ABSTRACTBackground: Edentulism post tooth extraction is an undesirable state among patients, specifically with the involvement of anterior dentition. Immediate denture in cases of anterior teeth extraction is a selected treatment to recover aesthetic and phonetic function. This treatment shows an impact on psychological stability and improves patients’ confidence level. Objective: This case report provides information on the management of immediate removable partial denture for maxillary and mandibular teeth. Case: A female teacher suffered from tooth loss of 26, 28, 36, 46, 47. Tooth 11, 14, 21, 22, 31, 32, 41, 42 were not supported with adequate periodontal tissue and presented with level 3 tooth mobility. Patient expected the teeth to be removed and managed with prosthodontic appliance, yet disfavoring the stage of edentulism because her profession demanded high social performance. Case management: The treatment for this case includes immediate removable partial denture with acrylic material in upper and lower jaw. Denture was fabricated prior to the extraction of tooth 11, 14, 21, 22, 31, 32, 41, 42, and inserted promptly. The insertion result showed good occlusion, retention, stabilization, and tissue adaptation of denture. It also demonstrated good aesthetic result. On 24 hours control post insertion, wound closure was gradually established with the presence of redness. A week after insertion, wounded tissue had been fully covered and redness had been diminished. Conclusion: Immediate removable partial denture provides good aesthetic and function. Patient was satisfied with the treatment because it improved her appearance without experiencing any period of edentulism.Keywords: Aesthetics, Edentulous, Extraction, Immediate Removable Partial Denture.

A 62-year-old female patient came to the Dental Hospital of Universitas Airlangga to maintain her remaining teeth and make dentures for the missing teeth, and expressed that she expected a high aesthetic result. The patient had lost bilateral posterior mandibular teeth, resulting in unstable occlusion. To stabilize the occlusion, this patient was treated by temporary denture. After 3 months of follow up and confirmed adaptation with the new occlusion, prosthodontic treatment was done consisting of bilateral attachment retained mandibular removable partial denture, aimed at the patient’s expectation of acquiring good aesthetic results and functional rehabilitation.

The paper presents the study of the stresses and the displacements that appear in different areas of the removable partial denture (RPD) with extracoronal attachments under occlusal loading, using FEA. The first step was to create the 3D model of a RPD with ball attachments in case of a class I Kennedy edentulous patient with all six anterior teeth as abutments. All materials were considered homogeneous, isotropic and having linear elasticity. There were assumed two types of constraints: in the first one the model is fixed under the action of the mastication forces; in the second one the rotational movement of the denture’s saddles in the sagittal plane was simulated, towards the edentulous ridge. In the first case, the 3D model and FEA were developed using Autodesk Inventor 2013 software. In the second case, the FEA was done using Autodesk Algor Simulation software. The loading of the model was symmetrically applied, considering the magnitude of the tangential and normal oral forces determined by Las Casas et al. These forces were applied to all the PM1, PM2, M1 and M2 pairs. Each loading case was analysed separately. The results of the two constrain situations are presented graphically, comparatively and show that the amount of the saddle displacement depends on the application area of the mastication force. Maximum displacement was computed at the application of the mastication forces on the M2 and decreases as the force is applied more mesial. The amount of the major connector’s deformation is very little influenced by the site of the force. The largest displacement of the denture was recorded at the distal end of the saddle, with values ​​that depend on the site of the forces. The existence of the rotational movement of the denture’s free end saddles in the sagittal plane, towards the edentulous ridge, changes the values ​​of accumulated stresses in denture during mastication and its Von Mises stress peaks. This analysis can be use to establish the principles of planning and designing the RPD with ball attachments having in mind to minimize the number of repairs caused by fractures of the components.

Oral rehabilitation with free-end removable partial denture (RPD) is one of the major challenges in prosthodontic dentistry. The absence of a distal abutment produces undesirable RPD movement during masticatory function due to the high resilience of the residual ridge, which limits the denture efficiency and may damage the abutment teeth and bone over time. The advent of dental implants made possible to substitute the missing teeth with fixed implant-supported dentures as the first choice treatment to overcome such inconveniences. However, this indication may not be suitable for all patients due to financial, anatomical or systemic health conditions. Nevertheless it is possible to improve free extension RPD by using implants in the posterior edentulous ridge to achieve biological, biomechanical, physiological and social benefits. This article aims to present a case report on oral rehabilitation in which a RPD was made combined with an implant for posterior support in a sizeable edentulous ridge. The patient appreciated the retention improvement and the aesthetic result. The combination of a RPD with a posterior implant is an alternative treatment for cases in which implant-supported fixed prosthesis is not indicated, reducing the displacement and minimizing the limitations of the free-end devices.

