Abstract An accumulating evidence suggesting the neurotropic characteristics of the SARS-CoV-2. Although the pathogenesis is unclear, the relationship between COVID-19 and Guillain Barre Syndrome(GBS) has been previously reported. We present a 66-year-old male with para- infectious COVID-19-related GBS admitted with a 2-day bilateral weakness in distal lower limbs. His neurological findings occurred on the third day of the diagnosis of COVID-19. A cerebrospinal fluid examination revealed albumin-cytological dissociation.To the best of our knowledge, this is the first para-infectious GBS case related to the SARS-CoV-2 reported from Turkey. Clinicians should be aware of this kind of complication to manage patients.

Abstract Background Guillain-Barré syndrome (GBS) is the most common and serious acute paralytic neuropathy and is usually caused by infection. It is thought to be the result of an aberrant response of the immune system. To our knowledge, GBS, especially severe GBS, after orthopaedic surgery has rarely been reported. Case presentation We herein report the case of a 58-year-old man who developed quadriplegia and respiratory failure on the 6th day after surgery for multiple fractures. The patient had no symptoms of respiratory or gastrointestinal tract infection within 4 weeks before the onset. The white blood cell count was normal, and there was no redness, swelling, heat or pain in the surgical incision. Brain, cervical and thoracic magnetic resonance imaging were normal, albuminocytological dissociation was found on cerebrospinal fluid examination, and electrophysiological examination showed that sensory and motor nerve evoked potentials could not be elicited. A diagnosis of post-traumatic GBS was made, and the patient was treated with intravenous immunoglobulin and plasma exchange, as well as supportive care and rehabilitation exercise. The length of stay was 18 months, and the in-hospital-related costs amounted to $127,171. At the last follow-up, the patient had recovered only grade 3 power in the upper limbs and grade 2 power in the lower limbs. Conclusions Severe GBS is a rare complication after orthopaedic surgery. When progressive weakness occurs in trauma patients, the possibility of GBS should be considered, and cerebrospinal fluid and electrophysiological examinations should be performed in a timely manner. For patients with severe GBS after trauma, the treatment costs may be high, and the prognosis may be poor.

Introduction: Guillain-Barré syndrome is an acute/subacute set of clinical features of immune mediated polyradiculoneuropathy, typically after respiratory or gastrointestinal viral infection. In this scenario, we described a case of the syndrome mentioned after infection by sars-cov-2 virus. Objectives and Methods: Description of a clinical case after analysis of medical history and complementary exams, in addition to literature review. Design and Setting: Case report, type of descriptive study, developed in the Institute of Medical Assistance to the State Public Servant of São Paulo. Results: A 68-year-old female patient with clinical and laboratory diagnosis of coronavirus infection, hospitalized in need of oxygen supplementation, which evolved after twelve days of symptoms with hypoesthesia on legs and feet and progressively ascending and symmetrical flaccid paraparesis that led to tetraparesis. Neurological examination showed tetraparesis (muscle strength: grade III in MMSS and grade II in MMII), hyporeflexia in MMSS and reflexes abolished in the lower limbs, plantar skin reflex in flexion in both feet, preserved facial mimicry. About complementary tests, it presented cerebrospinal fluid with albuminocytologic dissociation (cell: 1, protein: 89, glucose: 86), RT-PCR for sars-cov-2 research in cerebrospinal fluid, inconclusive, in addition to four-limb electroneuromyography performed after 19 days of onset of neurological condition, indicated polyradiculoneuropathy with involvement of sensory and motor fibers, primarily demyering. Treatment with human immunoglobulin 400 mg/kg/day for 05 days was started. The reported patient was dismissed from the hospital with significant improvement, presenting muscle strength: grade V in MMSS and grade IV in MMII and already with the ability to walk. Conclusions: the case describes a classic neurological complication associated with a virus that was once non-circulating, but currently with a big clinical relevance.

