The aim of this in vitro study was to evaluate the bond strength of Epiphany™ resin-based sealer to dentin walls after placement of calcium hydroxide [Ca(OH)2] dressings. Fifteen extracted single-rooted human teeth were instrumented using 2.5% NaOCl + EDTA as irrigants. The teeth were randomly assigned to 3 groups (n=5), according to the intracanal dressing: G1= Ca(OH)2 + saline; G2= Ca(OH)2 + 2% chlorhexidine gluconate (CHX) gel; and G3= saline (control). After 10 days of storage in 100% humidity at 37ºC, the dressings were removed and the root canals were filled with Epiphany™ sealer. After additional 48 h of storage, the specimens were sectioned transversally into 2-mm-thick discs. Push-out tests were performed (1 mm/min, Instron 4411) and the maximum loads at failure were recorded in MPa. One-way ANOVA and Newman-Keuls tests showed a statistically significant decrease in bond strength when a Ca(OH)2 dressing was used before root canal filling with Epiphany™ (G1= 10.18 ± 1.99 and G2= 9.98 ± 2.97) compared to the control group (13.82 ± 3.9) (p< 0.05). It may be concluded that the use of Ca(OH)2 as an intracanal dressing material affected the adhesion of Epiphany™ to the root canal walls, but even though the values were within the acceptable range found in the literature.

PURPOSE: The aim of this study was to evaluate, in vitro, the quality of calcium hydroxide [Ca(OH)2] paste filling (Ultracal, Ultradent) associated or not with Ca(OH)2-containing gutta-percha points (Calcium Hydroxide Plus PointsTM, Roeko) in curved root canals. MATERIAL AND METHODS: One hundred and twenty roots of extracted human teeth, randomly divided into three curvature ranges (mild - 0 to 14º; moderate - 15º to 29º; severe - >30º) were used. After chemomechanical preparation, the roots were assigned to 4 groups (n=30), according to the technique of intracanal dressing placement: group 1 - Ca(OH)2 paste was applied with a lentulo spiral; group 2 - Ca(OH)2 paste was applied with a lentulo spiral and a Ca(OH)2 point was inserted into the canal; group 3 - Ca(OH)2 paste was applied with a NavitipTM tip (supplied with Ultracal system); group 4 - Ca(OH)2 paste was applied with a NavitipTM tip and a Ca(OH)2 point was inserted into the canal. The roots were cleared and the quality of apical third filling was assessed by a calibrated experienced examiner. The specimens were examined under stereomicroscopy and scored 1 to 4 (i.e., from inadequate to complete root canal filling). The results were analyzed statistically by ANOVA and Duncan's post hoc test at 5% significance level. RESULTS: There were no statistically significant differences (p>0.05) among the curvature degrees in groups 1, 3 and 4. Severely curved roots in group 2 presented bordering significance (p=0.05). The groups that associated the use of Ca(OH)2 paste and points (2 and 4) showed better apical filling than the other groups, but this difference was statistically significant (p<0.001) only for roots with severe curvature. CONCLUSION: According to the results of this study, the curvature degree did not influence the quality of filling. The techniques that used Ca(OH)2-containing gutta-percha points yielded better filling of the apical third in roots with severe curvature.

Objectives: The aim of the present study was to assess the effect of calcium hydroxide as a root canal dressing material on dentin microtensile fracture strength in human primary teeth in vitro. Study design: Thirty primary anterior teeth with root canals packed with calcium hydroxide were divided into groups of ten and immersed in saline at room temperature for 7, 30 and 90 days. Ten teeth with root canals filled with sterile saline were the control group. Microtensile fracture strength was measured in Mechanical tester Lloyd testing machine. Results: There was a significant difference (P &lt; 0.05) between the fracture strength of the calcium hydroxide-filled teeth after 90 days (19.1 MPa) compared with the control (35.8 MPa). Dentin microtensile fracture strength of the calcium hydroxide-filled teeth decreased at an average of 0.142 MPa per day. Conclusion: Calcium hydroxide placed in root canals for an extended time had a significantly negative effect on root strength. Long-term success of root canal treatment in primary anterior teeth is estimated as 65% with most of the failures result from trauma recurrence. Clinical Relevance: Our results stress the need to evaluate the pros and cons of root canal treatment compared to extractions of non-vital primary incisors.

Calcium hydroxide has been used extensively in endodontic treatments, for instance as an intra-canal dressing; however, the exposure of root canal dentin to calcium hydroxide may affect its flexural strength and could have important clinical implications for endodontic treatment. The purpose of this in vitro study was to investigate the influence of calcium hydroxide on the flexural strength of root canal dentin.Seventy-two extracted single-rooted human mandibular premolars were used in this study. Each tooth was instrumented using crown-down technique and was irrigated using sterile saline. The teeth were assigned into three groups of 24 each. The prepared root canal system of each tooth was filled with calcium hydroxide mixed with sterile saline (group 1), a calcium hydroxide commercially available product (UltraCal®) (group 2) or saline solution (group 3, as control). The apices and access opening were sealed using composite resin, and the teeth were immersed in artificial saliva. After 7, 14 and 30 days of immersion, the inner root canal dentin of 8 teeth respectively from each group were sectioned to create dentin bars (1 X 1 mm, with 7 mm in length). Each dentin bar then was subjected to a three-point bending flexural test using MTS (Universal Testing Machine). Data gathered were then analyzed using two-way ANOVA, followed by Tukey’s test with the level of significance of 95%. The results showed that exposure to calcium hydroxide either using calcium hydroxide mixed with sterile saline or UltraCal® for 14 and 30 days can reduce flexural strength of root canal dentin compared to control group (p<0.05). In contrast, after 7 days exposure, there was no significantly different of flexural strength between three groups (p>0.05).   It can be concluded that calcium hydroxide reduced the flexural strength of root canal dentin. The longer the exposure to calcium hydroxide would produce a greater effect on flexural strength of root canal dentin.

