Background: Cubitus varus deformity is a common complication of untreated elbow fractures in children. However, cubitus varus in osteogenesis imperfecta (OI) children is a rare but challenging situation. To the author's knowledge, this is the first study discussing the correction of cubitus varus deformity in patient with OI.Case Presentation: Here we report a case of a 7-year-old OI girl with cubitus varus deformity due to a supracondylar fracture of humerus 3 year ago. The patient's parent gave a history of supracondylar fracture of left humerus in 2015. Without medical intervention, the patient was admitted into our institution for corrective surgery with the diagnosis of osteogenesis imperfecta and cubitus varus deformity in the left arm.Result: Medications including calcium, vitamin D and bisphosphonates were administered before the corrective surgery of cubitus varus, and a single locking plate was used to fixate the osteotomy. After the surgery, the appearance and range of motion (ROM) of the left arm was almost normal. Combined with gradual rehabilitation, the ROM of the left arm was normal without pain during daily use within the 1-year follow up. The hardware was removed as the nailing of the forearm fractures was performed at the same time. In the latest follow-up in September 2021, the appearance and ROM of the left arm was normal.Conclusion: Cubitus varus is a common deformity in children with elbow injuries, but it presents a challenging situation in compound fractures in OI patients. Locking plate combined with meticulous pharmacological intervention provides a good option for corrective surgery of cubitus varus in patients with OI.

Research has shown that social capital plays a significant role in achieving success. For children from disadvantaged communities in India—such as the urban poor—financial and cultural constraints severely limit their ability to complete their schooling, forcing them to enter the job market as unskilled workers. This chapter presents an educational intervention called Project Disha in an urban slum by Kriti Social Initiatives, a non-profit organization in India. Project Disha is a scholarship and mentoring program that aims to teach life skills and fill the social capital gap for these disadvantaged students. Due to the pandemic, the program switched to an online mode, an already challenging situation made more so due to the extremely limited technology available to the children. With limited internet connectivity, frequent power cuts, and access only to inexpensive smartphones, Project Disha now relies on a team of volunteer mentors and creative pedagogical strategies to keep students connected, engaged, and learning.

Managing the acute phase after a severe traumatic brain injury (TBI) with polytrauma represents a challenging situation for every trauma team member. A worldwide variability in the management of these complex patients has been reported in recent studies. Moreover, limited evidence regarding this topic is available, mainly due to the lack of well-designed studies. Anesthesiologists, as trauma team members, should be familiar with all the issues related to the management of these patients. In this narrative review, we summarize the available evidence in this setting, focusing on perioperative brain protection, cardiorespiratory optimization, and preservation of the coagulative function. An overview on simultaneous multisystem surgery (SMS) is also presented.

Abstract This paper addresses the challenging situation that was faced by Maintenance Teams in Offshore Sites to maintain and ensure the optimal conditions of the plants, conducting Routine Preventative Maintenance (RPM) as planned, achieving Company and Operational Excellence KPIs in the midst of COVID-19 pandemic. INTRODUCTION: COVID-19 pandemic has caused disasters and disorders all over the world. India is also going through a challenging situation as the number of infected/positive cases is increasing day by day. Due to the crisis of COVID-19 education system has come to a halt as schools and colleges are closed to slow down and prevent the spread of infection. Nursing institution also closed down and online mode of teaching was done. Hands on training is required to relate theory in to practice which was thwarted due to the pandemic COVID-19. Nursing students were panic and apprehensive about the situation even though they were asked to stay in the hostel. OBJECTIVE: To assess the level of stress and anxiety among the nursing students and to associate the level of stress and anxiety with selected demographic variables. METHODS: The study design was descriptive cross-sectional. The study was conducted in College of Nursing CIHSR among the 3rd Year GNM, 3rd Year and 4th Year B. Sc. Nursing Students. Total enumerative sampling method was used and sample size was 95. Standardized instrument on Perceived stress scale for assessing stress and Corona anxiety scale for assessing anxiety regarding COVID – 19 were used for the study. The study was approved by nursing research committee and informed consent was taken. Data was analysed using descriptive and inferential statistics. RESULTS: The study found that 80(84.2%) had moderate stress out of 95 respondents, 8 (8.4%) had high perceived stress, and 7(7.4%) had low level of stress. In the Coronavirus anxiety scale majority of the respondents, 91(95.8%) have little to no dysfunctional anxiety out of 95 respondents, 2 (2.1%) had possible dysfunctional anxiety and 2 (2.1%) had probable dysfunctional anxiety. There were no significant association between level of stress, anxiety and demographic variables. CONCLUSION: Study reveals that majority of the students had moderate level of stress and little to no dysfunctional anxiety regarding COVID - 19. Thus leaflet regarding cause, spread, prevention and management of COVID-19 given to each student to reduce their stress and anxiety.

