Changes of carotid and vertebral arteries in patents with arterial hypertension and hepatobiliary pathology

The study included 1172patients (410 men and 762 women) at the mean age of 60.3±10.4years with grade I-II (stage I-II) arterial hypertension (AH) admitted to the clinic of Institute of Experimental Medicine. The patients were divided into 2 groups based on the results of clinical and laboratory diagnostics. Group 1 (n=525) included patients with AH and hepatobiliary system (HBS) diseases, group 2 (n=647) patients with AH without HBS diseases. The patients group 1 had a thicker intima-media complex of carotid arteries, higher peak systolic bloodflow rate in the internal and vertebral carotid arteries, more pronounced coiling of internal carotid arteries than patients of group 2. Patients with AH and HBS diseases exhibited correlation between blood flow rate in external carotid arteries and atherogenicity coefficient. Duplex scanning of neck vessels of in patients with AH without HBS diseases revealed peculiar changes of the intima-media thickness and hemodynamically significant changes of the blood flow in the internal carotid arteries that may be of prognostic value in this nosological syntropy and require the personified approach to diagnostics, treatment, and prevention of these conditions.

Цель: сравнительный анализ госпитальных и отдаленных результатов открытой симультанной стратегии коронарного шунтирования + каротидной эндартерэктомии (КШ+КЭЭ) и гибридной стратегии чрескожного коронарного вмешательства + каротидной эндартерэктомии (ЧКВ+КЭЭ). Материалы и методы: В данное когортное, сравнительное, ретроспективное, открытое исследование включено 180 пациентов с сочетанным поражением коронарных и внутренних сонных артерий, направлявшихся на симультанное вмешательство или гибридную операцию в период с января 2011г по декабрь 2015г. и имевших сопоставимое поражение коронарного русла. Все исследуемые пациенты были разделены на две группы: группа 1 – КШ+КЭЭ (n=116) и группа 2 − ЧКВ+КЭЭ (n=64). В группе 1 средний период наблюдения составил 50±17 мес, в группе 2 – 53±17 мес. Комбинированная конечная точка включала в себя совокупность таких кардиоваскулярных событий, как: смерть + ИМ + ОНМК/ТИА. При критическом уровне значимости p<0,05 различия считались статистически достоверными. Результаты: в госпитальном периоде значимых различий по частоте осложнений получено не было. В группе 1 выявлено 2 летальных исхода (1,7%), 1 инфаркт миокарда (0,9%), 5 ишемических инсультов (4,3%). В группе 2 среди осложнений получено 3 кровотечения, потребовавших ревизии раны (4,7%) относительно 9 кровотечений группы КШ+КЭЭ, закончившихся ремедиастинотомией (7,8%), р=0,63. В отдаленном периоде наблюдения значимых различий по частоте неблагоприятных исходов получено не было, однако отмечено возрастание частоты комбинированной конечной точки в группе ЧКВ+КЭЭ против КШ+КЭЭ (15/64 (23,4%) vs 17/116 (14,9%), р = 0,2) за счет рестеноза/тромбоза стента, спровоцировавшего инфаркт миокарда и повторную незапланированную реваскуляризацию миокарда. Заключение: исследование не показало значимых различий результатов открытой и гибридной реваскуляризации миокарда и головного мозга. Однако выявлена тенденция в нарастании числа неблагоприятных кардиоваскулярных событий после ЧКВ+КЭЭ в отдаленном периоде относительно КШ+КЭЭ в виду возрастания числа тромбозов/рестенозов стента. Данная особенность подчеркивает преимущества более агрессивных методов реваскуляризации над интервенционными. Objective: A comparative analysis of the hospital and long-term results of the open strategy - simultaneous coronary artery bypass grafting + carotid endarterectomy (CABG+CEE) and the hybrid strategy - percutaneous coronary intervention + carotid endarterectomy (PCI+CEE). Materials and methods: This cohort, comparative, retrospective, open-label study included 180 patients with a comparable lesion of the coronary channel and combined lesions of the coronary and internal carotid arteries going for simultaneous intervention or a hybrid operation from January 2011 to December 2015. All studied patients were divided into two groups: Group 1 - CABG+CEE (n=116); Group 2 - PCI+CEE (n=64). In group 1, the average follow-up period was 50±17 months; in group 2 - 53±17 months. Results: in the in-hospital period, no significant differences in the frequency of complications were obtained. 2 deaths (1.7%), 1 myocardial infarction (0.9%) and 5 ischemic strokes (4.3%) were detected in group 1. There were 3 bleedings cases requiring wound revision (4.7%) in the PCI + CEE group versus 9 in the CABG + CEE group requiring remediastinotomy (7.8%), p=0.63. In the long-term follow-up no significant differences in the frequency of adverse outcomes were obtained, however, an increase in the combined endpoint frequency in the PCI+CEE group as compared to CABG+CEE group was noted (15/64 (23,4%) vs 17/116 (14,9%), р=0,2, due to the stent restenosis/thrombosis which provoked myocardial infarction and repeated unplanned myocardial revascularization. Conclusion: the study showed no significant differences when performing open or hybrid myocardium and brain revascularization. However there is a tendency to increase the number of adverse cardiovascular events in the long-term period after PCI+CEE as compared to CABG+CEE mostly due to stent thromboses/restenoses. This feature emphasizes the advantages of more aggressive methods of revascularization over interventional.

