The frequency of separation of NiTi rotary instruments has increased with the increasing use of these instruments, and this error may result from inadequate experience of using these instruments. This iatrogenic error can complicate root canal treatment, and special experience is required to manage this problem. This case report describes the management of the separation of an F3 instrument from a ProTaper NiTi rotary system. A 30-year-old man was referred to the College of Dentistry, Taibah University (Saudi Arabia) with an intracanal separated NiTi rotary instrument (F3 of the ProTaper system; Dentsply Maillefer, Ballaigues, Switzerland) in the canal of the maxillary right canine. The separated instrument was removed by using the modified hollow tube–based extractor system. The present case revealed that sometimes a simple method can be an effective alternative of a professional method; using NiTi rotary instruments with strict adherence is important for the prevention of instrument separation.

Objective: The present study was planned with an aim to evaluate the knowledge and awareness regarding root canal treatment in a sample of Saudi population. Material and methods: A cross-sectional questionnaire-based survey was conducted among a sample of participants in the Saudi population. Participants were asked questions to assess their knowledge about endodontic treatment. Results: A total of 568 patients (343 males and 225 females) participated in the study. When the patient’s responses were analyzed regarding endodontic treatment, it was found that 68.55 % of participants had a good experience of root canal treatment in the past. 27.9% of the respondents describe their knowledge of root canal treatment as average. Spontaneous toothache was considered the most important factor to seek endodontic treatment in majority (40.1%) of the participants and 86.6% of the participants preferred a specialist to undergo a root canal treatment. Conclusion: Pain associated with root canal treatment was the most important factor that dithered the surveyed participants from root canal treatment. The results of the survey reveal an improvement of knowledge and awareness of patients about root canal treatment

ABSTRACT Objective: This study examined the endodontic experience, perceptions of endodontic practice, and self-rated confidence of dental students enrolled in Taibah University, Saudi Arabia. Materials and Methods: A questionnaire was distributed to 41 undergraduate dental students registered in endodontic courses in the 2015 academic year. The questionnaire evaluated their confidence performing nonsurgical root canal treatment. The level of confidence was classified using a 5-point scale as “very confident,” “confident,” “neutral,” “not very confident,” or “not at all confident.” The data were analyzed using SPSS version 20.0 (SPSS, Chicago, IL, USA). Results: The participation rate was 93%. The maxillary incisor was the most common first tooth treated. The students were relatively confident, but their confidence levels were lower regarding endodontic radiology, evaluation of root canal obturation, and determining the correct recall period for the patient. Conclusion: The confidence of undergraduates in endodontics must be enhanced to increase their clinical competence when performing root canal treatment.

Introduction. Coronal leakage is detrimental to the long-term success of root canal treatment (RCT). While much emphasis is being placed on the quality of the final restoration, little attention is given to the temporary restoration placed in between root canal treatment appointments. The aim of this study was to survey temporization techniques practiced in Saudi Arabia and the frequency of observing temporary material breakdown or complete loss. Materials and Methods. An online questionnaire was distributed among general dentists, dental specialists, and clinical trainees in undergraduate and postgraduate dental programs. The sample size was estimated at 370 participants. Data were analyzed using descriptive statistics and chi-square tests. Results. The total number of participants who met the inclusion criteria was 525. The majority of them (94.6%) were practicing two-visit RCT. The most common temporization materials were Cavit (50.3%) followed by glass ionomer cement (32%). The majority (72.6%) of participants claimed they allow a thickness of 2-3 mm for temporary restorations. Many participants (60.4%) used a spacer material during temporization, and the cotton pellet was the most common spacer material. Temporary restoration breakdown or complete loss was a common observation. Although the duration between the two RCT visits was 2 weeks or less for 83.6% of participants, only 19.6% of participants claimed that they rarely observed temporization breakdown. Conclusion. Two-visit RCT is commonly practiced in Saudi Arabia, and endodontists performed significantly more single-visit procedures. Temporization practices may lack uniformity; however, clinicians were more likely to use calcium sulfate-derived material for two weeks or less. They allow for 2-3 mm thickness restoration and use a cotton pellet as a spacer. According to their clinical observation, temporary material breakdown or complete loss was frequent. This mandates further attention in research and education.

Abstract- - Microorganisms and their by-products are considered to be the major cause of pulp and peri-radicular pathologies, and for the dis-infection of these canals we need proper shaping, cleaning of these canals with mechanical instruments and chemical irrigants. Here we are discussing about the chlorohexidine with a concentration of 2% in root canal treatment. It has been found that there is a highly significant reduction in the number of microorganisms in the chlorhexidine-treated specimens after instrumentation and irrigation. Here we will discuss the merits and de-merits of chlorhexidine and its use in root canal as an irrigant or medicaments.

A Patient came with the complaint of pain in the lower right back region of the jaw. Root canal treatment was planned. While preparing for the bio-mechanical procedure, the Hand pro taper fractured in the apical third. Iatrogenic occurred as a result of the fracture of the endodontic instrument. Retrieval of the fractured instrument was planned to complete the cleaning and shaping of the canal. The removal of the fractured instrument was planned to be done under the Dental Operating Microscope. The use of an operating microscope enhanced the illumination and the magnification of the instrument. This illumination and magnification helped in the precision of removal. The ultrasonic tip enabled to reach of the fractured instrument in the canal and loosen the dentin around the fractured instrument. It allowed easy retrieval of the fractured instrument. During the retrieval procedure, the fractured instrument was bypassed before the use of the ultrasonic tip. After the removal of the fractured instrument, cleaning and shaping were completed, followed by obturation, definitive restoration, and prosthesis. As the removal of the fractured instrument enabled complete cleaning and shaping, it improved the prognosis of the case. When the endodontic instrument gets fractured, it should be analyzed over the radiograph to assess the fracture level, the anatomy of the root canal, size of the fractured instrument, check accessibility, stage of fracture, etc. If all the above criteria are met with the removal of the instrument only then, replacement should be tried. Otherwise, it may lead to a severe loss of root dentin, decreasing fracture resistance of the root.
