Reimplantation of an Inverted Maxillary Premolar: Case Report of a Multidiscplinary Treatment Approach

Inversion of premolars is an extremely rare condition, which usually requires extraction. This case report describes the inversion of an impacted maxillary second premolar in an 11-year-old male, and the multidisciplinary treatment approach for bringing the tooth into a normal position within the arch. In order to provide sufficient space for surgical reimplantation of the tooth, the mesially-drifted neighbouring maxillary first molar was first endodontically treated, followed by orthodontic distalization of the tooth. The inverted tooth was removed surgically and reimplanted without the use of splints for stabilization. After a 12-month follow-up period, the tooth maintained its vitality without any root resorption. Reimplantation of impacted inverted premolars can be a viable treatment alternative to extraction

Internal root resorption (IRR) is described as a resorptive defect of the internal aspect of the root caused by odontoclastic activity associated chiefly with chronic pulp inflammation and trauma. An important variation to consider is the presence of a root perforation, although it is rare. This paper defines the use of CBCT in the diagnosis and combined nonsurgical and surgical multidisciplinary management and follow-up of a maxillary central incisor with perforating IRR in a 9-year-old female patient. At 3-year follow-up, clinical and radiographic findings of the case were satisfactory.

Abstract Aim The purpose of this case report is to present the management of an impacted maxillary canine adjacent to a deviated palatal first premolar root. Background Maxillary canine impactions are of multifactorial etiology. The incidence of maxillary canine impaction ranges from 1% to 3%. One of the reasons for canine impaction might be a deviated palatal premolar root. To date, a total of six cases have been described where impacted maxillary canines were found in close proximity to a deviated palatal first premolar root. Report A male patient, aged 17 years 3 months, with a Class I molar relationship was referred for the continuation of orthodontic therapy. Leveling was completed in the lower and upper arches. Although sufficient space was available in the upper arch the left canine had not erupted. Radiographic examination disclosed a deviated palatal root of the first premolar in the canine eruption path. Summary A multidisciplinary treatment approach for the maxillary impacted canine adjacent to a deviated palatal premolar root is described. The premolar was endodontically treated and the deviated root surgically resected. The impacted canine was then orthodontically erupted into position. Clinical Significance A deviated maxillary premolar root may cause canine impaction. A multidisciplinary approach facilitates successful treatment results in such cases. The loosely structured periodontal ligament and low mineralized bone surrounding erupting teeth provide minimal resistance to an extrusive force. The maxillary central incisors are the most frequently avulsed due to trauma. The prognosis of replanted teeth is affected by extra-alveolar period and conditions. Implant-supported restorations are contraindicated in adolescent patients with poor prognosis traumatized teeth. Autotransplantation of premolar to the anterior region provides a viable treatment alternative.<br/>This case report describes the autotransplantation of premolars and esthetic rehabilitation in a teenage patient with traumatized incisors, poor oral hygiene, and severe crowding. Comprehensive and long-term treatments were performed to improve complex and challenging oral problems.

Dental trauma is common in young patients with immature teeth. When necrosis of pulp follows in such a tooth, apexification of the root is the most recognized and accepted treatment of choice. Calcium hydroxide paste is the most popular material used to induce the apical hard tissue barrier which may be composed of different tissues. Hertwig’s epithelial root sheath and (or) its remnants, the cell rests of Malassez may contribute to the apical closure. In the presented case, careful multidisciplinary treatment approach helped to regain esthetic and function. After 27 months the tooth showed every sign of success. Transmigrated mandibular canines increase treatment complexity in terms of both anchorage and biomechanical planning, posing challenges for both Orthodontists and Oral Surgeons. This case report presents the orthodontic treatment in a 12-year-old girl patient, with transmigrated and impacted mandibular right canine positioned horizontally below the apices of the mandibular incisors. The mandibular deciduous right canine was extracted followed by the traction of the transmigrated permanent canine. Eruption was properly guided, and the correct position of the tooth was orthodontically guided into its normal position in the arch. The patient’s occlusion was significantly improved with good aesthetic outcome, functional occlusion, and a stable result after 1.5 years of follow-up.

Adrenal cysts are rare lesions that could be epithelial, endothelial, parasitic or haemorrhagic[1], as well as pseudocysts. Haemorrhagic adrenal cysts are extremely rare and are often asymptomatic, so diagnosis can be really challenging. This can prove really difficult for primary care physicians who are often the frontline clinicians these patients tend to present to. They are usually benign lesions and do not often cause mortality if detected early and prompt surgery is done, as was the case with the patient in our case report[4]. When they do become symptomatic, they can present with different systemic symptoms as documented in literature, including in our case report[2,4]. Diagnosis is usually through Ultrasound and CT Scan and management is largely laparoscopic or open excision depending on the size of the lesion, surgical expertise and local protocol. Most patients make full recovery and mortality is extremely low [3]. The aim of this review is to provide a broader overview of the subject, highlight salient points in several studies relating to haemorrhagic cysts, provide an up to date follow up information on the index patient in our case report and to explore possible areas for future study [4,6]. This review also includes a suggested management algorithm and intends to emphasize the fact that patients who present in primary, urgent or emergency care settings with persistent non-specific symptoms should be investigated for rare diseases.

External resorption is sequelae of necrotic periodontal membrane over a large area of root following an injury to the tooth. This usually occurs after severe dental injuries such as intrusion, severe luxations or exarticulation injuries complicated by a prolonged extra oral period. This case report presents a clinical and radiographic follow up (13 months) of treatment of inflammatory external root resorption on maxillary central incisor using Vitapex®. Gradual healing of resorption was observed radiographically with no tenderness or pathological mobility.
