The possibility of using combined erbium and neodymium lasers to correct the age-related changes in the face and neck skin by the «Fotona» apparatus using the «Fotona 4D» technique was investigated. To evaluate the results of the study in dynamics an index of age-related changes in skin was developed and applied. The results of photographic fixation and ultrasound of the facial and neck skin before and after laser procedures were also compared. It was determined, that the combination of two wavelengths of 2940 nm (erbium laser) and 1064 nm (neodymium laser) and the «Fotona 4D» technique with 4 patented technologies allows to carry out the procedure at the level of superficial, middle and deep structures of the face and neck tissues. Nowadays it is the only technology for non-invasive volumetric reconstruction and facial skin voluminisation, which is the result of a combination of synergistic methods of laser exposure. It was shown that one month after the second procedure of laser treatment, the face skin turgor increased, its dryness, pore severity, quantity and depth of wrinkles decreased, side effects and complications were absent. Ultrasound examination of the facial skin and index of age- related changes assessing in the face skin confirmed the positive dynamics of correction. Thus, the thickness of the skin in the area of nasolabial folds and superciliary arches increased by 0,5 mm (from 1,9 to 2,4 mm), derma by 0,6 mm (0,8 to 1 mm). The index of age-related changes assessing in the facial skin decreased by 10-13 points (before the procedure it was within the range of 20-24 points (average degree of age changes), after the procedure it was 11 points (mild degree of age changes). Thus, the use of combined erbium and neodymium lasers using the «Fotona 4D» technique showed a high efficiency of laser correction of age-related changes in the facial skin. Implemention of modern technologies in practice of dermatological clinics will improve skills of medical personnel on the use of laser technology.

The results of postclinical trial of hyaluronic acid mesh material for plastic surgery ЭВГУЛОН®. Аrea of use: skin and soft tissue defects of the face and neck, the resulting injuries, features of constitutional development or age-related changes. ЭВГУЛОН® injected intradermal. The results showed a high efficiency of the material for leveling the relief in the face and neck. Of the complications observed isolated cases of hypereflexia and hemorrhage, will not affect the outcome. In conclusion: ЭВГУЛОН® it can be successfully used not only in cosmetic but also reconstructive surgery.

GalynaViktorovnaKhrushch plastic surgeon, maxillofacial surgeon, international candidat ASPS, Periorbital area is worthily considered as one of the most complicated regions in terms of correction of the age-related changes using injection techniques of medical aesthetics. According to the classification of I.I. Kolgunenko Russian (1974), tired morph type of age-related changes which is the most physiological type of aging, incorporates changes mainly focused in the middle third of the face, including in the periorbital area. These changes include the formation of grooves (tear trough, palpebromalar groove, nasojugal groove) (Figure 1), dark under-eye circles, mimic wrinkles formation, fat compartment displacement, change in mimic muscle tone, stretching of ligamentous structures, overhang of the upper eyelid.

Bipolar radio-frequency devices have all the theoretical prerequisites for use in combination with phototechnologies, while the properties of electro-optical synergy are manifested. Material and method. The study included 125 patients aged 40 to 50 years. Depending on the therapy, patients were divided into 4 groups and subgroups A and B, depending on the identified predictors of the effectiveness of laser therapy. The results of the study. After applying the combined methods, a more significant improvement in the quality characteristics of the skin was noted: an increase in skin moisture in the 1A group, the corneometry index increased by 24.6%, in the 1st group by 28.3%, in the 2A group by 38.9%, in the 2B group by 37.3%, in the 3A group by 29.1%, in the 3B group by 27.5%, in the 4A group by 38.8%, in the 4B group by 38.8%; an increase in skin elasticity in group 1A, the Ua/Uf indicator increased by 13.2%, in group 1B by 13.9%, in group 2A by 27.9%, in group 2B by 29.0%, in group 3A by 14.9%, in the 3B group by 14.6%, in the 4A group by 28.2%, in the 4B group by 28.2%, which was accompanied by a more pronounced decrease in the severity of wrinkles according to profilometry. The use of laser therapy (an erbium or neodymium laser) and microneedle RF therapy contributes to a more significant restoration of the epidermal-dermal structure of the skin, which is confirmed by ultrasound data: the microrelief has improved by more than 3 times compared with monotherapy, the thickness of the dermis is 2.2 times and acoustic density 1.7 times. Conclusions. Combined methods, to a greater extent than mono-laser therapy, contribute to improving the quality of life of patients according to the dynamics of the LHC index, which decreased in groups 1 and 3 by an average of 47.1% and 44.4% versus 57.8% and 64.3% in 2 and 4 groups, while there was a significant difference in the values of the index of HSCI in groups A and B (p 0.01). In accordance with the VAS and GAIS indicators, the optimal effect after a course of therapy with preservation of the results after 1 year was achieved in 25/12.5% of patients in group 1A, in 46.7/33.3% in group 1B, 7/37.5% of patients of group 2A, 78.6/71.4% of patients of group 2B, 33.3/20.0% of patients of group 3A, 50.0/25% of patients 3B groups, 58.8/58.8% and 75.0/75.0% in patients 4A and 4B of the group, respectively, which indicates the high efficiency of the combined methods and the persistence of the results.

Age-related changes in the face - at all times were not a desirable phenomenon in socially active and successful people. In modern society, success is primarily identified with an attractive appearance and a healthy lifestyle. Healthy lifestyle is actively promoted in mass media, this message has many followers all over the world. The most striking manifestations of age-related changes are concentrated in the lower third of the face and submental area, which is manifested in violation of the contours of the jaw line, as well as in the smoothing of the cervical - chin angle. In some genetic features, such as micrognathia, these changes lead to the visual perception of the face less young than it actually is, even without obvious manifestations in other areas of the face. We have developed and put into practice a minimally invasive method for correcting the lower third of the face and the submental area, using laser-assisted liposuction, with various degrees of age-related changes. Which allows you to effectively deal with this problem.

All areas and tissues of the face will go through age-related changes; however, aging occurs at a faster rate in the lower third of the face than in other areas. This study presents the anatomical and physiological aspects of age-related changes in all areas and tissues of the face, mainly in the lower third, such as the skin, subcutaneous fat, muscular-aponeurotic system, deep layers of adipose tissue, and bone structures of the face. Involutional changes were differentiated by taking into account the constitutional and architectonic features of the face. Heterochronous and heterotopic characteristics of age-related changes, cutaneous, and treatment components of facial aging were identified. The most commonly used visual scales to assess cosmetic effects and determine chronological aging included global aesthetic improvement scale, Glogau scale, Fitzpatrick wrinkle classification, and Merz visual 5-point aging scale. According to the classification of age-related changes proposed by Kolgunenko, five morphotypes of aging are distinguished: tired, wrinkled, deformation, mixed, and muscular. Understanding the pathogenesis of involutional changes in the lower third of the face in patients with different morphotypes of aging allows for an objective and individualized choice of optimal therapy methods.
