Abstract Background Global gains toward malaria elimination have been heterogeneous and have recently stalled. Interventions targeting afebrile malaria infections may be needed to address residual transmission. We studied the efficacy of repeated rounds of community-based mass testing and treatment (MTaT) on malaria infection prevalence in western Kenya. Methods Twenty clusters were randomly assigned to 3 rounds of MTaT per year for 2 years or control (standard of care for testing and treatment at public health facilities along with government-sponsored mass long-lasting insecticidal net [LLIN] distributions). During rounds, community health volunteers visited all households in intervention clusters and tested all consenting individuals with a rapid diagnostic test. Those positive were treated with dihydroartemisinin-piperaquine. Cross-sectional community infection prevalence surveys were performed in both study arms at baseline and each year after 3 rounds of MTaT. The primary outcome was the effect size of MTaT on parasite prevalence by microscopy between arms by year, adjusted for age, reported LLIN use, enhanced vegetative index, and socioeconomic status. Results Demographic and behavioral characteristics, including LLIN usage, were similar between arms at each survey. MTaT coverage across the 3 annual rounds ranged between 75.0% and 77.5% in year 1, and between 81.9% and 94.3% in year 2. The adjusted effect size of MTaT on the prevalence of parasitemia between arms was 0.93 (95% confidence interval [CI], .79–1.08) and 0.92 (95% CI, .76–1.10) after year 1 and year 2, respectively. Conclusions MTaT performed 3 times per year over 2 years did not reduce malaria parasite prevalence in this high-transmission area. Clinical Trials Registration NCT02987270.

Delivering community-based benefits is oftentimes cited to justify the high costs associated with hosting large-scale events. The current research is embedded in positive psychology to examine how an event impacts host community members’ PERMA domains, reflected through positive emotions, engagement, relationships, meaning, and accomplishment. Adopting a longitudinal approach, the authors interviewed 15 host community members before and after a large-scale sport event to determine if and how the event impacted their well-being. The findings uncovered evidence that the event activated positive emotions, relationships, and meaning across both phases, and evidence of accomplishment within the postevent phase. The findings contribute to the knowledge by examining the links between large-scale sport events and well-being throughout the event lifecycle. This research forwards implications for event bidding committees, event organizers, and host community officials to maximize community well-being through hosting large-scale events and to help justify associated expenses from a social–psychological perspective.

There is a significant knowledge-to-action gap in cancer survivorship care for First Nations (FN) communities. To date, many approaches to survivorship have not been culturally responsive or community-based. This study is using an Indigenous knowledge translation (KT) approach to mobilize community-based knowledge about cancer survivorship into health-care programs. Our team includes health-care providers and cancer survivors from an FN community in Canada and an urban hospital that delivers Cancer Care Ontario’s Aboriginal Cancer Program. Together, we will study the knowledge-to-action process to inform future KT research with Indigenous peoples for improving health-care delivery and outcomes. The study will be conducted in settings where research relations and partnerships have been established through our parent study, The National Picture Project. The inclusion of community liaisons and the continued engagement of participants from our parent study will foster inclusiveness and far-reaching messaging. Knowledge about unique cancer survivorship needs co-created with FN people in the parent study will be mobilized to improve cancer follow-up care and to enhance quality of life. Findings will be used to plan a large-scale implementation study across Canada.

Prior field studies and modelling analyses have individually highlighted the importance of age-specific and spatial heterogeneities on the risk for schistosomiasis in human populations. As long-term, large-scale drug treatment programs for schistosomiasis are initiated in subSaharan Africa and elsewhere, optimal strategies for timing and distribution of therapy have yet to be fully defined on the working, district-level scale, where strong heterogeneities are often observed among sublocations. Based on transmission estimates from recent field studies, we develop an extended model of heterogeneous schistosome transmission for distributed human and snail population clusters and age-dependent behaviour, based on a ‘mean worm burden+snail infection prevalence’ formulation. We analyse its equilibria and basic reproduction patterns and their dependence on the underlying transmission parameters. Our model allows the exploration of chemotherapy-based control strategies targeted at high-risk behavioural groups and localities, and the approach to an optimal design in terms of cost. Efficacy of the approach is demonstrated for a model environment having linked, but spatially-distributed, populations and transmission sites.
