There has been a lively exchange of views in the British Journal of Occupational Therapy about the role of occupational therapy in mental health. This opinion piece presents an argument that occupational therapy must be more flexible and develop generic working. Suggestions are made for future practice, research and education.

This opinion piece advocates the need for occupational therapists to undertake more profession-specific work. Its argument draws upon real life case studies as well as the service users' views expressed in the recent College of Occupational Therapists' 10-year strategy for occupational therapy mental health services.

Occupational therapy has an established role in the delivery of mental health services. Psychiatric Intensive Care Units (PICUs) are a development found in many psychiatric inpatient facilities and their numbers are increasing. The introduction of occupational therapy to a recently established PICU is examined. A comparison is made between PICU and forensic client groups, and the developing need for PICUs as a distinct service and the client group they serve are described. An example of successful service delivery of occupational therapy in a PICU is offered, and the challenges to the therapist working in this demanding and rewarding setting are outlined.

Abstract Background: In developing countries (DCs) such as Saudi Arabia (SA), previous studies have considered EDs to be rare and related to westernised culture, where a thin physique is idolised. However, the rapid transition in lifestyles in DCs makes it reasonable to hypothesise that ED prevalence may be rising in these countries. Similarly, studies in SA show that obesity levels are increasing. However, studies that include the role of psychology for obesity management or in the assessments for metabolic surgeries (MS) are lacking. These deficiencies urge to measure this phenomenon in SA and develop additional management strategies for these disorders. A survey was designed to evaluate the number of referrals for EDs and obesity, the management and level of availability of ED specialists as reported by health care staff and the use of mental-health services in treating EDs and obesity. Methods: A total of 15 health services in SA were surveyed and a total of 123 survey questionnaires were completed by health professionals. Participants were asked to respond to a written survey that included questions regarding the number of referrals for EDs and obesity and available treatment as well as body mass index criteria for MS and the number of patients referred for these procedures. Results: Findings from this study add to those from a few previous studies revealing that all EDs are equally prevalent in diverse cultures. Additionally, this survey showed that the obesity rate is high, however, there is limited psychological interventions in this regard. Conclusion: The perspective of narrowing the cultural categories to either western or non-western countries when considering the aetiology of EDs without considering other socioeconomic factors needs to be changed. There is a requirement for specialised ED training, a need to identify a conceptual framework to provide evidence-based management and increase the levels of involvement of psychiatric professionals in the management of obesity. Although this study was conducted in one region, the results should be generalisable to other areas and to other non-westernised countries were EDs are considered rare. O estudo propõe um debate sobre o cuidado em saúde mental na atenção básica, partindo do entendimento de que a produção do cuidado se dá em redes, não apenas institucionais, mas redes vivas e de potência criativa. O percurso metodológico se deu por meio da cartografia, de modo que foi possível rastrear o território para além dos serviços de saúde. Como meio de produção de dados utilizaram-se os diários de campo produzidos na vigência de doze meses do projeto de extensão: Saúde Mental na atenção básica- desafio e necessidade, desenvolvido em uma unidade integrada de saúde da família de um município do estado da Paraíba. A vivência e atuação no território se deram inicialmente pelo mapeamento das redes de cuidado territoriais, como também a problematização das ações de cuidado na atenção básica, com o intuito de ampliar o debate sobre o papel das redes vivas no cuidado em saúde. Tal discussão possibilitou o desenvolvimento de ações em saúde pautadas na integralidade do cuidado, coesão social, produção de vida. Estes resultados apontam para a necessidade em se pensar ações compartilhadas e inseridas no território, tendo em vista que a saúde envolve também os territórios existenciais pelos quais os sujeitos circulam e produzem relações. A terapia ocupacional tem contribuído significativamente na consolidação deste cuidado. The study proposes a debate on mental health care in primary care, based on the understanding that the production of care takes place in networks, not only institutional, but also living networks and creative power. The methodological approach was through the mapping, so it was possible to trace the territory beyond the health services. As a means of production, data field diaries were  produced in the presence of twelve months of the extension project: Mental Health in the primary care - challenge and need, developed in an integrated health unit family in a municipality of state of Paraíba. The experience and operation in the territory was given initially by mapping of care networks available in the territory, as well as the questioning of care actions in primary care, in order to broaden the debate on the role of living networks in health care. This discussion enabled the development of health actions based on integrality of care, social cohesion, production of life in primary care. These results point to the need to think about shared and embedded actions in the territory, with a view that health also involves the existential territories for which individuals circulate and produce relations. Occupational therapy has significantly contributed to the consolidation of territorial care.Keywords: Comprehensive health care; Primary health care; Social network; Mental health, Occupational therapy. El estudio propone un debate sobre el cuidado en salud mental en la atención básica, basado en el entendimiento de que la producción de la atención se lleva a cabo en las redes, no sólo institucional, sino redes vivas y con poder creativo. El enfoque metodológico fue a través de la medio de la cartografía, por lo que fue posible rastrear el territorio más allá de los servicios de salud. Como medio de producción de datos se utilizaron diarios de campo producidos en la vigencia de doce meses del proyecto de extensión: Salud Mental en la atención básica -  desafio y necesidad, desarrollado en una unidad de salud de la familia integrada un condado en el estado de Paraiba. La experiencia y las operaciones en el territorio fue dado inicialmente por el mapeo de las redes de atención disponibles en el territorio, así como el cuestionamiento de las acciones de atención en la atención primaria, con el fin de ampliar el debate sobre el papel de las redes vivas en el cuidado de la salud. Esta discusión permitió el desarrollo de las acciones de salud basados en la atención integral, la cohesión social, la producción de vida. Estos resultados apuntan a la necesidad de pensar acciones compartidas en el territorio, teniendo en cuenta que la salud involucra también los territorios existenciales por los cuales los sujetos circulan y producen relaciones. This chapter covers perinatal mental health problems, both those that develop in pregnancy and those that may be encountered as pre-existing when a patient becomes pregnant. Specific phobias relating to the state of being pregnant (tocophobia) or issues encountered in pregnancy (blood, injury, and injection phobia), as well as depression, schizophrenia, eating disorders, self-harm, and substance abuse are covered. Treatment without consent and the role of the mother and baby unit are discussed.