A minority of traumatic carotido-cavernous fistulas (CCF) are of the indirect variety, and then usually supplied only by meningeal branches of the ipsilateral external carotid artery (Type C). We describe a case of a Type D CCF due to traumatic injury of the inferolateral trunk (ILT), and describe its angiographic features and endovascular management through both the external and internal carotid arteries following a failed transvenous approach.

AbstractThe arterial pattern of the human body is one of the systems that show a large number of variations. Many reports are available regarding variations of common carotid, external and internal carotid arteries and branches of external carotid artery. We describe a very rare case of lateral position of external carotid artery. The external carotid artery was lateral to the internal carotid artery at the bifurcation of the common carotid artery. The clinical importance of this variation is discussed.

The accuracy, and possible clinical application of non-invasive ultra sound imaging of the cervical carotid, artery in patients with cerebrovascular disease has been investigated by comparing a pulsed multi-channel directional Doppler imaging devise (“MAVIS” - GEC Medical) with conventional carotid angiography. Of 23 normal or minimally diseased internal carotid arteries only 4 appeared as stenosed on Doppler angiography, the other 19 were normal; of 14 stenosed arteries only 1 appeared normal on imaging,1 appeared occluded end 12 showed clear images of stenosis; all 6 occluded arteries appeared occluded on ultra sound imaging. It was difficult to obtain satisfactory images of the external carotid arteries in all cases. The additional facility to display a velocity flow profile at preselected points along the arterialimage should increase the usefulness of this technique, particularly when there is only minor atheromatous irregularity or ulceration, and such a device will be briefly disrussed.

Collateral networks between the external carotid artery and internal carotid arteries become crucial for cerebral perfusion after occlusion of internal carotid arteries. We report the first case of a patient who received percutaneous transluminal angioplasty and stenting in a collateral vessel between the external and internal carotid artery for treatment of radiation induced severe stenosis of the internal carotid artery in the context of a contralateral internal carotid artery occlusion.

Background and Objective. Whether anterior cerebral artery occlusion occurs after pipeline embolization devices (PEDs) are implanted to treat posterior communicating artery aneurysm is controversial. The purposes of this study were to explore the effect of a PED covering A1 on patients’ clinical prognosis and to evaluate the factors related to vascular occlusion. Method. The clinical and imaging data of PEDs in the postmarket multicenter registry study (PLUS) in China were retrospectively analyzed, and patients were divided into two groups on the basis of the follow-up angiographic results: group 1 (no significant change in A1 blood flow) and group 2 (A1 occlusion or decreased blood flow). We collected patients’ baseline data and evaluated the following imaging indicators: diameter and ratio of bilateral A1, M1, and internal carotid artery (ICA) vessels before stenting and the ratio of the PED size (sPED) to the ipsilateral ICA (I-ICA) diameter on the implantation side. Results. A total of 1171 patients were included, of whom 48 met the inclusion criteria (17 in group 1 and 31 in group 2). In group 2, three patients experienced neurological deterioration at follow-up. From the univariate analysis of outcomes, single PED without coils, incomplete aneurysm occlusion (IAO), maximum aneurysm diameter, aneurysms involving the ICA bifurcation (ICAb), and large sPED/I-ICA diameter ratio were included in the multivariate analysis ( P < 0.20 ). The multivariate regression analysis results showed that the ratio of sPED/I-ICA diameter was the factor influencing A1 vessel occlusion. The area under the ROC curve was 73.2%. When the sPED/I-ICA diameter ratio was 1.14, sensitivity was 70.6%, and specificity was 77.4%. Conclusion. When an oversized PED is placed from M1 to the ICA, the higher porosity formed at the covered A1 orifice is conducive to maintaining stable A1 blood flow and reducing the risk of A1 vessel occlusion. BACKGROUND: Computational fluid dynamics provides a new method for the study of the blood flow characteristics of the formation and development of intracranial aneurysms. OBJECTIVE: To compare blood flow characteristics between the healthy internal carotid artery and normal intracranial aneurysms. METHODS: The internal carotid arteries were simulated to obtain hemodynamic parameters in one patient. RESULTS: The internal carotid artery associated with aneurysm presents low wall shear stress, high oscillatory shear index, and high particle retention time compared with the normal internal carotid artery. CONCLUSIONS: There are differences in blood flow between the normal internal carotid artery and intracranial aneurysm. The vortex of the aneurysm will produce turbulence, indicating that it is unstable, which results in the growth and rupture of the aneurysm.
