Mindfulness and meditation: treating cognitive impairment and reducing stress in dementia

Abstract This study investigates the relationship between mindfulness, meditation, cognition and stress in people with Alzheimer’s disease (AD), dementia, mild cognitive impairment and subjective cognitive decline. Accordingly, we explore how the use of meditation as a behavioural intervention can reduce stress and enhance cognition, which in turn ameliorates some dementia symptoms. A narrative review of the literature was conducted with any studies using meditation as an intervention for dementia or dementia-related memory conditions meeting inclusion criteria. Studies where moving meditation was the main intervention were excluded due to the possible confounding of exercise. Ten papers were identified and reviewed. There was a broad use of measures across all studies, with cognitive assessment, quality of life and perceived stress being the most common. Three studies used functional magnetic resonance imaging to measure functional changes to brain regions during meditation. The interventions fell into the following three categories: mindfulness, most commonly mindfulness-based stress reduction (six studies); Kirtan Kriya meditation (three studies); and mindfulness-based Alzheimer’s stimulation (one study). Three of these studies were randomised controlled trials. All studies reported significant findings or trends towards significance in a broad range of measures, including a reduction of cognitive decline, reduction in perceived stress, increase in quality of life, as well as increases in functional connectivity, percent volume brain change and cerebral blood flow in areas of the cortex. Limitations and directions for future studies on meditation-based treatment for AD and stress management are suggested.

AbstractCognitive complaints in the absence of objective cognitive impairment, observed in patients with subjective cognitive decline (SCD), are common in old age. The first step to postpone cognitive decline is to use techniques known to improve cognition, i.e., cognitive enhancement techniques.We aimed to provide clinical recommendations to improve cognitive performance in cognitively unimpaired individuals, by using cognitive, mental, or physical training (CMPT), non-invasive brain stimulations (NIBS), drugs, or nutrients. We made a systematic review of CMPT studies based on the GRADE method rating the strength of evidence.CMPT have clinically relevant effects on cognitive and non-cognitive outcomes. The quality of evidence supporting the improvement of outcomes following a CMPT was high for metamemory; moderate for executive functions, attention, global cognition, and generalization in daily life; and low for objective memory, subjective memory, motivation, mood, and quality of life, as well as a transfer to other cognitive functions. Regarding specific interventions, CMPT based on repeated practice (e.g., video games or mindfulness, but not physical training) improved attention and executive functions significantly, while CMPT based on strategic learning significantly improved objective memory.We found encouraging evidence supporting the potential effect of NIBS in improving memory performance, and reducing the perception of self-perceived memory decline in SCD. Yet, the high heterogeneity of stimulation protocols in the different studies prevent the issuing of clear-cut recommendations for implementation in a clinical setting. No conclusive argument was found to recommend any of the main pharmacological cognitive enhancement drugs (“smart drugs”, acetylcholinesterase inhibitors, memantine, antidepressant) or herbal extracts (Panax ginseng, Gingko biloba, and Bacopa monnieri) in people without cognitive impairment.Altogether, this systematic review provides evidence for CMPT to improve cognition, encouraging results for NIBS although more studies are needed, while it does not support the use of drugs or nutrients.

Prevalence studies have noted the cooccurrence of cognitive decline and depression in persons with heart failure. Cognitive impairment is associated with significant mortality and deteriorated quality of life, likely due to impairments in memory and executive function, which impact a patient’s ability to understand and comply with prescribed treatment plans. This is especially true in complex diseases such as heart failure. Evidence from literature supports the possibility of a pathophysiological relationship between cognitive impairment, depression, and heart failure. Yet, very few studies have sought to investigate this relationship. This paper reviews current literature on the association between depression and cognitive impairment in persons with heart failure and explores possible mechanisms explaining this complex triad.

