Introduction The polycystic ovary syndrome is primarily characterized by an association of hyperandrogenism and anovulation. The aim was to determine the incidence of menstrual cycle disorders in patients with PCO syndrome and the relation with ultrasonographic and hormonal findings. Material and methods A retrospective study examined the ultrasonographic findings and hormonal status in patients with polycystic ovary syndrome at the Department of Obstetrics and Gynecology of the General Hospital in Subotica. The sample comprised 39 patients. Results and conclusions Most frequently age at menarche was 14 years (29%) in obese and 10 years (45.5%) in normal weight patients. There was not a significant difference in duration of menstrual bleeding in both groups of examined patients, whereas amenorrhoea was significantly more frequent in obese patients (29.4%) than in patients with normal weight (4.7%). On the basis of presented results (increased percentage of severe disorders of menstrual cycle in all patients in subgroup C and increased percentage of menstrual disorders in patients with high stromal density in relation to those with normal echogenity of stroma) it can be concluded that there is a correlation between ultrasonographic findings of ovaries (number of microcysts, volume and density of the stroma) and menstrual cycle disorders, the polycystic ovary syndrome.

Polycystic ovary syndrome is a common cause of an ovulatory infertility. Drugs like Aromatase inhibitors, Human menopausal gonadotropin, used for ovulation induction. The aim of this study was carried out to compare the therapeutic effects of gonadotropin hormone versus oral ovarian stimulating agents. A prospective randomized controlled clinical trial was carried out in the Salahdeen general hospital in Tikrit from 1st Feb-30th August 2020. About 75 PCOs patients enrolled randomly in the study and divided equally into 3 groups as below: Group A treated with (75 IU intramuscular HMG gonadotropin) daily for 5 days starting Day 2 of menstrual cycle. Group B treated with oral clomiphene citrate 100 mg daily for 5 days starting Day 2 of menstrual cycle. Group C treated with oral Letrezole 5 mg daily for 5 days starting Day 2 of menstrual cycle. Multiple mature follicles were obtained commonly by HMG, followed by Letrozole, then Clomiphene, this relation was statistically significant. Endometrial thickness was higher among those treated with HMG (10.5±1.7) than those treated by Clomiphene (9.03±0.9), and then treated by letrozole (8.5±1.2). This is a significant difference in ET value between Clomiphene, letrozole, and HMG. Chemical pregnancy (early pregnancy loss that occurs shortly after implantation may account to 50-75% of all miscarriages) was higher among those treated with HMG (20%), while it was (16%) of those treated with Clomiphene, and (12%) of the Letrozole group, this relation was statistically not significant. In conclusion, HMG had the highest response rate, followed by Letrozole, and Clomiphene. The multiple mature follicles was obtained commonly by HMG, followed by Letrozole, then Clomiphene.

Introduction Polycystic ovary syndrome is commonly associated with hyperandrogenism and anovulation. The aim of this study was to investigate the impact of obesity on hormonal status in patients with polycystic ovary syndrome. Material and methods The study was performed at the Ward of Obstetrics and Gynecology of the General Hospital in Subotica. A retrospective investigation comprised 39 patients with polycystic ovary syndrome . All patients were in the fertile age-range: 18-38 years. Following ultrasonographic examination and anamnestic data, patients underwent hormonal analyses of follicle-stimulating hormone (FSH), luteinizing hormone (LH), testosterone, prolactin and insulin obtained from the pooled serum sample. Results Values of testosterone and insulin in the group of obese patients with polycystic ovary syndrome were significantly higher than in normal weight patients. Patients with normal body weight index had significantly increased levels of LH in regard to those with increased body weight index. Values of FSH, prolactin and LH/FSH ratio were not significantly different in both groups of polycystic ovary syndrome patients. Increased values of insulin were recorded in 43% of obese and 18.2% of normal weight patients. Conclusion Analysis of investigated results confirmed that obese patients with polycystic ovary syndrome and insulin resistance have been a special clinical entity, whereas an open question remains whether obesity is directly connected with polycystic ovary syndrome or it is only an additional factor interfering with metabolic and hormonal status of genetically predisposed and phenotypically indoctrinated women with polycystic ovary syndrome.