Introduction: A pivotal issue to achieve success in the treatment of the root canal is root canal disinfection. One of the most important bacteria that infect the root canal is Enterococcus faecalis. This study seeks to examine the effectiveness of 3 methods for disinfecting the root canal: photodynamic therapy, modified triple antibiotic paste (MTAP), and calcium hydroxide. Methods: Sixty-two single-rooted extracted anterior teeth were collected. After cleaning and disinfecting the teeth, their crowns were cut at the CEJ point. The root canals were shaped to the working length up to file F3 ProTaper (F1, F2, F3). EDTA 17% and sodium hypochlorite 2.5% were used for 5 minutes to wash and remove the smear layer, and then the apical foramen was sealed using composite. After that, the teeth were sterilized in an autoclave at 121°C for 15 minutes. Then 10 samples were taken randomly as the negative control. The remaining samples were immersed and cultivated in a suspension containing E. faecalis for 21 days. Then the samples were divided into 5 groups: 2 positive control groups, 1 group treated with the antibiotic paste with the concentration of 1 mg/mL, 1 group treated with calcium hydroxide, and 1 group treated with photodynamic therapy. Then, to collect the biofilm, the ProTaper file F4 was used. After that, the microbial suspension was provided and counting the colonies was carried out to compare the groups. Results: The findings indicated that the amount of CFU/mg of MTAP samples, including clindamycin, metronidazole, ciprofloxacin in the concentration of 1 mg/mL and photodynamic therapy and calcium hydroxide was lower than that in the control group. Antibiotic paste cleansed the root canal up to 99.9%. Photodynamic therapy reduced the amount of CFU/mg to 98.8%, and calcium hydroxide reduced the amount of CFU/mg to 94.13%. Conclusion: Using photodynamic therapy causes a reduction in the biofilm and inhibits the growth of the E. faecalis bacterium. In addition, in this study, MTAP with a concentration of 1 mg/mL was used, which expunged the bacteria completely. Meanwhile, calcium hydroxide had the weakest effect of all on the E. faecalis bacterium.

ABSTRACTObjective: Studies about the dispersion of hydroxyl ions through dentin of endodontically retreated teeth are scarce. The present study aims at examining the diffusion of calcium hydroxide in vitro by recording the pH changes of retreated root canal using two types of calcium hydroxide preparations. Materials and Methods: A total of 45 recently extracted single-rooted mandibular first premolars were collected and cleaned from calculus and remains of periodontal tissue. The teeth so collected were stored in thymol 10% solution till use. During collection, teeth with cracks, open apices, and restorations were excluded. Root canal preparations were performed using Protaper rotary system files, till #F3 file. All canals were obturated using cold lateral condensation and zinc oxide-based sealer. After 7 days, all gutta-percha were removed and the canals were irrigated with normal saline. The teeth were then randomly distributed into three groups. Canals in Group I were kept empty without any dressing to serve as a negative control group, Canals in Group II were filled with a freshly prepared mixture of calcium hydroxide powder with normal saline solution, and Group III was filled with Metapex. Periodically, the calcium hydroxide material was removed and the pH was measured using pH meter at 7, 10, 14, and 30 days. Results: The pH values of Group II and III ranged between 9.2 and 11.2. This was found to be highly significant against the negative control group. After 7 and 10 days, freshly mixed Ca(OH)2 showed the higher pH than the creamy Metapex, and statistically the difference was highly significant after 10 days (P<0.001). After 14 days, Metapex group pH was higher than freshly mixed Ca(OH)2, but it was not statistically significant as they reached nearly the same pH after 30 days (P>0.05). Conclusion: All Ca(OH)2 preparations had high pH around the roots after 7 and 10 days. Metapex continued to have higher pH after 14 days.

Introduction Root canal filling materials are intended to prevent microleakage and passing of microorganisms and their toxins along the root canal space and into the periradicular tissues. Objective Objective of this in vitro study was to evaluate and compare apical leakage of root canals obturated using a modified warm vertical condensation technique and different materials. Methods Sixty-five extracted single-root teeth were prepared according to the crown-down/ step-back technique. Each canal was rinsed with 3% NaOCl during and after the preparation. The teeth were divided into 3 groups. Every group was obturated by the same technique, but with different materials: RealSeal system (SybronEndo), gutta-percha with AH Plus (Dentsply Maillefer) and gutta-percha with Acroseal sealer (Septodont). The remaining 20 teeth were in the control group. The teeth were immersed in 1% methylene blue for 72 hours. After that period, the roots were split longitudinally, and dye penetration was measured using a stereomicroscope. Results Dye penetration occurred in all groups. The least dye penetration occurred in the RealSeal group (0.33?}0.29 mm), while the highest dye penetration occurred in the Acroseal group (1.11?}0.52 mm). According to the Student's t-test, Acroseal showed significantly more leakage (p<0.001) than RealSeal and AH Plus materials. The microleakage of RealSeal system was not significantly different (p>0.05) from that of AH Plus sealer with gutta-percha. Conclusion The results suggested that new material Resilon (RealSeal) and conventional combination AH Plus with gutta-percha had the least apical dye penetration and provided the best sealing.
