ABSTRACT This clinical report describes the treatment of a patient dissatisfied with the esthetics of the smile due to the presence of narrow teeth, yellow and the presence of diastema between teeth 11 and 21. Clinical examination revealed absence of the disocclusion guides. The treatment plan consisted in obtaining study casts, assembly in a semi-adjustable articulator and diagnostic waxing, to evaluate the possibilities of a restorative treatment. After the approved the viability of the procedure, dental bleaching was started in-office with 38% hydrogen peroxide associated with at-home bleaching with 15% carbamide peroxide, in order to reduce the yellowing of the teeth. Afterwards, the functional evaluation of the restorative procedure was carried out through the prior preparation of the mock-up. After functional approval of the mock-up, lip frenectomy was performed, since the frenulum is the cause of the diastema between the teeth 11 and 21. After 4 weeks was carried out the restorative procedure of teeth 13 to 23, with direct composite resin, improving the aesthetics of the smile and restoring the disocclusion guides.

Recovery of worn tooth surfaces using composite resin has been increasingly accepted by clinicians, since rehabilitation through additive techniques aims at minimal or no intervention and preserves healthy dental structure, ensuring longevity to the treatment. Adhesive dentistry can afford promising results through a correct diagnosis and treatment plan. Planning can offer not only dental rehabilitation, but also controlling cause of the wear process. Strategies that seek to prevent the progression of muscle activities such as bruxism, should be established. This article aims to present a clinical case report of a rehabilitation treatment planned and executed through the digital flow, in which the patient presents wear and tear caused by the habit of bruxism. The rehabilitation of the anterior upper teeth was carried out in direct composite resin, associated with the preparation and installation of an occlusal splint, used before, during and after the restorative treatment, ensuring the reestablishment of the shape, function and dental aesthetics.

Keeping in mind the final result is the basis of any type of treatment, especially those in which the morphology, size and proportion of the anterior teeth will be changed. This is where a good treatment plan based on a diagnostic wax-up tested with a mock-up and approved by the patient becomes crucial. This case report aims to exemplify how to transfer the diagnostic information to the patient’s mouth and direct it not only to the restorative dentistry, but also to the surgeon when performing the crown length. Diagnostic waxing was performed by the laboratory technician, obeying anterior posterior incisal and gingival criteria and curvatures, which were transported to an aesthetic guide through mock up to mark the surgical points. Then, surgery to increase the clinical crown with bone access and after healing, direct venners in composite resin also guided by waxing. In conclusion, a workflow can be established using the wax-up / mock-up that serves as a guide for the periodontist in the approach to surgical lengthening of the crown and for the rehabilitator who uses it to produce changes in the shape of dental dimensions .

PURPOSE: This study evaluated the effect of 10% sodium ascorbate (10SA), in gel (10SAg) or aqueous solution (10SAs) formulations, on fracture resistance of endodontically treated tooth submitted to dental bleaching procedures with 15% hydrogen peroxide associated with titanium dioxide (15HP-TiO2) nanoparticles and photoactivated by LED-laser. MATERIAL AND METHODS: Forty maxillary premolars were endodontically-treated and embedded in acrylic resin up to the cement-enamel junction. The specimens were divided into four groups (n=10): G1 (negative control): no bleaching, coronal access restored with composite resin; G2 (positive control): three dental bleaching  sessions using 15HP-TiO2 and LED-laser photoactivation and restored with composite resin (positive control); G3 (10SAg): similar procedures to G2, but applied  10SA, in gel formulation, for 24 hours before restoration; G4 (10SAs): similar procedures to G3, but applied  10SA, in aqueous solution formulation.  The 15HP-TiO2 was applied on buccal and lingual surfaces of the crown tooth and inside the pulp chamber and photoactivated by LED-laser. Between each bleaching session, the teeth were maintained in artificial saliva, at 37oC, for 7 days. In sequence, the teeth were submitted to fracture resistance testing using an eletromechanical machine test. The data was analyzed using Kruskal Wallis test (p = 0.05) RESULTS: There are no differences significant among the groups in relation to fracture resistance of endodontically treated teeth (p>0.05). CONCLUSIONS: The use of 10% sodium ascorbate, in gel or aqueous solution formulations, did not interfered on the fracture resistance teeth after dental bleaching using 15HP-TiO2 and LED-laser photoactivation.

Anterior spacing is a common esthetic problem of patient during dental consultation. The most common etiology include tooth size and arch length discrepancy. Maxillary lateral incisors vary in form more than any other tooth in the mouth except the third molars. Microdontia is a condition where the teeth are smaller than the normal size. Microdontia of maxillary lateral incisor is called as “peg lateral”, that exhibit converging mesial and distal surfaces of crown forming a cone like shape. A carefully documented diagnosis and treatment plan are essential if the clinician is to apply the most effective approach to address the patient’s needs. A patient sometimes requires a multidisciplinary approach to correct the esthetics and to improve the occlusion. This case report describes the management of an adult female patient with a proclined upper anterior teeth, upper anterior spacing, deep bite and peg shaped upper right lateral incisor tooth through orthodontic and restorative treatment approach.