Removable partial denture is a treatment option where fixed prosthesis is not indicated. Due to its esthetic problems in the anterior region various modifications have been designed for its fabrication. This article describes an esthetic alternative using a round rest distal depression clasp for maxillary anterior teeth abutment while restoring the missing teeth with a cast partial denture.

The realization of esthetic objectives and functional demands in the reconstruction of severely compromised dentition represents considerable clinical problems. This clinical report represents the prosthetic management of a severe dentofacial deformity with compromised periodontal status. Treatment objectives were based on Aesthetics enhancement, sociopsychological support of the patient, restoration of form function, phonetics and comfort. Following which extraction of upper incisors and alveoloplasty done. An immediate removable partial denture was fabricated, considering aesthetics, phonetics and comfort of the patient followed by replacement of incisors by metal- ceramic fixed partial denture done. Clinical relevance: prosthetic management of severlymalaligned natural maxillary anterior teeth by fixed partial denture, considering aesthetics and phonetics of the patient. The selective laser melting technology is an additive digital method that permits producing metal frameworks for removable partial dentures. The existing literature has compared this technique with the classic technique in terms of precision, quality of fit, and function. This work describes a clinical case of a metal framework produced by the selective laser melting technique in a patient rehabilitated with a removable partial denture. No adjustments were required when the metal framework was inserted, showing good adaptation and stability to the abutment teeth and surrounding tissues. Further studies are necessary for a significant assessment of this technique’s clinical applicability.

The construction of a removable partial denture (RPD) aims the replacement of lost dental units, incorporating them into the stomatognathic system, without harming the biomechanical balance. Besides, there is a great challenge today, of combining function and esthetics. The difficulty in achieving esthetic harmony by the presence of metal clasps on labial surfaces of anterior teeth justify studies aimed at solving this problem in the RPDs. There is still the fact that this procedure is simpler and less costly for the patient compared to other treatment options. Therefore, this study presents esthetic solutions for RPD and suggests the comparison among conventional metal clasps on buccal retainers with clips in the palatal area, through three clinical cases of patients with partial absence of anterior dental units. Two prosthetic pieces for each patient were made: 1) prioritizing only the biomechanical principles and 2) considering both the biomechanical and cosmetic principles. It was observed that MDL and Quereilhac clasps are viable alternatives to get esthetics in RPDs of anterior edentulous patients with Class III and IV of Kennedy. Therefore, it can be concluded that there was an improvement in smile of individuals partially edentulous in the anterior region, by not viewing the metal structure on the buccal area of the teeth.

This case report describes the oral rehabilitation of a patient with cleidocranial dysplasia who received a removable partial denture along with silicone-based permanent soft liner to improve esthetic and masticatory function. This patient was the candidate of neither implant nor orthodontic treatment due to medical conditions, history of mandible fracture, age, and risk of fracture after mandibular teeth extractions. Cone-beam computed tomography has made it possible to obtain comprehensive information regarding the morphology and positional relationship of impacted supernumerary teeth. Also, proper collaboration between surgeon and prosthodontist helped to achieve significant improvements in patient’s self-esteem, masticatory function, and esthetic.

This is a case report of a 40-year-old female patient with chief complaint of spacing in front teeth. Patient exhibited convex profile, incompetent lips, spacing in anterior maxillary and mandibular teeth. She had maxillary posterior long span fixed partial denture and generalised periodontitis. She was diagnosed as Class I malocclusion with proclined and spaced anterior teeth. Treatment objective include space closure, maintaining class I molar and canine relation, achieving ideal overjet and overbite and functional occlusion. Thorough oral prophylaxis was done prior to start of fixed mechanotherapy. The case was bonded (upper canine to canine and full lower arch) with 022 MBT bracket system. Aligning and levelling was done sequentially with 012,016,17x25 Niti wires. One bracket each side was bonded on the maxillary FPD abutment teeth for anchorage. Space closure was achieved using low forces with elastomeric chain on 19x25 SS wire. Total treatment time was 12 months. Ideal overjet and overbite achieved with functional occlusion. Periodontal condition improved and stable results were achieved. Patient is given fixed retention and removable hawleys retainer with tongue crib in upper arch. In the recent years, there has been an increase in number of adults seeking orthodontic treatment and orthodontists quiite often face patients with periodontal disease. Adult patients should follow regular oral hygiene measure in order to maintain periodontal health during and after active orthodontic therapy.