<p>When a large public building is constructed within any landscape it is often found to create adverse affects on the public life surrounding it. This investigation focuses on Te Papa in Wellington, New Zealand. It employs design strategies that emerge from site observation, analysis and exploration of design and behaviour literature. Site observations are undertaken at various scales to determine the behaviours that occur in and around the site, and analysis of this provides an understanding of why such actions can happen and why others cannot. Exploration of the literature is employed to understand past approaches to this design problem and to inform the analysis process. Design testing allows ideas to be played out in hypothetical scenarios and the outcomes of which will be compared to past approaches.  There is a strong focus on pedestrian movement as a catalyst of public life, in relation to the movement itself and the interactions people have with the surrounding environment when influenced by the movement flow. The overall goal of this thesis is to observe and analyse a large building in the public realm that, although is surrounded by movement, limits engagement in ways that results in a decrease of public life. The research leads to the exploration of how flows can be coordinated to generate eddies of interaction and pause and ways to activate and open up particular edges of this building in order to create new opportunities for the public to engage with the landscape. It finally attempts to find ways to not only create new public life, but also intensify the public life in this challenging situation.</p>

AbstractMass spectrometry-based proteomics allows to study all proteins of a sample on a molecular level. The ever increasing complexity and amount of proteomics MS-data requires powerful and yet efficient computational and statistical analysis. In particular, most recent bottom-up MS-based proteomics studies consider either a diverse pool of post-translational modifications, employ large databases – as in metaproteomics or proteogenomics, contain multiple isoforms of proteins, include unspecific cleavage sites or even combinations thereof and thus face a computationally challenging situation regarding protein identification. In order to cope with resulting large search spaces, we present a deep learning approach that jointly embeds MS/MS spectra and peptides into the same vector space such that embeddings can be compared easily and interchangeable by using euclidean distances. In contrast to existing spectrum embedding techniques, ours are learned jointly with their respective peptides and thus remain meaningful. By visualizing the learned manifold of both spectrum and peptide embeddings in correspondence to their physicochemical properties our approach becomes easily interpretable. At the same time, our joint embeddings blur the lines between spectra and protein sequences, providing a powerful framework for peptide identification. In particular, we build an open search, which allows to search multiple ten-thousands of spectra against millions of peptides within seconds. yHydra achieves identification rates that are compatible with MSFragger. Due to the open search, delta masses are assigned to each identification which allows to unrestrictedly characterize post-translational modifications. Meaningful joint embeddings allow for faster open searches and generally make downstream analysis efficient and convenient for example for integration with other omics types.Availabilityupon [email protected]

Abstract The COVID-19 pandemic is a challenging situation for many older adults at elevated risk for mortality. Social distancing and lockdown to prevent contagion may result in social isolation and feelings of loneliness, which can have adverse effects on health. We examined how depressive symptoms were associated with between-person differences and within-person variations in loneliness, social contacts, and daily physical problems during 8 weeks. We sampled 247 older adults (Mage = 71.1, SD = 7.3, range = 51 - 95), who participated at micro-longitudinal online surveys (baseline and 7 weekly follow-ups) from April 28 to June 23. Multilevel modeling analysis controlling age, gender, marital status, and education showed that depressive symptoms were significantly decreased during 8 weeks. Further, depressive symptoms were positively coupled with both loneliness and physical problems for both the within-and between-person levels. Increase in social contact was related to decreases in depressive symptoms only at the between-person level.
