Type 2 diabetes mellitus (T2DM) is a chronic, low-grade inflammation disease. T follicular helper (Tfh) cells and T cell immunoglobulin and mucin domain 3 (Tim-3) are implicated in many immune diseases. This study aims to explore whether Tim-3 expression on Tfh cells is associated with T2DM progression. White blood cells (WBCs) were harvested from 30 patients with T2DM and 20 healthy donors. The abundance of circulating Tfh cells (cTfh) and the frequency of Tim-3 were analyzed by flow cytometry. Levels of fasting plasma glucose (FPG), insulin, hemoglobin A1C (HbA1C), and fasting plasma C-peptide were measured. Body mass index (BMI) and diabetes duration were also recorded. Patients with T2DM had higher numbers of cTfh cells. In addition, cTfh cells showed a negative correlation with HbA1C and diabetes duration, a positive correlation with fasting plasma C-peptide. The frequency of Tim-3 on cTfh cells was higher among T2DM patients compared with healthy donors. The in vitro experiment showed that high glucose levels increased the abundance cTfh cells but had no effect on Tim-3 expression. Our results suggest that cTfh cells and associated Tim-3 frequency may contribute to the progression of T2DM, and high glucose levels may influence cTfh cells directly.

The functions of absorption of dietary glucose by the small intestine and reabsorption of filtered glucose by the renal proximal tubule are strikingly similar in their organization and in the way they adapt to uncontrolled diabetes mellitus. In both cases, transepithelial glucose and Na+ fluxes are augmented. The epithelial adaptations to hyperglycemia of uncontrolled diabetes are accomplished by increasing the glucose transport surface area and the number of the efflux glucose transporter GLUT2 located in the basolateral membrane. The signals that modify the size of the epithelium and the overexpression of basolateral GLUT2 are not known. It was speculated that high glucose levels and enhanced Na+ flux may be important factors in the signaling event that culminates in a renal and intestinal epithelium that is modified to transport higher rates of glucose against a higher extracellular level of glucose.

Abstract—COVID-19 is a major emerging disease that affects any certain condition. However, a recent report suggests the occurrence of hyperglycemia without any present diabetes in COVID-19 patients. This study aimed to systematically review recent evidence on hyperglycemia in COVID-19 patients. Literature research was done using four search engines, consist of Google Scholar, PubMed, ScienceDirect, and ProQuest, and limited to English manuscript only and published from February 2020 to September 2020. SARS-CoV-2 could damage the pancreas by causing the destruction of the β-cell structure that leads to impairment of glucose metabolism and worsen pre-existing diabetes or determine the appearance of hyperglycemia in non-diabetes. Inflammation also plays a major important role in hyperglycemia related to COVID-19.  Hyperglycemia increased the vulnerability of the lung, by promoting and facilitating the entry of the SARS-CoV-2 into the host cells, and decreasing lung function. Moreover, the mortality and morbidity rate conceivable increased due to hyperglycemia. The presence of high glucose levels is linked with the progression of COVID-19 severity. Thus, the glucose level should be concerned, either in a patient with present diabetes or without any presence of diabetes. Examination and monitoring of glucose levels might be a useful tool to prevent the seriousness of COVID-19 Keywords: diabetes mellitus, SARS-CoV-2, high glucose level, pulmonary infection   Abstrak—COVID-19 adalah penyakit yang muncul yang mempengaruhi kondisi  tertentu. Abstract Background Ethnobotany knowledge in a community has shaped local wisdom in utilizing plants to treat diseases, such as the use of Malaka (Phyllanthus emblica) flesh to treat type 2 diabetes. This study presented evidence that the phenolic extract of the Malaka flesh could reduce blood sugar levels in the diabetic induced rats. Methods The phenolic extract of the P. emblica was administrated to the glucose-induced rats of the Wistar strain Rattus norvegicus for 14 days of treatment where the Metformin was used as a positive control. The data generated were analyzed by the two-way ANOVA Software related to the blood glucose level and by SAS Software related to the histopathological studies at a significant 95% confidence. Results The phenolic extract with concentrations of 100 and 200 mg/kg body weight could reduce blood glucose levels in diabetic rats. The post hoc Dunnet test showed that the administration of the extract to the rats with a concentration of 100 mg/kg body weight demonstrated a very significant decrease in blood glucose levels and repaired damaged cells better than administering the extract at a concentration of 200 mg/kg weight body. Conclusion The evidence indicated that the phenolic extract of the Malaka flesh can be utilized as anti type 2 Diabetes mellitus without damaging other organs.