Abstract Nothing could be more desirable than a sparkling white smile. A shining, beautiful smile has been a symbol of health & vitality since the centenary. Discoloration of a tooth is a common esthetic problem caused by either extrinsic or intrinsic factor. The normal color of tooth can be re-established by decolorizing the stain with the powerful oxidizing agent such as 35 percent hydrogen peroxide, which is directly placed on the labial surface of the tooth to be treated. Dental bleaching offers a conservative & simplified approach to change the color of discolored teeth. This case report reflects the remarkable change of tooth color by in-office bleaching.

Dental bleaching is a method of treatment for tooth discoloration that uses hydrogen peroxide in various concentrations. For this dental maneuver to be successful, the etiological diagnosis of dental dyschromia and the treatment plan must be rigorously established. In this article we present the criteria underlying the selection of patients who can benefit from dental bleaching, grouped in a questionnaire. Adequate case selection suitable for teeth whitening, as well as the appropriate choice of the specific bleaching method are essential in obtaining satisfactory aesthetic result for the patient, immediately after completing the treatment, but also in the long term.

SUMMARY The present clinical case report describes the clinical steps of enamel microabrasion associated with dental bleaching to restore severely-pitted fluorosed teeth. The process of removing the fluorotic superficial stains started by using macroabrasion with a water-cooled fine tapered 3195 FF diamond bur. Rubber dam isolation of the operative field was used to remove the remaining enamel stains and superficial irregularities with the Opalustre microabrasive compound (6.6% hydrochloric acid associated with silicon carbide particles) followed by polishing using fluoridated paste and subsequent 2% neutral fluoride gel topical application. After one month, dental bleaching was performed using 10% carbamide peroxide in custom-formed acetate trays for two hours/day for 42 days. The association of enamel microabrasion with dental bleaching was effective for reestablishing the dental esthetics of a patient with severe dental fluorosis.

Clinical Relevance Combined techniques to expose circumferential tooth structure associated with subsequent restoration may represent a valid option in many situations. This case provides an example of the successful management of the anterior tooth rehabilitation combining root displacement and crown lengthening. SUMMARY Rehabilitation of an extensively compromised single anterior tooth represents an intriguing challenge for dentists, particularly when the rehabilitation involves esthetic, psychosocial, and functional requirements. The success in rehabilitating a patient with a conservative approach depends on a critical evaluation of the remaining structures, precise treatment plan, systematic treatment strategies, and patient compliance. In this case, the patient’s chief complaint was the undesirable appearance of the maxillary right lateral, caused by the displacement of the post and crown. Clinical examination revealed a remaining tooth with coronal fracture, severe loss of tissue due to caries, and absence of ferrule effect. Radiographic examination revealed that the fracture margin was located subgingival. This case report describes a single-tooth rehabilitation involving a combination of root displacement via orthodontic extrusion and crown lengthening. The rehabilitation was followed by post-and-core restoration using a prefabricated glass fiber post associated with a disilicate lithium crown. The clinical decision making and combined effect of both treatment strategies are explained in this report. The treatment required three months, including recovery times after surgery and the placement of the provisional crown. The patient was esthetically and functionally satisfied with the restoration. Patient follow-up examination was performed 24 months after the treatment. This clinical report contributes to the clinical practice and exemplifies the possibility of rehabilitating the natural tooth using combined techniques, which may offer particular advantages regarding prognosis and invested efforts.

During dental bleaching the staining potential of the surface would increase. This study aims to evaluate the staining susceptibility of one bleached composite resin after the exposure to three different beverages: Peruvian purple corn based beverage (chicha morada), green tea, and distilled water. Thirty disk-shaped specimens of one nanofill composite resin were prepared. The specimens were then divided into six groups (n=5): purple corn (P), purple corn + bleaching (PB), green tea (T), green tea + bleaching (TB), distilled water (W), and distilled water + bleaching (WB). In groups that received bleaching, two sessions of bleaching with 35% hydrogen peroxide were done. Following bleaching, specimens were exposed to each liquid thirty minutes daily. Color was measured with a digital spectrophotometer. For statistical analysis, color measurement differences between the obtained results were used: during bleaching, after bleaching, and during + after bleaching. Two-way ANOVA was used to compare the color changes in the resins of all groups (p<0.05). We conclude that all the evaluated beverages produced changes of color in the composite resin regardless of the bleaching procedure. However, purple corn was the only beverage that caused a perceptible color change (ΔE>3.3).
