Subject. Pulp tissue diseases are characterized by a wide prevalence, which makes it possible to consider the quality of endodontic dental treatment one of the most urgent problems of modern dentistry. High-quality treatment of complicated forms of caries is the prevention of odontogenic inflammatory processes, which include periodontitis, periostitis, osteomyelitis, abscess and phlegmon. Therefore, new tools and methods of treatment of complicated forms of caries are proposed, aimed at long-term preservation of teeth as a functional unit of the dental system. Objective: to analyze modern laser technologies in the treatment of complicated forms of caries. Materials and methods. The study was based on the search and study of original articles on the endodontic treatment of complicated forms of caries using laser radiation in the databases: Russian state library, eLibrary, PubMed, the Cochrane Library, Google Scholar. The main selection of materials was carried out by keywords. Results. The literature review describes the positive properties of laser radiation, its mechanism of action, the basics of its use in endodontic treatment, and its application in various branches of dentistry. Conclusions. Analyzing the literature data, we can conclude that the use of laser technologies in the treatment of complicated forms of caries is promising. It is possible to achieve high-quality cleaning of the root canal system and the best long-term results. However, the literature does not sufficiently cover the use of Er, Cr:YSGG laser with a wavelength of 2780 nm in the treatment of purulent pulpitis and pulp necrosis. In addition, taking into account the possibility of using lasers at a therapeutic appointment, it is possible to significantly improve the professional level of a dentist, improving the quality of dental care, reducing the number of complications and preventing various dental diseases.

Abstract Purpose: Advances in surgical techniques and perioperative care have improved patients' short-and mid-term postoperative outcomes with Biliary Atresia (BA). However, the long-term results of these patients have not been thoroughly investigated. This systematic review aims to determine the long-term outcomes and the patients' health-related Quality of life (HrQoL) with their native livers or liver transplantation. Methods: A systematic literature-based search for relevant cohorts was performed using Pubmed/Medline, Cochrane Library from its inception to August 2021. Original studies reporting on BA, Hepatoportoenterostomie, portoenterostomy, Kasai, Liver transplantation, Quality of life, or HrQoL were included. Pooled prevalence has been calculated for cholangitis, secondary liver transplantation, or associated malformations using MetaXL (version 5.3). Subgroup analysis on HrQoL followed surgical treatment after BA was calculated by using RevMan (version 5.4).Results: 12 articles were considered for data synthesis. Nine studies compared biliary atresia patients to an age-matched healthy reference group. 4/9 (n = 338) of these studies indicated lower scores for biliary atresia patients; 5/9 (n = 127) stated similar health status. A Forest plot analysis including all studies with total HrQoL showed a tendency of higher scores towards healthy controls (MD -0.79, 95% CI: -6.00-4.41). Comparing patients after Kasai Hepatoportoenterostomy with healthy controls demonstrated favorable outcomes for the control group (MD -3.22, 95% CI: -7.20-0.75) with no statistical significance (p = 0.11). The pooled estimation of the prevalence of cholangitis, secondary liver transplantation and associated malformations are 0.33 (95% CI: 0.06–0.66), 0.59 (95% CI: 0,42–0.75) and 0.13 (95% CI: 0,01–0.33).Conclusion: Biliary atresia patients have an overall high prevalence of progressive liver-related complications and risk of lower HrQoL compared to their healthy peers. Furthermore, those patients who received liver transplantation appear to have the same Quality of life as those living with their native livers. Targeted and evidence-based follow-up procedures and transitional care are essential to meet these patients' long-term care needs. Prospective and multicenter research das focuses on the attributes and predictors of the long-term prognosis of patients with biliary atresia are necessary.