Adults over the age of 65 years are now the fastest growing segment of the Canadian population. Although individual differences exist, aging is commonly associated with impairments in executive function. The present pilot study investigated the effectiveness of a mindfulness-based stress reduction (MBSR) program on improving executive functioning in older adults, while additionally decreasing perceived stress and improving psychological wellbeing. It was hypothesized that compared with an active control group, the MBSR group would display significant improvements in executive functioning, mood, self-esteem, and perceived stress. Participants were randomly assigned to either MBSR (n=40) or an active control group (n=32). Results suggest that MBSR is associated with significantly enhanced quality of life compared with the control condition. No additional significant results were found. A full-scale study will be necessary to clarify the present results.

Approximately 14.9% of Canadians aged 65 and older are living with cognitive impairment, including dementia, which means that there is likely an equal or even greater number of Canadians involved in their care. Relative to non-caregivers, caregivers of persons with dementia typically show more cognitive difficulties (e.g., executive function, memory), and report increased perceived stress, depression, and poor quality of life. The first aim of this dissertation was to compare caregivers’ cognitive and psychological function with that of non-caregiving older adults. The second aim was to assess whether a standardized mindfulness-based stress reduction (MBSR) intervention could improve cognitive function and well-being in caregivers, compared to a psychoeducational control group. In Study 1, 57 older adult caregivers completed an assessment of cognitive and psychosocial functioning. Their performance was compared with that of 97 healthy, non-caregiving older adults who completed a similar battery of measures. It was hypothesized that caregivers would show decreased performance on measures of cognitive function, as well as significantly poorer psychosocial outcomes relative to the non-caregiver group. In Study 2, caregivers were randomized into an eight week MBSR program (n = 33) or a psychoeducation control group (n = 24). At post-program, all participants completed a re-assessment of cognitive and psychosocial functioning. Results of Study 1 showed that relative to non-caregivers, caregivers made significantly more errors on a measure of cognitive flexibility, generated fewer words on measures of phonemic fluency and semantic fluency, and learned significantly fewer words on a list learning task. Caregivers also reported significantly more stress and depressive symptoms, and reported lower quality of life. Results of Study 2 showed that post-intervention cognitive performance did not significantly differ as a function of group assignment. However, participants in the MBSR group showed a significantly larger reduction in depression and perceived stress relative to controls. Results of this RCT provide preliminary evidence for the selective benefits of MBSR relative to psychoeducation in a group of older caregivers of individuals with dementia.

IntroductionCommunity-dwelling older adults living with subjective cognitive decline or mild cognitive impairment may experience decreased efficiency in their overall functional performance. This decreased cognitive efficiency may result in anxiety, low mood, perceived stress and decreased emotional well-being and quality-of-life. These psychological symptoms may further exacerbate cognitive decline.Exploring non-pharmacological interventions such as mindfulness within primary care is vital in enabling individuals to develop strategies to manage cognitive impairment or psychological symptoms. Mindfulness-based stress reduction (MBSR) is an 8-week programme that is beneficial in alleviating psychological symptoms; however, its impact on perceived satisfaction on overall functional performance with this population has not been evaluated. The primary objective of this study is to explore the feasibility of conducting a randomised controlled trial of an occupational therapist-led MBSR programme within primary care.MethodsConvergent mixed-methods, randomised control feasibility trial with 40 participants from an interprofessional primary care team in Toronto, Ontario. Participants are randomised into the 8-week MBSR group or wait-list control will be compared at baseline, postintervention and 4weeks follow-up. The primary aim is to determine the feasibility of the intervention with this population and setting. The secondary aim is to examine perceived satisfaction with functional performance as measured by the Canadian Occupational Performance Measure. Secondary clinical outcomes include psychological symptoms.AnalysisInvestigators will analyse the quantitative and qualitative data strands separately. Descriptive statistics, focus group and interviews will then be merged and further analysed to best understand the feasibility and preliminary clinical outcomes from the study.Ethics and disseminationThe study is approved by Women’s College Hospital (2017–0056-E), and Queen’s University, Kingston, Ontario (6026418). The study will follow Standard Protocol Items: Recommendations for Interventional Trials.
