The clinical course of hearing loss associated with retinitis pigmentosa is outlined in four brief case summaries. This incidence of hearing loss in retinitis pigmentosa is 10% and occurs several years before clinical visual abnormalities. A battery of audiometric tests all suggest that the hearing defect is within the cochlea and that it is not rapidly progressive. Onset age has not been established. The employment of more than routine hearing tests, testing of other family members, electroretinograms, and historical information about certain features of familial vision are useful in establishing early diagnosis.

To ensure optimal auditory development for the acquisition of spoken language, children with hearing loss require early diagnosis, effective ongoing audiological management, well fit and maintained hearing technology, and appropriate family-centered early intervention. When these elements are in place, children with hearing loss can achieve developmental and communicative outcomes that are comparable to their hearing peers. However, for these outcomes to occur, clinicians—early interventionists, speech-language pathologists, and pediatric audiologists—must participate in a dynamic process that requires careful monitoring of countless variables that could impact the child's skill acquisition. This paper addresses some of these variables or “red flags,” which often are indicators of both minor and major issues that clinicians may encounter when delivering services to young children with hearing loss and their families.

Background:Muckle-Wells syndrome (MWS) is a monogenic autoinflammatory disease caused by a NLRP3 gene mutation. It is the most common variant of cryopyrin-associated periodic syndromes (CAPSs) and can be observed in rheumatology practice. It manifests itself in fever, urticaria-like rash, arthralgias/arthritides, conjunctivitis/uveitis, sensorineural hearing loss, acute-phase markers (ESR, CRP). The disease’s onset usually takes place in infancy. There are examples of family cases. Targeted therapy: interleukin-1 inhibitors (anakinra, canakinumab).Objectives:to provide characteristics of MWS patients, family cases in the rheumatology practice of the Federal Rheumatology Center in Russia.Methods:in a 10-year period (2009 to 2019), MWS was diagnosed in 42 outpatient and inpatient patients, among them were 24 children, 18 adults, and 9 family cases. All of them underwent a standard rheumatology examination, including a ECR, CRP, ophthalmologist examination, and an audiogram. A molecular genetic test of the NLRP3 gene was carried out for all patients, the diagnosis was confirmed in all of them.Results:Out of 18 adult patients aged between 19 and 59 years, women were prevalent (16 to 2), the onset age was 0 to 53 years, in 88,9% cases the onset took place before a patient was 18 years old. When diagnosed, the disease duration varied from 6 to 46 years. Most patients demonstrated fever, urticarial-like rash, arthralgias/arthritides, which were observed in 16 patients (88.9%), conjunctivitides were observed in 15 patients (83,3%), sensorineural hearing loss – in 8 patients (44,4%), abdominal pain, nausea, vomiting – in 4 patients, headache, dizziness – in 6 patients. There also were rare manifestations, such as: recurrent oral ulcers (8), genital ulcers (3), erythema nodosum (3), sore throat and raids on the tonsils (PFAPA-like phenotype) was observed in 2 patients. In 3 patients manifestations were triggered by cold temperature. All patients had an increased ESR and C-reactive protein concentration. Eight family cases of MWS were identified (in total 26 family members aged between 2.5 and 62 years) with a number of affected in one family ranging from 2 to 6 people of different age (8 children, 18 adults, out of which 20 were female, and 6 were male). Most patients had fever (17), urticarial-like rash (18), conjunctivitides (12), oral ulcers (7), articular syndrome (14), sensorineural hearing loss (5), and 2 patients died of renal insufficiency (probably due to amyloidosis of the kidneys). The heterozygous mutations in NLRP3 have been identified in pts: T348M (3 families), R262W (2 families), A439V (1), V198M (1), Pro294Ser (1). Ten patients received canakinumab for a period of 6 months to 6.5 years, and 5 patients received anakinra before canakinumab.Conclusion:MWS is an orphan autoinflammatory disease, however it sometimes can be observed in rheumatology practice. It is very important to acquire family medical history to identify affected family members and prescribe therapy in a timely manner. IL-1 inhibitors are an effective and safe treatment option for MWS patients.Disclosure of Interests:None declared

