Introduction The polycystic ovary syndrome is primarily characterized by an association of hyperandrogenism and anovulation. The aim was to determine the incidence of menstrual cycle disorders in patients with PCO syndrome and the relation with ultrasonographic and hormonal findings. Material and methods A retrospective study examined the ultrasonographic findings and hormonal status in patients with polycystic ovary syndrome at the Department of Obstetrics and Gynecology of the General Hospital in Subotica. The sample comprised 39 patients. Results and conclusions Most frequently age at menarche was 14 years (29%) in obese and 10 years (45.5%) in normal weight patients. There was not a significant difference in duration of menstrual bleeding in both groups of examined patients, whereas amenorrhoea was significantly more frequent in obese patients (29.4%) than in patients with normal weight (4.7%). On the basis of presented results (increased percentage of severe disorders of menstrual cycle in all patients in subgroup C and increased percentage of menstrual disorders in patients with high stromal density in relation to those with normal echogenity of stroma) it can be concluded that there is a correlation between ultrasonographic findings of ovaries (number of microcysts, volume and density of the stroma) and menstrual cycle disorders, the polycystic ovary syndrome.

Abstract Background Polycystic ovary syndrome (PCOS) is a common reproductive endocrine disease in women. Insulin resistance (IR) has emerged as a central contributor to the pathogenesis of this disease. According to traditional Chinese medicine (TCM), kidney deficiency is the main syndrome of PCOS. The deficiency of the kidney cannot vaporize water-dampness, and the retention of water-dampness accumulates into phlegm dampness stagnation, resulting in visceral dysfunction and metabolic disorder. TCM involving syndrome differentiation and treatment is widely used to adjust women’s menstrual cycles. Our patented formula Bushen Huatan Decoction (BSHTD) has been proven to be effective in the clinical treatment of IR-PCOS. Baduanjin also plays an important role in improving metabolic syndrome through lifestyle intervention. This study investigates the clinical efficacy of Bushen Huatan Decoction combined with Baduanjin in IR-PCOS, to form a specific TCM-behaviour intervention plan in the treatment of IR-PCOS. Methods/design This is a randomized controlled trial involving 190 participants diagnosed with IR-PCOS. All participants will be randomly allocated into 5 groups: group A will receive metformin; group B, BSHTD; group C, Baduanjin; group D, BSHTD combined with metformin; and group E, BSHTD combined with Baduanjin. One course of treatment lasts 3 months, a total of two courses. The primary outcomes are changes in the homeostatic model assessment of insulin resistance (HOMA-IR) and improvements in the oral glucose tolerance test (OGTT) and insulin-releasing test (INS). The secondary outcomes are improvements in the menstrual cycle, ovulation rate, clinical pregnancy rate, basic serum sex hormone levels, free androgen index (FAI), Ferriman-Gallwey scores, body mass index (BMI) and TCM syndrome scores. The related observation indexes will be collected at baseline, during the process of treatment and at the 6-month follow-up. Simultaneously, close monitoring of possible adverse events will be performed throughout the trial process. Discussion This trial will investigate the efficacy of the comprehensive intervention program of Bushen Huatan Decoction combined with Baduanjin on the adjustment of the menstrual cycle, improvement of insulin resistance and correction of glucose metabolism disorder in IR-PCOS patients. It is expected to form an alternative treatment of TCM-behaviour intervention therapy for IR-PCOS and promote the Chinese fitness Qigong Baduanjin in the application of lifestyle diseases. ABSTRACT Objectives The objectives of the study were to find out the clinical features, biochemical and hormonal profile of patients with polycystic ovary syndrome (PCOS) in Bangladesh. Material and Methods A case control and cross-sectional study was undertaken among 78 PCOS patients attending infertility clinic and 33 controls at BIRDEM Hospital, Dhaka. Inclusion criteria for cases were oligo/amenorrhea, transvaginal sonography suggesting PCO and/ or features of hyperandrogenemia with exclusion of other causes. Controls were women with regular menstrual cycle. Results Age and height of cases and controls were similar. BMI >25 was 67% among cases and 19% among controls (P<0.001); waist hip ratio >0.8 was 64% among cases and 29% among controls (P<0.001). Mean BMI of cases was 28.2 + 4.5 and that of cases was 21.05 + 4.1; mean fasting glucose among cases was 5.93 + 1.08 and among controls was 4.4 + 1.11 mmol/L (P<0.01); mean fasting serum insulin level was 32.15+ 12.13 among cases was 11.32 +10.02 ìU/ml among controls(P<.001); insulin resistance (fasting HOMA-IR>6.8) was 42.32% in cases and 12% in control (P<.001). Patients with PCOS had following clinical and biochemical parameters: oligomenorrhea- 74%, amenorrhea- 26%, mean Ferriman-Galaway score -19.89 + 5.06. At day 3 of menstrual cycle meanserum LH was 12.79+7.1 mmol/L, serum FSH was 5.23 + 2.5 miu/ml and serum prolactin was 415.15+180.5 mmol/L; 30% had biochemical hyperandrogenemia. Conclusions PCOS patient in Bangladesh are usually overweight, hirsute (grade I and II), hyperandrogenemic, insulin resitant and have altered LH to FSH ratio.
