Aim: to study parameters of arterial stiffness and subclinical atherosclerosis in patients with axial spondyloarthritis (axSpA) without clinically manifest cardiovascular pathology depending on the presence of anemia.Material and methods. 102 patients with axSpA (mean age – 37.7±9.8 years, axSpA duration – 13.5±8.7 years, 66 (64.7%) men) were included. The BASDAI index and C-reactive protein (CRP)-based ASDAS score were measured, the hemogram, the erythrocyte sedimentation rate (ESR), and the level of CRP were studied. Parameters of arterial stiffness were studied by oscillography and photoplethysmography, intima-media thickness (IMT) was evaluated by B-mode ultrasound according to standard protocols.Results. During analysis of pulse wave properties, no statistically significant differences in parameters of vascular wall stiffness were found in axSpA patients with and without anemia. Aortic pulse wave velocity (PWVao) in patients with anemia was 7.4±1.5 m/sec, aortic augmentation index (Aix-ao) was 19.1±13.7%, stiffness index (SI) was 8.2±1.7 m/sec; in patients without anemia – 7.4±1.4 m/sec, 17.3±10.6% and 8.8±2.0 m/sec, respectively (p >0.05 for all). IMT in patients with anemia was 0.70±0.13 cm, in patients without anemia – 0.73±0.16 cm (p >0.05). Correlation analysis was performed and significant correlations were noted between Aix-ao, PWVao, SI, IMT and age (r=0.488, r=0.516, r=0.289, r=0.461, respectively, p <0.05); Aix-ao, PWVao and the BASDAI index (r=0.243, r=0.253, respectively, p <0.05). Significant correlations between PWVao and Aix-ao (r=0.442, p <0.001), SI (r=0.273, p=0.011) and IMT (r=0.236, p=0.034) were found.Conclusion. The present study did not confirm the negative effect of anemia on vascular wall stiffness parameters and IMT in patients with axSpA. We consider that potential protective effect of anemia, due to well-known pathophysiological patterns — a decrease in blood viscosity and the induction of nitric oxide synthesis, plays an important role. Further studies are required to assess relationship between hemoglobin levels and markers of endothelial dysfunction in patients with axSpA.

Objective. To assess the relationship between arterial stiffness parameters in hypertensive patients with and without atherosclerotic lesions.Design and methods. We included 127 subjects who were divided into 3 groups: patients with hypertension (HTN) without atherosclerosis (n = 42); patients with HTN and subclinical atherosclerosis (SА) (n = 52) and control group which consisted of individuals without HTN, SA, or coronary artery disease (n = 33). All groups matched by age and gender. All subjects underwent following examinations: ultrasonography of extracranial segments of carotid arteries, 24-hour blood pressure monitoring with the assessment of arterial stiffness parameters.Results. In subjects with HTN compared to controls, pulse wave velocity in aorta (PWVao) was significantly higher (11,3 ± 1,5; 12,3 ± 1,8 vs 10,4 ± 1,3 m/s; p < 0,05), as well as pulse pressure (PP) (46,4 ± 9,8; 45,6 ± 10,6 vs 39,9 ± 6,5 mmHg; p < 0,05), central pulse pressure (PPао) (35,5 ± 8,5; 34,9 ± 8,5 vs 30,9 ± 5,4 mmHg; p < 0,05), and arterial stiffness index (ASI) (141 (127, 159); 139 (128,5, 160,5) vs 126 (118, 138) mmHg; p < 0,05). In subjects with HTN and SA, PWVao was significantly higher compared to other groups (p < 0,05). No significant difference in augmentation index was found (–32,5 (–45, –12); –22 (–36, –12); –37 (–50, –17); p = 0,25). Аmbulatory arterial stiffness index was higher in controls (0,5 ± 0,2) compared to HTN group (0,4 ± 0,2; p = 0,05), while HTN and SA group did not differ significantly (0,5 ± 0,2; p = 0,3). PWVao above 11,15 m/s is associated with 4,3 (2,3–8,2) times higher rate of atherosclerosis plaque detection.Conclusions. In HTN patients, arterial stiffness is changed compared to healthy individuals. PWVao above 11,15 m/s is associated with 4,3 (2,3–8,2) times higher rate of atherosclerosis plaque detection. In patients with HTN and SA arterial stiffness is higher, which might have additional predictive value in risk stratification.

Abstract Objective PsA patients who achieved sustained minimal disease activity (sMDA) had less subclinical atherosclerosis progression. The vascular effects of achieving other potential treatment targets, including the PsA Disease Activity Score (PASDAS) and the Disease Activity in PsA (DAPSA) score, remained uncertain. This study aimed to compare the vascular effects of achieving different treatment targets in PsA patients. Method This is a post hoc analysis of a 2 year treat-to-target study aimed at MDA. A total of 101 consecutive PsA patients without overt cardiovascular disease were recruited. High-resolution carotid ultrasound and arterial stiffness markers were assessed annually. Low disease activity (LDA) was defined as MDA, DAPSA ≤14 or PASDAS ≤3.2. Sustained disease control was defined as achieving these targets at each visit from month 12 until month 24. Results Ninety patients [52 male (57.8%), age 50 years (s.d. 11)] who completed 24 months of follow-up were included in this analysis. A total of 44%, 48% and 45% of patients achieved sustained DAPSA LDA (sDAPDA-LDA), sustained PASDAS LDA (sPASDAS-LDA) and sMDA, respectively. Patients who achieved sMDA had significantly less progression of carotid intima-media thickness than those who did not (P = 0.031). Using multivariate analysis, achieving sMDA and sPASDAS-LDA had a protective effect on plaque progression, less increase in total plaque area, reduced mean intima-media thickness and reduced augmentation index after adjusting for covariates. In contrast, no significant differences in the progression of vascular parameters were demonstrated between patients who did or did not achieve sDAPSA-LDA. Conclusion Achieving sMDA/sDASPAS-LDA, but not sDAPSA-LDA, was associated with a protective effect in subclinical atherosclerosis and arterial stiffness progression. A multidimensional domain of disease control might be better in minimizing cardiovascular risk in PsA.

