Horizontal maxillary ridge augmentation with ridge split procedure and simultaneous implant placement: A case report

Treatment of edentulous maxillary anterior region with horizontal ridge atrophy presents a clinical situation in which the placement of endosseous implants might be complex or sometimes impossible without a ridge augmentation procedure. This case report presents management of horizontally deficient maxillary anterior ridge with ridge split procedure using piezotome and simultaneous implant placement. In contrast to other ridge augmentation techniques, ridge splitting allows for immediate implant placement following surgery reducing the overall treatment time and eradicates the possible morbidity from a second surgical site.

Immediate implant placement combined with hard and soft tissue grafting has been suggested because it may preclude dramatic postextraction bone loss and may decrease overall discomfort with reduction in the number of surgeries and in treatment time. In this case report, the acellular dermal matrix was used as a membrane for immediate implantation via a bone augmentation procedure in an esthetically challenging situation. The author suggests that this surgical technique provides the clinician with an option for an immediate implant therapy when primary closure is not intended. Additional randomized controlled trials conducted over long periods are necessary to establish whether this procedure offers long-term benefit to patients.

Relevance. Immediate implant placement with immediate temporisation is method of choice in case of central incisors extractions. In nowadays there are many protocols aimed on optimisation of the immediate prosthetic result. Some of them are based on using CAD/CAM technology, others stress the importance of peri-implant grafting procedures as a means of clinical success. The aim of the study is to analyse tactic of treatment in cases of immediate implant prosthetic replacement of maxilla central incisors using assessment of clinical example and elaboration of the optimal treatment option. Materials and methods. We conducted the evaluation of the 54 year old male patient with the use of clinical and paraclinical examination methods (OPG, CBCT). Implant placement in the position of the teeth 1.1, 2.1 was performed utilising new NobelParallel Conical Connection TiUltra (NobelBiocare, USA) 4.3*13 mm followed by implant supported Procera Zirconia prosthesis. We used international indices PES and it’s modification WES for the assessment of the achieved results. Results. The final result received following scores: PES index = 7, WES index = 8. Conclusion. Choice of implant design is a major influencing factor on the immediate implant placement prognosis. Utilising Nobel Active implant in the described clinical situation combining its placement with grafting of the connective tissue taken from the tuberosity area led to the decrease in total treatment time and achievement of the superior esthetic and functional result.

Abstract Insufficient width of the alveolar ridge often prevents ideal implant placement. Guided bone regeneration, bone grafting, alveolar ridge splitting and combinations of these techniques are used for the lateral augmentation of the alveolar ridge. Ridge splitting is a minimally invasive technique indicated for alveolar ridges with adequate height, which enables immediate implant placement and eliminates morbidity and overall treatment time. The classical approach of the technique involves splitting the alveolar ridge into 2 parts with use of ostetomes and chisels. Modifications of this technique include the use of rotating instrument, screw spreaders, horizontal spreaders and ultrasonic device. The purpose of this article is to thoroughly describe all the different approaches in ridge splitting technique. 2 interesting clinical cases of narrow alveolar ridges treated with ridge splitting and immediate implant placement are also presented.

The nature of immediate implant placement followed by an immediate restoration protocol makes it particularly suited to the anterior maxilla. In addition to saving treatment time and avoiding additional surgical procedures, this protocol has been reported to improve aesthetic outcomes by supporting the peri-implant tissues during the implant healing phase through the use of a provisional restoration. This case report documents the use of this protocol in a patient with a failing maxillary anterior tooth and reports on the soft and hard tissue changes over an observation period of 10 years. An implant was immediately placed after removal of a failing maxillary central incisor followed by the provision of a screw retained provisional crown on the same day. A definitive restoration was placed after a 3-month healing period. Not only did this protocol manage to maintain peri-implant bone levels over the 10-year follow-up period, excellent aesthetic outcomes and very limited soft tissue recession were observed with the use of this technique.

Background: The concept of immediate implant loading has recently become popular due to less trauma, reduction in overall treatment time, decrease in hard and soft tissue resorption, increase in patient’s acceptance, along with better function, aesthetics and has a psychological satisfaction to the patient. The purpose of this study is to evaluate the results of implant placement immediately after tooth extraction. Materials and method: The study consisted of 32 patients with 43 implants have been placed immediately after tooth extraction in the Odonto-Maxillo-Facial Hospital of Ho Chi Minh City. Evaluation results after dental implants 1 week, 1 month, 3 months, 6 months. Results: Good clinical results after 1 week was 55.8%, after 3 months and 6 months were 100%. After 1 week of implant placement, good healing took 59.4%. After 1 month, 3 months and 6 months, 100% of the patients are well healed. After 3 months, 6 months without any implant has the status of shaky. Most dental implants have good levels of bone graft. The level of good after 3 months of implants was 72.1% and after 6 months all cases are good. The success rate for dental implants was up to 97.7%, failing only 2.3%. After 3 months the implants had a good level of 72.1% and after 6 months, all cases were good. Patients with success in implant placement accounted for a high rate of 97.7%, failure only accounted for 2.3%. Conclusions: Success rate in immediate implant is high. It is necessary to develop this method to restore lost teeth to achieve optimum results. Key words: Dental implant, immediate implant after tooth extraction

INTRODUCTION: Immediate dental implants have greatly reduced the treatment time and the number of surgical interventions. Recently it has been noted that this treatment modality can be used in aesthetically demanding cases especially the anterior maxilla. CASE REPORT: In the present case report a 24 year old male patient reported to our institution with fractured upper front teeth. After careful examination and treatment planning immediate implant treatment was initiated. The teeth were extracted atraumatically. We placed three implants into the extraction sockets. The prosthetic rehabilitation was done with metal ceramic crowns. CONCLUSION: It was found that the immediate implant therapy has several advantages such as reduced treatment length, preservation of soft and hard tissues surrounding the implant and reduced number of operations. Immediate implant treatment therefore has a great future in the treatment of aesthetic zones.

Narrow dentoalveolar ridges pose a major challenge for the successful placement of endosseous implants. This case report focuses on a Piezoelectric driven mandibular Ridge Split technique without vertical osteotomy for an immediate implant placement in a narrow alveolar ridge measuring 3.0mm. Following anaesthesia, a mucoperiosteal flap was elevated after giving a mid-crestal incision distal to 35 along the entire edentulous ridge. Then, with the help of a micro-saw and horizontal spreaders, an osteotomy site of required diameter was achieved. Finally twist drills were used and implants were placed in the expanded site. Clinically, healing was uneventful with no step defect in the expanded buccal bone and the final occlusion obtained was satisfactory. The Piezo-electric driven Ridge Split technique promises to be a minimally invasive option for horizontal augmentation of narrow alveolar ridges- predictability within a short interval of time and with minimal risk of fracture.

Implant placement becomes complicated when the height and width of the bone is compromised especially in the posterior maxillary region due to the presence of sinus. This case report presents the rehabilitation of the left maxillary premolar region by using indirect sinus lift and ridge splitting followed by immediate implant placement where the bone height and bone width was compromised. It helps in reducing the treatment time and increases the patient comfort.