The aim – to review and analyze the data on the long-term population-based efficiency of the screening procedures and other measures implemented according to Stage 1 periodical health checkup in the Russian Federation (by Order №36ан of 2015).Material and methods. This systematic search was carried out independently by two authors who used four databases: Scientific electronic Library, Cochrane Library, Medline/PubMed, and USPSTF. They looked into systematic reviews quoted by these sources. The criteria of the screening efficacy were the total / specific mortality and disability. The quality of the systematic reviews was assessed by the two authors using the international AMSTAR questionnaire.Results. According to the specific mortality rate, only the fecal occult blood test and the abdominal aorta ultrasonography are found to be efficient as screening methods for certain age and gender groups. The methodological quality of the studied reviews is high or medium to high. As follows from these high quality studies, the efficacy of mammography in female populations remains to be proven.

The widespread prevalence of pulp and periodontal diseases indicates the need for continuous improvement of the method of endodontic treatment and determines the relevance of the development of new methods of complex therapy. The primary etiologic agents of apical periodontitis are microorganisms and their by-products that have invaded the pulpal space and established multispecies biofilm communities in the root canal system. Biofilms are involved in all stages of root canal infection and can be found on root canal walls, in dentinal tubules, and on extraradicular surfaces. The success of endodontic dental treatment is determined by careful mechanical processing using modern instruments, drug treatment and subsequent three-dimensional hermetic obturation of the root canal. Only a high-quality and optimal solution to the three problems allows you to achieve high-quality long-term results of treatment. Instrumentation disrupts biofilms which colonize infected soft and hard tissues and provides access for irrigation and exposure to antimicrobial solutions for disinfection of the root canal system. Disinfection is achieved by the use of both antimicrobial agents and the mechanical flushing action of irrigation, with the goal being the disruption, displacement and removal of pulpal remnants, microorganisms, metabolic byproducts, debris and the smear layer created during instrumentation. The multistage, duration and laboriousness of drug treatment of root canals makes it not always effective, which can subsequently cause unsuccessful endodontic treatment. A practicing dentist should be able to rationally and efficiently utilize standard disinfection protocols in the irrigation and medication of root canal spaces.

The immediate and long-term results of treatment of 30 patients with severe pain syndrome of the lumbar region, who underwent operations on denervation of DOS, were studied. The aim of the study was to evaluate the effectiveness of minimally invasive technologies for the treatment of pain in the lumbar region (denervation of DOS), to study the near and distant results of these treatment methods. Denervation DOS is an effective minimally invasive method for the treatment of facet syndrome caused by spondylarthrosis. It allows in the early and distant postoperative periods to significantly reduce the pain syndrome and improve the quality of life of patients.

This paper describes the Passive microwave Neural network Precipitation Retrieval algorithm for climate applications (PNPR-CLIM), developed with funding from the Copernicus Climate Change Service (C3S), implemented by ECMWF on behalf of the European Union. The algorithm has been designed and developed to exploit the two cross-track scanning microwave radiometers, AMSU-B and MHS, towards the creation of a long-term (2000–2017) global precipitation climate data record (CDR) for the ECMWF Climate Data Store (CDS). The algorithm has been trained on an observational dataset built from one year of MHS and GPM-CO Dual-frequency Precipitation Radar (DPR) coincident observations. The dataset includes the Fundamental Climate Data Record (FCDR) of AMSU-B and MHS brightness temperatures, provided by the Fidelity and Uncertainty in Climate data records from Earth Observation (FIDUCEO) project, and the DPR-based surface precipitation rate estimates used as reference. The combined use of high quality, calibrated and harmonized long-term input data (provided by the FIDUCEO microwave brightness temperature Fundamental Climate Data Record) with the exploitation of the potential of neural networks (ability to learn and generalize) has made it possible to limit the use of ancillary model-derived environmental variables, thus reducing the model uncertainties’ influence on the PNPR-CLIM, which could compromise the accuracy of the estimates. The PNPR-CLIM estimated precipitation distribution is in good agreement with independent DPR-based estimates. A multiscale assessment of the algorithm’s performance is presented against high quality regional ground-based radar products and global precipitation datasets. The regional and global three-year (2015–2017) verification analysis shows that, despite the simplicity of the algorithm in terms of input variables and processing performance, the quality of PNPR-CLIM outperforms NASA GPROF in terms of rainfall detection, while in terms of rainfall quantification they are comparable. The global analysis evidences weaknesses at higher latitudes and in the winter at mid latitudes, mainly linked to the poorer quality of the precipitation retrieval in cold/dry conditions.