Objective: This project aims to look at the Audiovestibular Physician’s practice at Salford and how closely it aligns with the gold standard guidelines set in the protocol lately published by the British Association of Audiological Physicians. Method: An audit was done retrospectively on 20 patients suffering from sensorineural hearing loss. As such, patient notes were utilised to ascertain which aetiological investigations have been completed and which were not. Any inadequacy in the aetiological work up has been dissected to help know the underlying reasons. Results: All patients had a thorough history taken and were comprehensively physically examined. 95% of patients underwent imaging in the form of MRI/CT. 80% received CMV testing. 75% underwent ECG testing. 60% received family hearing tests. Only 35% had ophthalmology examinations and 25% underwent urine and genetic testing. Conclusion: In some cases, the low compliance rates were due to the Audiovestibular Physician not ordering the investigation as part of the aetiological work up. This could be improved with the use of a dedicated checklist to act as an aid to the physician. Moreover, genetic counselling has been proposed to attempt to boost the compliance rates with genetic testing and similarly, leaflets briefing patients’ families about the importance of undergoing hearing tests themselves is another promising proposition to help improve the adherence

In this lecture information on the etiology, pathogenesis, and clinical course of chronic inflammation of the middle ear is systematized. The authors focus on the nature of the manifestations of otitis media, depending on the size and location of the perforation of the eardrum. Based on personal experience and accumulated knowledge, in detail reveal the issue of cholesteatoma, its precursors and possible complications. A lot of attention is paid to methods of examination and interpretation of audiometric and radiological control data. Speaking about the chronic course of the ear disease, the article describes frequently encountered situations such as tympanosclerosis, fistula, etc. In addition to their own point of view on the problem, the authors present literature data reflecting the views of other otorhinolaryngologists. Alzheimer’s disease (AD) is the most common cause of dementia. AD progression leads to a significant deterioration in cognitive function, resulting in a significant impact not only on patient quality of life but also on the quality of life of family members/care-givers. Some physicians do not feel that diagnosing AD is necessary due to the absence of any cure and the stress related to diagnosis. Nevertheless, it is important to diagnose AD as early as possible. Early diagnosis of AD allows the patient to be more involved in treatment planning, since at this stage the patient’s cognitive function will be near normal. Many forms of the disease do not progress rapidly, and early diagnosis and treatment will allow a good quality of life for the patient, family members and care-givers. The general practitioner has a special role in detecting and following up patients diagnosed with AD.

During the early twentieth century, otologists began collaborating with organizers of the New York League for the Hard of Hearing to build a bridge to “adjust the economic ratio” of deafness and create new research avenues for alleviating or curing hearing loss. This collegiality not only defined the medical discourse surrounding hearing impairment, anchoring it in hearing tests and hearing aid prescription, but, in so doing, solidified the notion that deafness was a “problem” in dire need of a “solution.” Public health campaigns thus became pivotal for spreading this message on local and national levels. This paper focuses on how, from the 1920s to 1950s, as otologists became more involved with social projects for the deaf and hard of hearing — advocating lip-reading, community work, and welfare programs — at the same time, they also mandated for greater therapeutic regulation, control of hearing aid distribution, and standardization of hearing tests. The seemingly paradoxical nature of their roles continued to reinforce the stigmatization of deafness: with widespread availability of effective help, the hearing impaired were expected to seek out therapeutic or technological measures rather than live with their affliction.

As a result of examination and questionnaire of 143 patients who applied for orthopedic treatment of defects of teeth and dental rows, it was found that signs of dysfunction of temporomandibular joint with preserved dental rows are diagnosed in 36—55% cases, and with defects of dental rows — in 45—90% cases. The absence of eighth teeth in the dental row does not significantly change the functional state of the dental-jaw system. A statistically significant frequency of signs of EHS dysfunction has been identified among individuals having terminal dentition defects.In patients with terminal dentition defects, each 3rd patient is diagnosed with cochleovestibular syndrome, and in each 2nd, sounds are determined in the area of VNHS when the lower jaw moves. It has been found that if 15—13 and 12—11 pairs of antagonist teeth are preserved, the signs of dysfunction are determined in 55—45% cases, if the number of teeth having antagonists is reduced to 10—5 (in 90.0% these are patients with preserved 7—8 pairs of antagonist teeth), then the frequency of dysfunction signs increases to 75.0% (t1-3=1.33; t2-3=2.00), in such patients significantly more often — in 75.0% of cases, mandibular deviation is diagnosed when opening and closing the mouth than in persons with a large number of preserved antagonist teeth, respectively 55.0% (t=2.66) and 45.0% (t=3.93) in 1 and 2 groups. Thus, the identification of such signs as crunching, clicking in the joints, hearing loss or tinnitus, suggests the presence of intra-articular disorders in such patients. The deviation of the jaw from its main trajectory when opening the mouth indicates the possible involvement of the masticators muscles in the pathological process. All this requires the dentist to carry out early diagnosis and timely orthopedic treatment.
