The most common malignant transformation of mature cystic teratoma of the ovary (MCTO) is squamous cell carcinoma, whereas, carcinoid tumor and papillary thyroid carcinoma are extremely extraordinary. This paper presents two rare cases of malignant transformation from MCTO. First, in a 33 year old, nulligravid, who underwent oophorocystectomy for a dermoid cyst. Histopathologic and Immunohistochemical studies revealed a carcinoid tumor arising from a mature cystic teratoma. Second, in a 52 year old who underwent, total abdominal hysterectomy with bilateral salpingo-oophorectomy for an ovarian new growth, with a final histopathologic and immunohistochemical analysis showed follicular variant of papillary carcinoma arising from mature cystic teratoma. Preoperative diagnosis of MCTO can be made through physical examination and imaging however not the concealed malignancy. Thorough extensive sampling and histopathologic examination of a dermoid cyst is necessary to detect malignant transformation. The optimal management of the disease is uncertain because of its rarity, but complete excision of tumor renders excellent prognosis.

Mature cystic teratoma is the most common ovarian germ cell tumor with a rare malignant conversion. We report a case of 36 years lady, with an incidental post-operative finding of papillary carcinoma of the thyroid in histopathology of the left ovarian dermoid cyst. Postoperatively she was evaluated for the presence of primary thyroid tumor with thyroid ultrasonography, thyroid function test, and anti-thyroglobulin antibody which revealed normal thyroid gland with euthyroid status and negative anti-thyroglobulin antibody, which had ruled out primary thyroid tumor. There are limited case reports on papillary thyroid carcinoma in a mature cystic teratoma in absence of a struma ovary and specific recommendations on the plan of its management are not available. This study aims to report a rare case of papillary thyroid carcinoma arising in a mature cystic teratoma of the ovary in absence of a struma ovary.

ABSTRACT Mature cystic teratoma or dermoid cyst constitutes about 10 to 20% of all ovarian tumors in the reproductive age group. Malignant transformation is seen in these tumors in about 1 to 2%. Squamous cell carcinoma (SCC) constitutes about 75 to 85% of malignant transformation. Imaging characters and serum tumor markers are two important modalities to differentiate benign and malignant lesions. We are presenting a rare case of SCC arising from mature teratoma. The aim of this presentation is to stress on the significance of preoperative risk assessment of SCC in mature cystic teratoma in postmenopausal age group for optimal treatment. Papillary thyroid carcinoma frequently metastasizes to regional lymph nodes. However, cervical lymph node metastasis as a sole manifestation of occult papillary thyroid carcinoma is rarely observed. Ectopic thyroid is an uncommon condition defined as the presence of thyroid tissue at a site other than pretracheal area. Approximately 1–3% of all ectopic thyroid tissue is located in the lateral neck. This entity may represent the only functional thyroid tissue in the body. Malignant transformation of ectopic thyroid is uncommon; but even rarer is the development of papillary carcinoma on it. We present a case of a 33-year-old man with an incidental lateral neck mass diagnosed after a motor vehicle accident. Total thyroidectomy and lymph node resection were completed without evidence of papillary thyroid carcinoma. Malignant transformation of heterotopic thyroid tissue was the final diagnosis. The possibility of an ectopic thyroid cancer should be considered in the differential diagnosis of a pathological mass in the neck. The uniqueness of this case strives in the rarity that the thyroid gland was free of malignancy, despite ectopic tissue being positive for thyroid carcinoma. Management strategies, including performance of total thyroidectomy, neck dissection, and treatment with radioiodine, should be based on individualized risk assessment.

We present a very rare case of malignant transformation of a benign mature cystic teratoma. The pathology report revealed malignant transformation of both the epithelial and sarcomatous elements of a benign dermoid cyst. To the best of our knowledge, this appears to be the third case of a malignant fibrous histiocytoma and a squamous cell carcinoma developing from a mature cystic teratoma. Malignant transformation of a dermoid cyst is usually diagnosed postmenopausally, but our patient was premenopausal. The etiology and prognosis of malignant transformation of this benign condition remain unknown.
