Dental traumatic injuries most commonly occur in the primary and mixed dentition, but vertical complicated crown-root fractures are rarely seen in children. Clinical and radiographic examination of these injuries helps in accurate diagnosis and management. According to the International Association of Dental Traumatology guidelines, treatment usually involves extraction followed by placement of a space maintainer. Cases of complicated crown and root fracture in primary posterior teeth are often unnoticed by the clinician, at the time of injury. Two such patients are presented, who reported symptoms a few weeks after their accident. They were managed conservatively by initial stabilization with stainless steel crowns, followed by root canal therapy. This report highlights the need for referral to specialists and emphasizes the importance of conservative management of primary teeth to maintain functional demands.

Clinical Relevance Crown-root fractures extending well below the alveolar crest, coupled with root perforations of posterior teeth, can be successfully treated with the combination of mineral trioxide aggregate and an adhesive restorative approach in a one-step restoration procedure.

Objective: To assess primary molars early loss in mixed dentition prevalence. Method: A study was carried out in children aged 6 to 12 years. Deciduous molar loss was considered early when it occurred before Nolla stage 6 (full formed crown) of the permanent successor or one year before its physiological exfoliation. Periapical radiographs were obtained to verify the presence of the permanent successor and the level of root formation, as well as the need for placement of fixed space maintainer lingual arch or Nance button. For the study of frequency distribution of early dental loss in relation to gender and dentition, we used the Chi-square test of 5% probability level and the non-parametric Mann-Whitney test for adequate statistical analysis. Results: Of the 315 children examined, 159 (50.47%) were male and 156 (49.52%) were female. Only 25 (7.9%) presented multiple dental losses in the posterior region, 19 in the lower arch and 6 in the upper arch. In the inferior one there was predominance of first and second deciduous molars loss. There was no difference in the association between genders and the occurrence of multiple dental losses (χ² (1) = 0.03, p>0.05). Conclusion: The prevalence of deciduous teeth premature loss was of 7.9% and the most affected teeth were the first and second molars in the lower arch. Lingual arch space holders were installed in 6 children, who will remain with it until the permanent successors erupt into the oral cavity.

Complicated crown-root fractures account for a small percentage of traumatic dental injuries seen in children; however, management of these injuries can be very challenging to clinicians. Factors such as complexity of the injury, patient’s age and dentition stage, patient’s cooperation, and parental demands may have some bearing on the type of treatment undertaken and its outcomes. In some children, these injuries may have significant impact on their quality of life. The purpose of this article is to describe two cases of complicated crown-root fracture which were successfully managed through orthodontic extrusion using a sectional fixed orthodontic technique. The basis for the treatment technique and its favourable outcomes were highlighted with its advantages and drawbacks.

ABSTRACT Traumatic injury to oral cavity with accompanying tooth fracture can be a tragic experience for the patient and is a problem that requires experience, judgment and skill of the dentist. The dental health and appearance marred by an unsightly injury must be restored to normal as soon as possible. Though, root fractures comprise 0.5 to 7% of injuries affecting the permanent dentition, improper and delayed treatment can lead to loss of tooth. This report records a clinical case of intraalveolar horizontal mid root fracture in maxillary right central incisor with subluxation of coronal segment, which was managed endodontically using mineral trioxide aggregate (MTA), as an apical barrier at the fracture site. Maxillary right lateral and left central incisors affected by oblique crown-root fracture which were restored by custom-cast post and crown. After one year follow-up, the teeth were asymptomatic and showed signs of healing apical to the MTA barrier.

Crown-root fractures are one of the most challenging trauma cases to treat. Reattachment of tooth fragment to a fractured tooth being a simple procedure conserves the tooth structure, maintains the natural esthetics value and is thus considered as a favorable treatment option. The reattachment procedure using composite resin should be considered if the subgingival fracture can be exposed to provide isolation after a careful evaluation of the biologic width involvement. This case report presents a complicated crown-root fracture of permanent maxillay left central incisor, involving the biologic width in an 11-year-old boy. The traumatized tooth was treated endodontically and reinforced by using glass fiber-post. Access to the subgingival margins was gained by electro surgery. The fractured fragment was reattached using bonding system and composite resin.

