Effects of Visiting Occupational Therapy on Cognition, Social Participation, and Daily Life of People With Disablities in the Community: Focusing on WHODAS 2.0

O cotidiano compreende as atividades laborais, de lazer, de estudo, de autocuidado, de convívio social, entre outras, sendo também considerado como uma perspectiva de análise e intervenção orientadora da prática do terapeuta ocupacional. A condição de deficiência pode, muitas vezes, afetar a realização dessas atividades. O artigo tem como objetivo apresentar as dificuldades e possibilidades de realização de atividades cotidianas de jovens com deficiência. Com base no método Photovoice, que prioriza o ponto de vista do pesquisado, quatro jovens realizaram fotografias de atividades, locais, ou objetos relevantes em seu dia-a-dia, em uma semana típica de suas rotinas. Uma segunda rodada de fotografias referentes aos obstáculos enfrentados cotidianamente foi realizada, e apenas dois jovens participaram. A análise das fotografias ocorreu em conjunto com os participantes, quando relataram individualmente para a pesquisadora o contexto e motivos de realização das fotos. No texto comparou-se os registros de dois jovens, identificando diferenças extremas em seus cotidianos. O relato e reflexão sobre as fotos foi considerado significativo para um dos participantes que afirmou ter gostado de realizar os registros fotográficos. Pelas imagens, registradas pode-se observar que um dos participantes tem menos oportunidades de participação social, uma vez que seus registros foram todos realizados dentro de sua casa. Ambos participantes registraram limitações voltadas sobretudo às condições de acessibilidade arquitetônica e geográfica, seja dentro de casa ou em espaços de uso coletivo, revelando a grande interferência desses impedimentos para sua vida cotidiana. Abstract The daily life is marked by work, leisure, study, self-care, social interation, also considered as a perspective and analysis of the occupational therapist's practice. The disability condition can often affect the performance of these activities. The article aims to present the difficulties and possibilities of carrying out daily activities of young people with disabilities. Based on the Photovoice method, which prioritizes from the subject's point of view, the youngsters took photographs of relevant activities, places, or objects in their daily life in a typical week of their routines. A second round of photographs regarding the obstacles faced daily was held, and only two young people participated.. The analysis of the photographs occurred together with the participants, when they individually reported to the researcher the context and reasons for the photos. In the text, the records of the two young people were compared, identifying extreme differences in their daily lives. The report and reflection on the photos was considered significant for one of the participants, who liked to carry out the photographic records. From the recorded images, it can be observed that one of the participants has fewer opportunities for social participation, since their records were all carried out inside their house. Both participants registered limitations mainly on the architectural and geographical accessibility conditions, either indoors or in spaces of collective use, revealing the great interference of these impediments to their daily life.Keywords: Daily life; Photography; Young with disabilities; Occupational therapy.

Introduction This study aimed to assess the impact of a holistic combination of Occupational Therapy and Social Skills Training on occupational performance, social participation, and clinical symptoms in people with schizophrenia. Method 60 people with schizophrenia were randomly assigned to two groups. One group received standardized Social Skills Training once a week for a total of 10 sessions, while the other group received a combination of Occupational Therapy and Social Skills Training once a week for a total of 16 sessions. Results A greater increase was determined in the scores of COPM total performance/satisfaction and the Community Integration Questionnaire in the Occupational Therapy and Social Skills Training group. Furthermore, these achievements were sustained in the Occupational Therapy and Social Skills Training group compared to the Social Skills Training group at the 6-month follow-up. Conclusion The clients received the combination of Occupational Therapy and Social Skills Training showed a better improvement compared to the Social Skills Training group in terms of occupational performance, social participation, and severity of clinical symptoms. The use of Occupational Therapy in a holistic approach in psychosocial rehabilitation of people with schizophrenia can increase their functionality and social participation. Further studies are needed to assess long-term effects of Occupational Therapy in schizophrenia.

The purpose of this study is to understand the functional status distribution and to explore the factors associated with changes in functional status and social participation in people with depression using two-year follow-up data. Subjects were selected from the Taiwan Databank of Persons with Disabilities (TDPD) if they had an evaluation date between July 2012 and 31 December 2017. We used data for 1138 individuals with multiple evaluation records and who were diagnosed with depression. The WHO Disability Assessment Schedule 2.0 (WHODAS 2.0) was the primary functional status measure. Other factors selected from the TDPD included social demographic data, living situation, employment status, economic status, and educational level. The results show scores in all dimensions of the WHODAS 2.0 declined over two years, especially in the domains of cognition, household activities, social participation, and total WHODAS 2.0 score. Aging groups showed poor recovery in cognition, getting along with others, and household activities. People living in suburban areas showed poorer recovery than people living in rural and urban areas in cognition, self-care, and general function (total score of WHODAS 2.0). Employment was also strongly associated with functional recovery in household activities, social participation, and general function. The original scores for cognition and getting along with others showed a significant negative relationship with social participation improvement. Our results can be used by policy makers to provide resources and conduct investigations, and by clinicians when making rehabilitation plans.

ResumoEsse relato foi feito na tentativa de ilustrar uma história de vida de uma mulher que vive em uma casa nas margens de rios e palafitas da cidade de Santos. Esta mesma mulher vive complexidades e possibilidades que foram enunciadas a partir dos encontros que ocorreram. As tramas vividas pela mulher ganharam (ainda mais) vida a partir dos momentos em que se deu voz aos acontecimentos, quando entrou em contato com suas próprias memórias e perguntas, com sua vida presente, com suas próprias palavras que ganharam tons, (múltiplas) vozes, espaços, escutas. Na tentativa de compor com seus dias, foram levados fragmentos literários para o despertar de encontros: com ela mesma, com novos afetos, com outros mundos. Os momentos vividos durante o processo foram subsidiados pelo método da Cartografia. Assim, este trabalho não se desdobra em constatações, afirmações e certezas, mas em sensações e perguntas lançadas nas entrepalavras que permeiam os (nossos) cotidianos.Palavras-Chave: Narrativas; Literatura; Encontro; Terapia Ocupacional; Práticas em Saúde. AbstractThis work was written in an attempt to illustrate a life story of a woman who lives in a house along the riverbanks and palafittes in the city of Santos.  This very woman faces complexities and possibilities which have been raised up in the meetings that have happened. The storylines this woman has experienced have gained (even more) life due to the moments when she gave voice to occurrences, when she got in contact with her own memories and questionings, with her current life, with her own words that emerged and gained tones, (multiple) voices, spaces, listening. In order to compose her daily life, literary passages were used to awake meetings: with herself, with new feelings of affection, with other worlds. The moments experienced during the process were subsidized by the Cartography method. Therefore, this work does not unfold in findings, statements and certainties, but instead in sensations, doubts and questions released in wordings that pervade (our) everyday lives.Keywords: Narratives; Literature; Meeting; Occupational Therapy; Health Practices.

Occupational therapy has tremendous, unfulfilled potential. The lecture critically analyzes that potential in light of occupational therapy's social vision. The analysis is built on emerging views of occupational therapy's core philosophy of client-centred practice using the ordinary occupations of daily life as a form of therapy. Critical analysis shows that foundational features of occupational therapy are consistent with foundational features of social justice. However, analysis also shows that occupational therapy's social vision is narrowed to comply with dominant community, managerial and medical approaches to disability and aging. Given therapists' good intentions, critical analysis is unnerving, but it provides a guide for transforming practice and developing occupational therapy's potential.