AbstractPolycystic ovary syndrome (PCOS) is the most common reproductive endocrine disorder in pre-menopausal women having complex pathophysiology. Several candidate genes have been shown to have association with PCOS. CYP19 gene encodes a key steroidogenic enzyme involved in conversion of androgens into estrogens. Previous studies have reported contradictory results with regard to association of SNP rs2414096 in CYP19 gene with PCOS and hyperandrogenism in different ethnic populations. Present study was aimed to investigate the impact of SNP rs2414096 polymorphism of CYP19 gene on susceptibility of PCOS and hyperandrogenism in Kashmiri women. Further we also studied the genotypic-phenotypic association for various clinical and biochemical parameters of this polymorphism. Case control study. 394 PCOS cases diagnosed on the basis of Rotterdam criteria and age matched 306 healthy women. We found a significant differences in genotypic frequency (χ2 = 18.91, p < 0.05) as well as allele frequency (OR 0.63, CI 0.51–0.78, χ2 = 17.66, p < 0.05) between PCOS women and controls. The genotype–phenotype correlation analysis showed a significant difference in FG score (p = 0.047) and alopecia (p = 0.045) between the three genotypes. Also, the androgen excess markers like DHEAS (p < 0.001), Androstenedione (p < 0.001), Testosterone (p < 0.001) and FAI (p = 0.005) were significantly elevated in GG genotype and showed a significant difference in additive model in PCOS women. rs2414096 polymorphism of CYP19 gene is associated with the risk of PCOS as well as with clinical and biochemical markers of hyperandrogenism, hence suggesting its role in clinical manifestations of PCOS in Kashmiri women.

