Osteomalacia is mainly reduced to changes in the bones - they lose their mineral substances and along with this, abrupt changes occur in the organic structure of bones and in the bone marrow - and the bone and bone marrow soften. Along with this, degenerative changes occur in the muscular system. The clinical picture is first expressed by pain in different parts of the skeleton, early fatigue, increased reflexes; then, with strong development, bone curvature, persistent catarrh of the respiratory and digestive tract; patients die with symptoms of insanity. The disease lasts from 1 to 10 years and affects mainly women who have already given birth. Neither poverty nor prolonged feeding seems to matter.

Abstract Purpose The purpose of the present study was to evaluate the clinical and radiological outcomes of arthroscopic bone marrow stimulation (BMS) for the treatment of osteochondral lesions of the talus (OLTs) at long-term follow-up. Methods A literature search was conducted from the earliest record until March 2021 to identify studies published using the PubMed, EMBASE (Ovid), and Cochrane Library databases. Clinical studies reporting on arthroscopic BMS for OLTs at a minimum of 8-year follow-up were included. The review was performed according to the PRISMA guidelines. Two authors independently conducted the article selection and conducted the quality assessment using the Methodological index for Non-randomized Studies (MINORS). The primary outcome was defined as clinical outcomes consisting of pain scores and patient-reported outcome measures. Secondary outcomes concerned the return to sport rate, reoperation rate, complication rate, and the rate of progression of degenerative changes within the tibiotalar joint as a measure of ankle osteoarthritis. Associated 95% confidence intervals (95% CI) were calculated based on the primary and secondary outcome measures. Results Six studies with a total of 323 ankles (310 patients) were included at a mean pooled follow-up of 13.0 (9.5–13.9) years. The mean MINORS score of the included studies was 7.7 out of 16 points (range 6–9), indicating a low to moderate quality. The mean postoperative pooled American Orthopaedic Foot and Ankle Society (AOFAS) score was 83.8 (95% CI 83.6–84.1). 78% (95% CI 69.5–86.8) participated in sports (at any level) at final follow-up. Return to preinjury level of sports was not reported. Reoperations were performed in 6.9% (95% CI 4.1–9.7) of ankles and complications related to the BMS procedure were observed in 2% (95% CI 0.4–3.0) of ankles. Progression of degenerative changes was observed in 28% (95% CI 22.3–33.2) of ankles. Conclusion Long-term clinical outcomes following arthroscopic BMS can be considered satisfactory even though one in three patients show progression of degenerative changes from a radiological perspective. These findings indicate that OLTs treated with BMS may be at risk of progressing towards end-stage ankle osteoarthritis over time in light of the incremental cartilage damage cascade. The findings of this study can aid clinicians and patients with the shared decision-making process when considering the long-term outcomes of BMS. Level of evidence Level IV.

The manifestations of muscular syndrome affecting the gravity and prediction of the disease take place in the most widespread and invalidizing rheumatic diseases: osteoarthrosis, rheumatoid arthritis and ankylosing spondylarthritis. Paraclinical studies showed heterogeneity of pathogenetic mechanisms in lesion of skeletal muscles in rheumatic diseases. The differentiated methods of the treatment of patients with osteoarthrosis, rheumatoid arthritis and ankylosing spondylarthritis taking into account the clinical picture and pathogenesis of muscular syndrome, as well as the diagnosis criteria and classification of muscular system lesion are suggested.

The influence of the Chernobyl accident in 1986 on the Finnish reindeer herding area was much smaller than the effects of the nuclear bomb tests in the 1960s. Only in one small area somewhat more Cs-137 was deposited than in the rest of the reindeer herding area. From that area 20 reindeer were chosen for investigation of the distribution of Cs-137. All tissues, organs, the skeleton, digestive tract, hide, head and hooves were sampled quantitatively. Three reindeer were pregnant and also the foetuses were studied. The Cs-137 amounts were determined by gammaspectrometric measurements. The results showed that the differences in the Cs-137 concentrations between muscle tissue from different parts of an individual reindeer were not more than 10 percent. Thus it is not essential from which part of the reindeer meat samples for surveillance purposes are taken. The concentration of Cs-137 in edible tissues other than muscle was lower except in the kidneys and scapula cartilage.

Three experiments have been carried out with altogether 16 Ayrshire calves in order to determine the digestibility of timothy hay cured at the early flowering stage and containing some red clover. The ages of the calves varied from 2 ½ to 9 months. Hay was given either totally or almost ad lib. Lignin was used as tracer. In experiment 1 the ages of the six experimental calves varied from 3 to 9 months. The calves aged 5—9 months digested hay equally efficiently when com-pared with each other. The youngest calf aged 3 months digested hay less efficiently (Table 1). In Experiment 2 the ages of the six experimental calves varied from 10 to 22 weeks. In the trial two control cows were also included. The calves aged 10—12 weeks digested hay less efficiently and the calves aged 4—5 months almost as efficiently as the control cows (Table 2). The ability of calves of the same age to digest hay was quite variable. Experiment 3 was carried out with four 7 months old calves. At the end of the trial the calves were slaughtered and the contents of the different parts of the digestive tract were weighed and sampled. On the ground of the analyses of the hay, the contents of the abomasum and the faeces, the digestibilities of N-free organic matter and N-free non-lignin organic matter in the proventriculi and in the whole digestive tract were esimated. The share of the proventriculi in the total digestibility of the said substances was 82—88 per cent (Table 3). The contents of the reticulo-rumen of the calves in Experiment 3 as well as those of younger calves aged 3 ½—4 months were weighed in connection with the slaughtering. The quantity of the contents in calves of 7 months was 30—40 kg and that in calves aged 3 ½—4 months 12—15 kg.

Many antispasmodic and anticholinergic drugs have been shown to reduce motility and secretion in different parts of the digestive tract, but in doses large enough to be effective these drugs invariably produce side effects. Oxyphencyclimine (Daricon - Pfizer) inhibits gastric secretion in animals without the peripheral anti-cholinergic effects seen with comparably effective gastric anti-secretory doses of other anticholinergic drugs tested. 1 Studies in man suggest that oxyphencyclimine has a selective action on the stomach, 2 that it can relieve symptoms of duodenal ulceration, 3 and that it has a prolonged action.

Abstract A case of neuroblastoma in an adult with extensive metastatic lesions is presented. The initial symptoms and findings suggested idiopathic thrombocytopenic purpura. The blood picture and changes in the clinical picture later led to a diagnosis of acute leukemia. Autopsy disclosed the correct diagnosis, which would have been possible antemortem had the implication of pseudorosette arrangement of immature cells in the bone marrow and the possible occurrence of neuroblastoma in an adult been fully appreciated.

We describe a case of autoimmune lymphoproliferative syndrome (ALPS), which is very unusual with regard to a clinical onset soon after birth, and a clinical picture dominated by splenomegaly, jaundice, and consumptive peripheral blood cytopenias, with minimal lymphadenopathy. Our documented close follow up demonstrated initial involvement of the spleen, followed by involvement of the bone marrow and the peripheral blood. The patient underwent bone marrow transplant and is alive and well 20 months after diagnosis.

The article presents two cases of hepatitis-associated aplastic anemia in the last year. The literature data on the new nosology are presented. The clinical picture, diagnosis and treatment are discussed. The questions of diagnosis and treatment that affect the prognosis are raised. Treatment preference should be given to allogeneic bone marrow transplantation, as it has better results than aggressive immunosuppressive therapy that is difficult for patients to tolerate.