AbstractLow cardiorespiratory fitness is associated with higher cardiovascular risk, whereas high levels of cardiorespiratory fitness protect the cardiovascular system. Carotid intima-media thickness and arterial distensibility are well-established parameters to identify subclinical cardiovascular disease. Therefore, this study investigated the influence of cardiorespiratory fitness and muscular strength on carotid intima-media thickness and arterial distensibility in 697 children and adolescents (376 girls), aged 7–17 years. Cardiorespiratory fitness and strength were measured with the test battery FITNESSGRAM; carotid intima-media thickness, arterial compliance, elastic modulus, stiffness index β, and pulse wave velocity β were assessed by B- and M-mode ultrasound at the common carotid artery. In bivariate correlation, cardiorespiratory fitness was significantly associated with all cardiovascular parameters and was an independent predictor in multivariate regression analysis. No significant associations were obtained for muscular strength. In a one-way variance analysis, very fit boys and girls (58 boys and 74 girls>80th percentile for cardiorespiratory fitness) had significantly decreased stiffness parameters (expressed in standard deviation scores) compared with low fit subjects (71 boys and 77 girls<20th percentile for cardiorespiratory fitness): elastic modulus −0.16±1.02 versus 0.19±1.17, p=0.009; stiffness index β −0.15±1.08 versus 0.16±1.1, p=0.03; and pulse wave velocity β −0.19±1.02 versus 0.19±1.14, p=0.005. Cardiorespiratory fitness was associated with healthier arteries in children and adolescents. Comparison of very fit with unfit subjects revealed better distensibility parameters in very fit boys and girls.

Objective. To investigate the relationship of arterial wall parameters (flow-mediated dilatation of the brachial artery, augmentation index, pulse wave velocity, stiffness index, carotid intima-media thickness) to conventional cardiovascular risk factors and cardiovascular risk assessed by SCORE system. Material and methods. A total of 209 subjects aged 40–65 years without clinically overt cardiovascular disease were examined. Parameters of arterial stiffness were obtained by two methods: augmentation index and carotid-radial pulse wave velocity by applanation tonometry and stiffness index by the means of finger photoplethysmography. Flow-mediated dilatation of the brachial artery, reflecting endothelial function, and carotid intima-media thickness was determined using a high-resolution B-mode ultrasonography. Results. Age and the presence of diabetes strongly influenced all parameters of the arterial wall (diabetes was not independent predictor when evaluating augmentation index). Mean arterial pressure and gender were independent predictors for arterial stiffness parameters – carotid-radial pulse wave velocity and augmentation index. Flow-mediated dilatation was strongly dependent on the diameter of the brachial artery, age, and body mass index. Using logistic regression, it was found that pulse wave velocity (P=0.014), intima-media thickness (P=0.004), and flow-mediated dilatation (P=0.020) were important parameters dividing subjects to the groups of increased (³5%) and low (<5%) cardiovascular risk assessed by SCORE system. The cutoff values for intima-media thickness and pulse wave velocity were 0.078 cm and 8.95 m/s, respectively. Conclusions. Arterial wall parameters are closely associated with conventional risk factors; they are influenced by age and the presence of diabetes. Arterial stiffness parameters are also influenced by mean arterial pressure; high-density lipoprotein cholesterol has influence on carotid intima-media thickness. Cutoff values for carotid intima-media thickness and carotid-radial pulse wave velocity could help to discriminate patients with increased cardiovascular risk.

Cardiovascular disease is one of the major causes of death throughout the world. Early detection of target organ damage is important for more successful prevention of cardiovascular diseases and improvement of patient outcomes. One of these target organs is the vascular wall, and its damage consists in loss of elastic properties and increase in stiffness. Many studies have shown that the increasing stiffness of the vascular wall is an independent predictor of cardiovascular risk. Objective: To evaluate parameters of the ambulatory arterial stiffness monitoring in patients with bronchial asthma. Materials and methods. The study enrolled 119 patients with asthma. The group of control included 30 practically healthy volunteers comparable by gender and age with the patients of the main group. Each patient underwent ambulatory blood pressure and arterial stiffness monitoring using BPLab MnSDP-2 device (Petr Telegin, Russia). Results: In patients with asthma higher values of augmentation index, arterial stiffness index and pulse wave velocity in the aorta in comparison with patients of the control group, as well as violation of the 24-hour profile of arterial stiffness were noted. Conclusion. Patients with asthma demonstrated significantly increased arterial stiffness in comparison with control group.