Abstract Background Polycystic ovary syndrome (PCOS) is a common reproductive endocrine disease in women. Insulin resistance (IR) has emerged as a central contributor to the pathogenesis of this disease. According to traditional Chinese medicine (TCM), kidney deficiency is the main syndrome of PCOS. The deficiency of the kidney cannot vaporize water-dampness, and the retention of water-dampness accumulates into phlegm dampness stagnation, resulting in visceral dysfunction and metabolic disorder. TCM involving syndrome differentiation and treatment is widely used to adjust women’s menstrual cycles. Our patented formula Bushen Huatan Decoction (BSHTD) has been proven to be effective in the clinical treatment of IR-PCOS. Baduanjin also plays an important role in improving metabolic syndrome through lifestyle intervention. This study investigates the clinical efficacy of Bushen Huatan Decoction combined with Baduanjin in IR-PCOS, to form a specific TCM-behaviour intervention plan in the treatment of IR-PCOS. Methods/design This is a randomized controlled trial involving 190 participants diagnosed with IR-PCOS. All participants will be randomly allocated into 5 groups: group A will receive metformin; group B, BSHTD; group C, Baduanjin; group D, BSHTD combined with metformin; and group E, BSHTD combined with Baduanjin. One course of treatment lasts 3 months, a total of two courses. The primary outcomes are changes in the homeostatic model assessment of insulin resistance (HOMA-IR) and improvements in the oral glucose tolerance test (OGTT) and insulin-releasing test (INS). The secondary outcomes are improvements in the menstrual cycle, ovulation rate, clinical pregnancy rate, basic serum sex hormone levels, free androgen index (FAI), Ferriman-Gallwey scores, body mass index (BMI) and TCM syndrome scores. The related observation indexes will be collected at baseline, during the process of treatment and at the 6-month follow-up. Simultaneously, close monitoring of possible adverse events will be performed throughout the trial process. Discussion This trial will investigate the efficacy of the comprehensive intervention program of Bushen Huatan Decoction combined with Baduanjin on the adjustment of the menstrual cycle, improvement of insulin resistance and correction of glucose metabolism disorder in IR-PCOS patients. It is expected to form an alternative treatment of TCM-behaviour intervention therapy for IR-PCOS and promote the Chinese fitness Qigong Baduanjin in the application of lifestyle diseases. Abstract Context Increased aldo-keto reductase 1C3 (AKR1C3)-mediated conversion of androstenedione (A4) to testosterone (T) promotes lipid storage in subcutaneous (SC) abdominal adipose in overweight/obese polycystic ovary syndrome (PCOS) women. Objective To examine whether an elevated serum T/A4 ratio, as a marker of enhanced AKR1C3 activity in SC abdominal adipose, predicts metabolic function in normal-weight PCOS women. Design Prospective cohort study. Setting Academic center. Patients Nineteen normal-weight PCOS women; 21 age- and body mass index-matched controls. Intervention(s) Circulating hormone/metabolic determinations, intravenous glucose tolerance testing, total body dual-energy x-ray absorptiometry, SC abdominal fat biopsy. Main Outcome Measure(s) Serum T/A4 ratios, hormone/metabolic measures and AKR1C3 expression of adipocytes matured in vitro were compared between female types; serum T/A4 ratios were correlated with serum lipids, adipose insulin resistance (adipose-IR), homeostatic model assessment of insulin resistance (HOMA-IR) and insulin sensitivity (Si). Results Increased serum T/A4 ratios (P=0.040) and log adipose-IR values (P=0.002) in PCOS women versus controls were accompanied by AKR1C3 mRNA overexpression of PCOS adipocytes matured in vitro (P=0.016). Serum T/A4 ratios in PCOS women, but not controls, negatively correlated with log triglycerides (TG: R=-0.65, P=0.002) and the TG index (R=-0.57, P=0.011). Adjusting for serum free T, serum T/A4 ratios in PCOS women remained negatively correlated with log TG (R=-0.57, P=0.013) and TG index (R=-0.50, P=0.036), respectively, without significant relationships with other metabolic measures. Conclusion An elevated serum T/A4 ratio, as a marker of enhanced AKR1C3 activity in SC abdominal adipose, predicts healthy metabolic function in normal-weight PCOS women.

AbstractMetformin and Thiazolidinediones are insulin sensitizers used as the first-line treatment of PCOS, which are recently being examined for their extensive beneficial impacts at biochemical and molecular level of body function in Polycystic ovary syndrome. Therefore, purpose of this review is to compare the effectiveness of Metformin and Thiazolidinediones on vascular endothelial function in PCOS. MEDLINE (1966- October 2018), CINAHL (1982 to October 2018), Cochrane Central Register of Controlled Trials and Cochrane Menstrual Disorders and Subfertility Group trials register (October 2018) and Google Scholar were searched electronically without language restriction. Three Randomized controlled trials (RCTs) were selected with a total of 90 participants comparing effect of Metformin (Intervention) to Thiazolidinediones (Comparator) with an outcome on endothelial function among women with PCOS. In results, Meta-analysis shows no statistically significant difference on endothelial-dependent vasodilation between Metformin and Thiazolidinedione (p value= 0.59 at 95% CI - 1.31, 0.74).

The objective of this study is to explore how hyperinsulinemia and insulin resistance relate to the clinical, endocrine and metabolic factors in the infertile women with polycystic ovary syndrome. This study was conducted on 121 consecutive infertile women with polycystic ovary syndrome attending the Infertility unit from January 2017 to December 2017. They were divided into two groups: insulin resistant and insulin sensitive. There was significant difference in body mass index and waist circumference between the two groups. Serum lipids were not associated with insulin resistance. Hyperinsulinemia was significantly associated with metabolic syndrome. Reducing body mass index and waist circumference may improve insulin resistance in infertile women with polycystic ovary syndrome. Screening the infertile women with polycystic ovary syndrome for hyperinsulinemia and insulin resistance and subsequent counseling is recommended to address the long-term risks of metabolic syndrome.