Aim: To investigate the prevalence of the early unitary loss of deciduous and the need for space maintainer in the posterior region, of the band-loop type. Methods: The clinical examination of 568 children between 6 and 12 years of age in mixed dentition was performed in two municipal schools. After the diagnosis of premature loss of one or two deciduous molars (unilateral, one or both arches, respectively), without successors initiating the eruptive process in the oral cavity, the child was referred to the Clinical School of Dentistry of UESPI (CSD ) for installation of the band-handle space maintainer. Results: The prevalence of early molar loss was 4.04%, the etiological factor of which was dental caries for all sample. Only 13 attended the CEO for detailed examination. After radiographic examination, it was verified that 8 did not need space maintainers because they were approximately 2/3 of the permanent successors root formation. In 5 children was placed the maintainer of space, band-handle, who are being supervised every 3 months until the eruption of the permanent successors. Conclusion: The prevalence of early premolar molar loss was 4.04% with no statistical difference between genders. The use of space maintainers after the early loss of a deciduous tooth is a preventive measure of malocclusion, being the band-loop appliance an excellent choice after loss of the deciduous unilateral molar.

The management of patients with traumatic injuries to their dentition poses a serious challenge in everyday general dental practice. For the rehabilitation of the complicated subgingival crown fracture of anterior teeth, multidisciplinary approach is often indicated. A combination of endodontic, orthodontic, periodontal and prosthodontic approach may be required. Orthodontic or periodontal intervention becomes an integral part for the exposure of the sound tooth structure of fractured anterior teeth with fracture line extending subgingivally. The aim of this paper is to discuss the immediate endodontic management followed by orthodontic extrusion of traumatized upper anterior teeth with fracture at the subgingival level. In order to expose the sound tooth structure for prosthodonticintervention, orthodontic extrusion was performed after endodontic treatment. To avoid extraction of the involved teeth, the multidisciplinary approach was adopted and finally the teeth were restored prosthodontically. The final result was esthetically pleasant and periodontically sound.

Trauma of maxillary incisors is frequent and dental practitioners have to be competent in managing the consequences. Complicated crown root fractures of anterior teeth can have profound aesthetic and psychological implications on patients. In younger patients, if extractions are considered, it can lead to complex prosthodontic challenges in the future. Ideally, an attempt should be made in restoring the traumatized teeth, so an effective aesthetic and functional result can be reached. This case report highlights the treatment of complicated crown root fractures on maxillary incisors using endodontics and fragment re-attachment of the fractured crowns. Twelve-month follow-up of the treatment demonstrated aesthetic and functional success and delayed the need for prosthetic replacement. CPD/Clinical Relevance: This paper describes traumatic injuries of teeth and how management via fragment re-attachment of the fractured crowns has led to delay of definitive prosthetic treatment.

Fracture of tooth after trauma is distressing to a person because of the discomfort and pain due to pulpal injury. Crown root fractures of anterior teeth cause concomitant periodontal injury and there will be concern about appearance, and aesthetics. Management of pulpal and periodontal tissue relieves pain and restoration of tooth form regains patients confidence. Restoration of fractured tooth will be accepted readily if it is minimally invasive, less expensive, and aesthetically acceptable. Reattachment is an option for restoration of anterior teeth compared to other artificial replacements because of its appearance as natural. This method is favourable when the fractured fragment is intact and available. Utilization of pulp space for retention of fragment is achieved by the insertion of a dentine bonding post. This case report describes a case of tooth reattachment after trauma in which the pulp space is utilized to bond a fiber-reinforced post for retention after periodontal tissue management.

Complicated crown-root fractures of primary teeth often present with a greater challenge to the pediatric dentist. Extraction of the involved tooth is the routine treatment indicated. But, early loss of this primary tooth may lead to esthetic and psychological problems and also causes a detrimental effect on the development of occlusion and the alveolar bone. The present case report described the management of crown-root fracture in a primary canine by surgical extrusion and showed a satisfactory prognosis at one-year follow-up.
