The aim of the study was to evaluate the effectiveness of Tadalafil-SZ in treatment of erectile dysfunction in men suffering from type 2 diabetes mellitus or prediabetes. Two patient groups of thirty people with type 2 diabetes mellitus and prediabetes were identified. Patients mean age was 52.2 3.4 years and 48.5 2.8 years in groups with type 2 diabetes mellitus and prediabetes respectively. All patients suffered from erectile dysfunction, which significantly reduced their quality of life. To assess the erectile function, the ICEF (International Index of Erectile Function) and QoL (Quality of Life) questionnaires were used. Patients with prediabetes took Tadalafil-SZ 5 mg once a day for 3 months and patients with type 2 diabetes mellitus took Tadalafil-SZ 20 mg 2 times a week (Monday and Thursday) for 3 months as a treatment for erectile dysfunction. Results of this study showed that the administration of Tadalafil-SZ not only led to a significant improvement in the quality of life, a significant reduction in weight, body mass index, waist size in patients of both groups, but also led to a glycated hemoglobin level normalization without increasing the dose of glucose-lowering therapy in patients with uncomplicated type 2 diabetes mellitus and a pronounced decrease in morning insulin levels measured in the blood in patients with prediabetes. Almost half of patients in both groups increased testosterone blood level. According to the ICEF questionnaire, erectile function was significantly improved: in the group of patients with prediabetes: the ICEF-5 index prior to taking Tadalafil-SZ averaged 18.68 points (mild ED) whereas after taking the drug, a statistically significant increase in the ICEF index to an average of 23.02 points (p 0.05) was observed, which indicates the absence of erectile dysfunction in this group of patients during intake of Tadalafil-SZ. In the 2nd group of patients (with type 2 diabetes mellitus), the ICEF-5 index before treatment averaged 12.18 points, which reflects the average degree of erectile dysfunction, and at the end of the study there was a statistically significant increase in the ICEF-5 index to 18.44 points on average, which indicates a decrease in the severity of erectile dysfunction from moderate to mild (p 0.05). Overall Tadalafil-SZ is an effective treatment for erectile dysfunction in both patients with prediabetes and patients with type 2 diabetes mellitus.

Abstract Erectile dysfunction (ED) is the inability to develop and/or maintain an erection that is sufficient for satisfactory sexual intercourse. The prevalence of erectile dysfunction in diabetic men is 28-75%, this percentage rising with patient’s age and duration of diabetes. The aim of the present study was to investigate erectile dysfunction and quality of life in patients with type 2 diabetes mellitus (T2DM) after treating them with transdermal testosterone or with alpha-lipoic acid. Materials and methods: The effect of a 12-week treatment with transdermal testosterone or alpha-lipoic acid on the erectile function and quality of life of 45 men with ED and T2DM was studied in a randomized, prospective, open clinical, comparative study. The parameters we measured in the patients were body weight and body mass index (BMI); the albumin, lipids, HbA1C, testosterone (T), sex hormone-binding globulin (SHBG), follicle stimulating hormone (FSH), luteinizing hormone (LH) and microalbuminuria levels; the International Index of Erectile Function (IIEF) and Health related quality of life (SF-36) questionnaires were completed to evaluate ED and quality of life before and after 12 weeks of treatment with alpha-lipoic acid (600 mg, parenterally, for 7 days, followed by 600 mg received per os) or with transdermal testosterone in a dose of 50 mg daily. Results: Testosterone treatment decreased BMI significantly (p < 0.01), increased testosterone concentrations (p < 0.01) and raised the SHBG levels (p < 0.05), improved the glycemic control and lipid profile (total cholesterol, p < 0.05; HDL cholesterol, p < 0.05; triglycerides, p < 0.05). The patients treated with alpha-lipoic acid had their BMI (p < 0.01), HbA1C (p < 0.01), total cholesterol (p < 0.01), HDL-cholesterol (p < 0.01) and triglycerides (p < 0.01) significantly reduced. The indicators for ED in both groups were also statistically significantly improved. There was improvement for all patients’ self-assessment score for “physical functioning” (p = 0.001), for “role limitations due to physical health” (p < 0.001) and for “general health perception” (p = 0.021). Conclusions: Transdermal testosterone and alpha-lipoic acid have a tangible beneficial effect on erectile dysfunction and on metabolic disorders in T2DM patients and can be used to treat such patients.

The aim of the study was to evaluate associations of emotional state and quality of life with lipid concentration, duration of the disease, and the way of treating the disease in males and females with type 2 diabetes mellitus. A total of 53 persons with type 2 diabetes mellitus (27 males and 26 females; mean age, 58.7±8.9 years) and 56 healthy persons (26 males and 30 females; mean age, 54.7±8.3 years) participated in the study. Emotional state was evaluated by means of Profile of Mood State and quality of life by means of WHO Brief Quality of Life Questionnaire. Emotional state and quality of life were significantly worse, tension-anxiety and fatigue-inertia were significantly higher, vigor-activity was significantly lower in male patients with type 2 diabetes mellitus than in healthy males. In females, no significant differences in emotional state and quality of life comparing type 2 diabetes mellitus group and controls were detected. In females with type 2 diabetes mellitus, emotional state and quality of life were significantly better, scores of tension-anxiety, depression dejection, anger-hostility, and fatigue-inertia were significantly lower, and score of vigor-activity was significantly higher than in males with type 2 diabetes mellitus. Some significant correlations were found. In males, vigor-activity correlated with total cholesterol level and negatively correlated with triglyceride level. In females, significant correlations were found between scores of emotional state (tension-anxiety, depression-dejection, confusion-bewilderment, and total score of emotional state) and lipid levels (total cholesterol, triglyceride, and low-density lipoprotein cholesterol levels). There were no significant associations of emotional state and quality of life with duration of the disease in males and females with type 2 diabetes mellitus. No significant differences in emotional state and quality of life were found between males and females with type 2 diabetes mellitus, who were treated with oral antidiabetic preparations and insulin preparations.
