in challenging dual-task conditions all contribute to the risk of falling.

Purpose Prior investigations suggest that simultaneous performance of more than 1 motor-oriented task may exacerbate speech motor deficits in individuals with Parkinson disease (PD). The purpose of the current investigation was to examine the extent to which performing a low-demand manual task affected the connected speech in individuals with and without PD. Method Individuals with PD and neurologically healthy controls performed speech tasks (reading and extemporaneous speech tasks) and an oscillatory manual task (a counterclockwise circle-drawing task) in isolation (single-task condition) and concurrently (dual-task condition). Results Relative to speech task performance, no changes in speech acoustics were observed for either group when the low-demand motor task was performed with the concurrent reading tasks. Speakers with PD exhibited a significant decrease in pause duration between the single-task (speech only) and dual-task conditions for the extemporaneous speech task, whereas control participants did not exhibit changes in any speech production variable between the single- and dual-task conditions. Conclusions Overall, there were little to no changes in speech production when a low-demand oscillatory motor task was performed with concurrent reading. For the extemporaneous task, however, individuals with PD exhibited significant changes when the speech and manual tasks were performed concurrently, a pattern that was not observed for control speakers. Background. Poststroke individuals use their paretic arms less often than expected in daily life situations, even when motor recovery is scored highly in clinical tests. Real-world environments are often unpredictable and require the ability to multitask and make decisions about rapid and accurate arm movement adjustments. Objective. To identify whether and to what extent cognitive–motor deficits in well-recovered individuals with stroke affect the ability to rapidly adapt reaching movements in changing cognitive and environmental conditions. Methods. Thirteen individuals with mild stroke and 11 healthy controls performed an obstacle avoidance task in a virtual environment while standing. Subjects reached for a virtual juice bottle with their hemiparetic arm as quickly as possible under single- and dual-task conditions. In the single-task condition, a sliding glass door partially obstructed the reaching path of the paretic arm. A successful trial was counted when the subject touched the bottle without the hand colliding with the door. In the dual-task condition, subjects repeated the same task while performing an auditory–verbal working memory task. Results. Individuals with stroke had significantly lower success rates than controls in avoiding the moving door in single-task (stroke: 51.8 ± 21.2%, control: 70.6 ± 12.7%; P = .018) and dual-task conditions (stroke: 40.0 ± 27.6%, control: 65.3 ± 20.0%; P = .015). Endpoint speed was lower in stroke subjects for successful trials in both conditions. Obstacle avoidance deficits were exacerbated by increased cognitive demands in both groups. Individuals reporting greater confidence using their hemiparetic arm had higher success rates. Conclusion. Clinically well-recovered individuals with stroke may have persistent deficits performing a complex reaching task.

AbstractWhen humans are required to perform two tasks concurrently, their performances decrease as the two tasks get closer together in time. This effect is known as dual-task interference. This limitation of the human brain could have lethal effects during demanding everyday tasks such as driving. Are the two tasks processed serially or in parallel during dual-task performance in naturalistic settings? Here, we investigated dual-task interference in a simulated driving environment and investigated the serial/parallel nature of processing during dual-task performance. Participants performed a lane change task on a desktop computer, along with an image discrimination task. We systematically varied the time difference between the onset of the two tasks (Stimulus Onset Asynchrony, SOA) and measured its effect on the amount of dual-task interference. Results showed that the reaction times (RTs) of two tasks in the dual-task condition were higher than those in the single-task condition. SOA influenced RTs of both tasks when they were presented second and the RTs of the image task when it was presented first. Manipulating the predictability of the order of the two tasks, we showed that unpredictability attenuated the effect of SOA by changing the order of the response to the two tasks. Next, using drift-diffusion modeling, we modeled the reaction time and choice of the subjects during dual-task performance in both predictable and unpredictable task order conditions. The modeling results indicated that performing two tasks concurrently, affects both the rate of evidence accumulation and the delays outside the evidence accumulation period, suggesting that the two tasks are performed in a partial-parallel manner. These results extend the findings of previous dual-task experiments to more naturalistic settings and deepen our understanding of the mechanisms of dual-task interference.