<p class="abstract"><strong>Background:</strong> The primary goal of this study was to evaluate the long-term influence of tonsillectomy on the quality of life and voice performance of opera singers. Survey study which was reviewing the long-term effects of tonsillectomy.  </p><p class="abstract"><strong>Methods:</strong> Retrospective review of patients’ records and surveys in which patients have answered the questions about the influence of tonsillectomy on their voice. A total group of 30 singers was included in the study. They answered the questions about the indications for surgery, symptoms, and changes in their voice after surgery. The study group consisted of 19 women and 11 men. Mean age was 18.53 years old at the moment of surgery. The group included eleven sopranos, six mezzos, two contraltos, four tenors, five baritones, and one bass singer.  </p><p class="abstract"><strong>Results:</strong> A most common indication for tonsillectomy in opera singers was recurrent tonsillitis and was present in over 83% of cases. After surgery, the voice performance was better in 60% of cases, breathing improved in 50% of cases, voice emission was higher in 53% of cases. Over 80% of singers would recommend that surgery to another opera singer.</p><p class="abstract"><strong>Conclusions:</strong> Singers evaluated long-term influence of tonsillectomy as positive. Over 80% admitted improved effect on emission and the function of voice.</p>

Extrapyramidal symptoms (EPS) are movement disorders due to side effects of dopamine receptor blocking agents. Symptoms of EPS include dystonia, akathisia, and parkinsonism. Symptoms of EPS in this case are found in normal pressure hydrocephalus (NPH) which does not consume dopamine receptor blocking drugs. Normal pressure hydrocephalus is hydrocephalus which does not coincide with intracranial pressure (ICT) elevation. Reported a case of a 57-year-old male with decreased consciousness and extrapyramidal symptoms and Normo pressure hydrocephalus (NPH), a V-P Shunt operation was performed. The operation was carried out under general anesthesia, using a non-kinking ETT no. 7.5, controlled ventilation. Premedication given midazolam 2 mg iv, Co induction with oxycodon 10 mg iv. Induction with propofol 150 mg iv, intubation facilities with rocketuronium 30 mg iv, maintenance with O2: Air (50: 50), sevoflurane, propofol continuous 100 mg/hour, rokuronium 20 mg/hour. Stable hemodynamics, SBP 130–150 mmHg, DBP 80–90 mmHg, HR 50–70 x/min, O2 saturation 99–100%, etCO2 35–37. After surgery the patient was treated in the intensive care unit (ICU) for monitoring blood pressure and extrapyramidal symptoms. The main goal of treatment should not be solely for the treatment of acute symptoms of EPS but also for the management of basic disease causing EPS related to morbidity and maintaining quality of life. Multidisciplinary management (neurosurgery, neurosurgery, intensive anesthesia and medical rehabilitation) are needed for better long-term results.

Successful endodontic treatment depends upon the clinician’s precise knowledge and ability to manage complex internal anatomy in order to avoid failure during endodontic treatment. Mandibular canines normally have single root with centrally placed single canal and internal anatomy as simple as could be expected, but there are such canines with a single root with two canals, two roots or fused roots. The occurrence of two canal joining to one for a men may be a rare entity with 4-15% prevalence. Canine is the cornerstone of mouth due to its position and very important as abutment for any type of restoration, so long term success of the restorative depend directly on the quality of endodontic treatment. This article presents a case series of successful endodontic management of mandibular canine with two canals which merge into one canal with one foramina and mandibular canine with two roots and two canals.
