<p class="abstract"><strong>Background:</strong> <span lang="EN-IN">Management of fracture non-union of the neck of femur is age specific and is quite demanding. There is a need to salvage the femoral head</span><span lang="EN-IN">in younger individuals which demands a procedure which suits the requirements. The aim of this study was to evaluate the role of valgus osteotomy with dynamic hip screw fixation</span><span lang="EN-IN">(DHS) in the management of these fractures and to compare the results with studies of other authors as available in literature</span>.</p><p class="abstract"><strong>Methods:</strong> <span lang="EN-IN">This was a prospective study of 15 patients with fracture nonunion of the femoral neck conducted between January 2012 to January 2013 with a follow up period of 3 years</span>.<strong></strong></p><p class="abstract"><strong>Results:</strong> <span lang="EN-IN">There were 9 males and 6 female patients in our study with the right side being more commonly affected. The mean age of the patients was 45.2 years ranging from 36 to 58 years. All fractures united at the end of 6 months. Post operatively the mean Pauwels angle was 34<sup>0</sup> while the neck shaft angle was 135.2<sup>0</sup>. There was a significant increase in the Oxford score from a mean of 20.4 preoperatively to 37.9 in the postoperative period. All patients were happy with the procedure and the functional outcome. </span></p><p class="abstract"><strong>Conclusions:</strong> <span lang="EN-IN">Valgus osteotomy with DHS fixation is a good option for the management of fracture nonunion of the neck of femur in younger patients where there is a need to salvage the femoral head and it gives good functional results.</span></p>

Objectives: The present study aimed to record the femoral-neck shaft angle was carried out which would be of help to the orthopedicians while carrying out surgical repairs around the hip joint especially in fracture of the neck of femur. Materials and Methods: A total of 100 North Indian unpaired dry adult human femora (Right-50, Left-50) of unknown sex were studied. Neck-shaft angle (NSA) of the femur was measured with the help of goniometer and measurements were recorded in degrees. The raw data obtained were statistically analyzed. Range, mean, standard deviation, and standard error of mean were determined. Results: The mean NSA was found to be 127.63°+3.48° (Right=127.80°+3.56° and Left=127.46°+3.42°) with a range of 122°–137° (Right=122°–137° and Left=122°–136°). No significant difference in values of neck-shaft angle was found in the right and left femora. Conclusion: The overall goal of this study was to generate information that would be useful for geometric modeling of femora and collecting data which could prove useful for the development of prosthetic implants

Abstract Introduction: Evaluating the role of laparoscopic for diagnosis and treatment of undescended testis in children. Material and Methods: Restrospective study, between 6/ 2014 and 6/2017. All the patients are aged from 1 to 16 years with undescended testis underwent laparoscopic surgery for diagnosis and treatment in Deparment of pediatric surgery – Viet Duc hospital enrolled. Results: Of 95 patiens in total had 106 undescended testis diagnosed and treated by laparoscopy. The mean age of patients was 7,5 ± 3,8 years. 44,2% undescended were on the left side, 44,2% were on the right and 11,6% were undescended bilateral. The correct diagnosis by ultrasound accounted in 79,4%. The locations of testis diagnosed by laparoscopic are : intra abdomen in 45,3%, deep inguinal orifice in 16,9%, extra inguinal orifice in 26,4%, no testicle found in 11,4%. The mean time of operation were 67,33± 28,01 pht. Scrotal positions were achieved 74,5%, remove atrophic testis accounted in 7,6%. Stephen- Flowler technique including step I were in 4,7%, step II in 1,9%. The outcome evaluated by testicular positions following 3 months after operation are : good in 79,2%, moderate 13,2%, poor in 7,6%; By classification of Aubert are : good in 81,1%, moderate in 11,3% and poor in 7,6 %. Conclusion: Laparoscopic surgery is not only a highly sensitive diagnostic method to find accurately the location and size of the testes, but also the most effective method to treat impalpable undescended testes.