The extent and nature of the ability to control the allocation of mental resources between the components of a dual task was investigated in three separate experiments. Using a variable priority (demand) methodology it was found that subjects could manipulate their performance level, however their ability to meet specific demand levels was limited. Training subjects under single or dual-task conditions using a wide range of task demand significantly improved dual task performance and degree of control over resource allocation as compared to performance following practice under a narrow range of task demands or under single task fixed demand conditions. Single task performance among all groups improved to the same degree. It was concluded that training subjects under a wide range of task demands increases the range of performance levels over which mental resources can be flexibly allocated for those tasks and improves time sharing performance. Implications for the design of training for complex task performance are discussed.

The purpose of this study was to investigate physical and cognitive performance during dual task conditions of upper-extremity (UE) or full-body (FB) rowing exercise. In a crossover counterbalanced design, college-aged male and female participants completed five conditions: (a) Sitting, (b) Single task UE rowing, (c) Single task FB rowing, (d) Dual task UE rowing, and (e) Dual task FB rowing. For single task UE and FB rowing conditions, participants were asked to row as hard as possible. After sitting and dual-task conditions, we administered the Paced Auditory Serial Addition Test (PASAT) and a word-list memory test. We analyzed participants’ absolute differences (single task – dual task) in power output and their cognitive test scores to compare UE and FB rowing. There were no significant absolute differences from sitting to dual task conditions of UE and FB rowing for either PASAT ( p = 0.958) or word list memory ( p = 0.899) cognitive scores. Absolute power output loss from single to dual task conditions was significantly higher in FB versus UE for PASAT ( p = 0.039; d = 0.54) and word list memory ( p = 0.021; d = 0.66) in the dual task condition. These results suggest that, while cognitive performance was preserved regardless of the amount of muscle mass activated during dual task rowing, physical performance suffered more during FB than UE rowing under the dual task condition. These findings have important implications for optimizing cognitive and physical performance in dual task situations.

(1) Background: The aim of this study was to investigate the association between single, dual task performance and impulsiveness personality traits of young adults in the community. (2) Methods: As a cross-sectional study, the Korean version of the Barratt Impulsiveness Scale-11-Revised test, which is an impulsiveness indicator test tool, was conducted on a total of 62 healthy young adults in the community. In order to assess the task performance ability, single task, motor dual task, and cognitive-motor dual task of Timed up and go (TUG) test were conducted. (3) Results: In order to identify the mean difference of the three types of TUG task performance according to the total score of impulsiveness test of all subjects, one-way ANOVA analysis was performed. As a result, there was no statistically significant difference by each task type. Upon investigating the correlation between the three subtypes (cognitive impulsiveness, motor impulsiveness, non-planned impulsiveness) of impulsiveness and total score and TUG task performance, cognitive impulsiveness and TUG-cognitive task performance showed statistically significant correlation. (4) Conclusions: There was an association with the degree of cognitive impulsiveness when performing motor task and cognitive task simultaneously.

PURPOSE: Aside from typical symptoms such as dizziness and vertigo, persons with vestibular disorders often have cognitive and motor problems. These symptoms have been assessed in single-task condition. However, dual-tasks assessing cognitive-motor interference might be an added value as they reflect daily life situations better. Therefore, the 2BALANCE protocol was developed. In the current study, the test-retest reliability of this protocol was assessed. METHODS: The 2BALANCE protocol was performed twice in 20 healthy young adults with an in-between test interval of two weeks. Two motor tasks and five different cognitive tasks were performed in single and dual-task condition. Intraclass correlation coefficients (ICC), the standard error of measurement, and the minimal detectable difference were calculated. RESULTS: All cognitive tasks, with the exception of the mental rotation task, had favorable reliability results (0.26≤ICC≤0.91). The dynamic motor task indicated overall substantial reliability values in all conditions (0.67≤ICC≤0.98). Similar results were found for the static motor task during dual-tasking (0.50≤ICC≤0.92), but were slightly lower in single-task condition (–0.26≤ICC≤0.75). CONCLUSIONS: The 2BALANCE protocol was overall consistent across trials. However, the mental rotation task showed lowest reliability values.