Objective Antioxidants have been used as a therapeutic measure for several causes of hearing loss, and this study aims to examine the use of antioxidants in children with congenital cytomegalovirus (cCMV)–related hearing loss. Study Design Case-control study. Setting Academic pediatric hospital. Subjects and Methods A retrospective chart review of pediatric patients with cCMV-related hearing loss treated with and without antioxidants (vitamins A, C, and E and magnesium, known as ACE-Mg) was completed. The primary end point was the mean change in hearing thresholds for the right and left ears after therapy. An evaluation of the mean change in thresholds was evaluated at the following frequencies: 500 Hz, 1000 Hz, 2000 Hz, and 4000 Hz. A 2-sample t test and multiple linear regression were used to evaluate the data. Results A total of 78 children with cCMV-related hearing loss were included in the study, of whom 10 were treated with antioxidants. The average amount of time in which antioxidants were taken was 387 days. When comparing cases and controls, there was no differences in the mean change of hearing thresholds at each frequency for both the right and left ears ( P > .05). Length of antioxidant therapy and age at which therapy was initiated had no effect on hearing scores ( P > .05). Conclusions Oxidative stress plays a role in the pathogenesis of cCMV-related hearing loss. ACE-Mg is a safe adjuvant therapy for the treatment of hearing loss in children; however, this study demonstrates no hearing-related benefit from ACE-Mg antioxidant therapy.

Objective: This paper analyzes a series of ultrasound (US)-guided orbital fine needle aspirations (FNAs) which provide diagnostic information that cytopathologists approaching orbital lesions for the first time can find useful and underlines the importance of teamwork. Study Design: The investigators retrospectively obtained data from 24 consecutive orbital FNAs. For all patients, a complete clinicoradiological database was created. FNAs were performed under US guidance with 25-gauge needles and an aspiration biopsy syringe gun, and sent to the Department of Pathology for examination and data management. Results: The mean age of the patients was 54 years. Imaging studies included US, magnetic resonance imaging and computed tomography scans; 9 lesions involved the right orbit and 15 the left orbit. The mean lesion size was 23.6 ± 7.2 mm. After microscopic examination, 7 smears were labeled as ‘nondiagnostic', while in 17 cases a definitive diagnosis was proposed, which always proved to be correct (70.8%, specificity = 100%). Conclusions: The investigators believe that FNA biopsy of orbital masses is a necessary step; its weaknesses lie in the particularly delicate site of sampling and the extreme heterogeneity of lesions. Nevertheless, when orbital FNA is performed within a well-coordinated multidisciplinary team, it is a powerful tool that can be used to define the most appropriate management of these patients.

When it comes to zakat obligations, people only know certain types of zakat that are obligated to be issued. There are two types of zakat; zakat fitrah and zakat property. However, zakat’s division may vary according to the state's fatwa. In Selangor, Lembaga Zakat Selangor (LZS) is the body that governs the administration and implementation of Zakat. The breakdown of zakat property is Zakat on Income, Zakat on Business, Zakat on Savings, Zakat on Gold and Silver, Zakat on Investments and Shares, Zakat on KWSP, Zakat on Paddy, Zakat on Agriculture, Zakat on Wealth and Treasure, and the Zakat on Takaful. However, this study will only focus on Zakat Takaful. Under Zakat Takaful, there are several divisions which are Zakat Wang Takaful/Takaful Investment, Zakat on Maturity Policy, Takaful Compensation, Tabarruq Fund, Benefit of Total and Permanent Disability, and Compensation of Conventional Insurance Policy. Each of these sections has its calculation method which will be explained further. This study was conducted to identify the level of knowledge and acceptance of Takaful Muslim consumers on the implementation of Zakat Takaful that has been introduced by LZS. Besides, this study will also look at the knowledge of Takaful agents and their willingness to disseminate information of Zakat Takaful to their customers. This study was conducted by using the sampling method and descriptive data analysis. The study found that on average, respondents' knowledge on Zakat Takaful is low with a mean of 2.32. However, the mean for acceptance is 3.93 which is set at a high level thus showed a good acceptance of the survey respondents. Through this study, it can be concluded that knowledge and acceptance are both very important in achieving the objective of implementing Zakat Takaful. Knowledge by Takaful participants who are also assisted by agents who play the role of conveying knowledge of Zakat Takaful is directly perceived to have a more effective effect in addition to the behavior/attitude that forms the intention of Takaful participnts to accept the implementation and to pay Zakah Takaful. This study is also expected to be the push factor among the zakat agents to play a role in disseminating the right information about Zakat Takaful to their customers, which also can be regarded as one of the da'wah forms.

This study was aimed to find out the variations of the femoral head diameters in relation to sex in Bangladeshi people. The study was based on 123 paired human femurs (radiological view) of which 52 were male and 71were female. It was observed that the mean vertical and transverse diameters of the head of the male femur were significantly greater than that of female (P<0.001).No statistical significant difference was present between the measurement of the right and left side of the same sixth identification points that were derived from the vertical and transverse diameters of the head of the femur were higher in males than in females. The results showed that diameters of the femoral head as well as the identification points that are derived from them are of use in sex identification. The present study is of value to the forensic experts, orthopaedicians and prosthetists as it gives the idea about the dimensions of the femoral head in the present area.   doi: 10.3329/taj.v18i2.3172 TAJ 2005; 18(2): 84-88

Background: Femoral neck fracture nonunion is a frequent and challenging complication, with several surgical options. Objectives: The present study reported the outcome of valgus osteotomy for treating femoral neck nonunion in patients aged <65 years. Methods: This case series research included young patients who underwent valgus osteotomy for treating femoral neck nonunion or device failure of the index surgery. The fixation of the osteotomy site was performed using either a dynamic hip screw or a fixed angle blade. The union of the neck and osteotomy site (subtrochanteric region) was evaluated by monitoring callus formation in the postoperative radiographs. Limb length discrepancy, Pauwels’ angle, and the neck-shaft angle were evaluated before the surgery and at the last follow-up. Postoperative complications were extracted from the explored patients’ profiles. Results: Twenty patients with a Mean±SD age of 27.2±10.8 years were examined in this study‎. The Mean±SD follow-up time of the patients was 6.1±3.9 years. The femoral neck was united in 18 patients in a Mean±SD period of 5.1±2.3 months. The Mean±SD limb length discrepancy was measured as 1.3±0.8 cm before and 0.5±0.7 cm after the ‏osteotomy. The Mean±SD Pauwels’ angle was calculated as 65.5±16.9º before and 32.4±16.2º after the osteotomy. The Mean±SD neck-shaft angle was computed to be 121.9±22.8º before and 144.5±17.7º after the osteotomy. Revision surgery was performed for 6 (30%) patients. This measure included device removal in 4 and total hip arthroplasty in 2 patient. Conclusion: Valgus osteotomy is a safe and efficient technique for managing femoral neck nonunion. Therefore‎, this approach is suggested as a good alternative for total hip arthroplasty, particularly in young and active patients‎.

Background. In models of temporal lobe epilepsy and in patients with this pathology, high frequency oscillations called fast ripples (FRs, 250–600 Hz) can be observed. FRs are considered potential biomarkers for epilepsy and, in the light of manyin vitroandin silicostudies, we thought that electrical synapses mediated by gap junctions might possibly modulate FRsin vivo.Methods. Animals with spontaneous recurrent seizures induced by pilocarpine administration were implanted with movable microelectrodes in the right anterior and posterior hippocampus to evaluate the effects of gap junction blockers administered in the entorhinal cortex. The effects of carbenoxolone (50 nmoles) and quinine (35 pmoles) on the mean number of spontaneous FR events (occurrence of FRs), as well as on the mean number of oscillation cycles per FR event and their frequency, were assessed using a specific algorithm to analyze FRs in intracranial EEG recordings.Results. We found that these gap junction blockers decreased the mean number of FRs and the mean number of oscillation cycles per FR event in the hippocampus, both during and at different times after carbenoxolone and quinine administration.Conclusion. These data suggest that FRs may be modulated by gap junctions, although additional experimentsin vivowill be necessary to determine the precise role of gap junctions in this pathological activity associated with epileptogenesis.