Introduction: Coronavirus disease 2019 (COVID-19), started in December 2019, affects many organs and systems of the body, such as the central nervous system. Case Presentation: We describe a case of a 66-year-old female patient with positive results for COVID-19 using real-time PCR (RT-PCR) testing from her cerebrospinal fluid (CSF) and the coronavirus antibodies (Ig G and Ig M) using serology test. Her clinical presentation showed the misbalance and acute weakness of upper and lower limbs with preference of proximal parts of the limbs, pinprick, and the absence of deep tendon reflexes. Taken together, these symptoms, neurological tests, and the result of CSF analysis (high protein) were in favor of Guillain-Barre syndrome (GBS). In addition, because the neurological symptoms worsted four days after hospitalization, plasmapheresis was started over the course of 8 days (on every alternate day) with a volume of 2000 mL. Although the results of electromyogram (EMG) - nerve conduction velocity (NCV) following plasmapheresis showed the motor and sensory neuropathy, which was generally accompanied by demyelination, but it leads to the recovery of upper and lower limbs from 2/5 to 4/5 and improvement of neck flexor muscles from 3/5 to 5/5. Conclusions: GBS should be considered as a neurological outcome of COVID-19. In addition, regarding the beneficial effects of plasmapheresis on GBS (following coronavirus infection), it can be suggested as a part of the treatment protocol in these patients.

Objective: To examine clinical features, biochemical markers, demographic features, antecedent infections, frequency and treatment strategies. Methods: This case-control study was conducted at Pakistan Institute of Medical Sciences (PIMS), Islamabad, Pakistan and District Headquarters Hospital (DHQ), Rawalpindi, Pakistan from 2018 to 2020. Ninety consecutive patients of Guillain-Barré syndrome (GBS) and 77 healthy controls were enrolled. Patients were diagnosed by clinical presentation, Nerve conduction study (NCS), Electromyography (EMG), Cerebrospinal fluid analysis (CSF) and biochemical profile. Data was analyzed on IBM SPSS version 23. Results: Symmetrical ascending weakness was the striking clinical feature. Mean age was 40.20±14.90 years and male to female ratio was 2.1:1. Acute inflammatory demyelinating polyneuropathy (AIDP) was found to be the most common electrophysiological variant of GBS (46%). There was considerable difference in Interleukin-17(IL-17) levels between GBS patients 23.12 ± 3.41 pg/ml and healthy controls 8.82 ± 2.49. Mean IL-17 level was markedly increased in GBS patients, P=0.006, P<0.05. Gastrointestinal infection was the most common preceding infection (56.66%). Mean CSF protein was 100.83 g/dl with ± 51.32 standard deviation and albumio cytologic dissociation (ACD) was different in all four variants of GBS, P= 0.005. Conclusion: GBS was presented by all ages. Males were more affected than females. About two third of GBS patients showed an antecedent infection before GBS onset. Increased levels of cytokine (IL-17) showed involvement of autoimmunity. ACD differentiated it from poliomyelitis. Plasmapheresis and intravenous immunoglobulin (IVIG) therapy were used to treat patients. Introduction: Guillain-Barré syndrome is an acute / subacute inflammatory polyradiculoneuropathy that classically results in flaccid areflex palsy. However, there are other possibilities of clinical presentation that must be remembered so that an adequate diagnosis and treatment is carried out. Case report: Female patient, 23 years old, without comorbidities, with complaint of paresthesia in extremities and right peripheral facial paralysis, having diagnosis until then of Bell’s Palsy. She denied previous or current infectious complaints. The neurological examination revealed facial diparesis, proximal weakness of the lower limbs that made walking difficult, tactile and painful hypoesthesia in the feet, with reflexes 1+/4+ in the lower limbs and 3+/4+ in the upper limbs. An investigation was started with CSF collection that showed albuminocytological dissociation (proteins 440 mg/dl and leukocytes 01 mm3). Neuroimaging exams showed contrast impregnation in facial and trigeminal nerves. A diagnosis of acute inflammatory polyradiculoneuropathy was made and treatment with human immunoglobulin was initiated for 5 days. Electroneuromyography showed peripheral, sensory-motor polyradiculoneuropathy and questioned the physiopathological possibility of juxtaparanodopathy. The patient presented a significant and early improvement after treatment. Conclusions: It is essential to consider that Guillain-Barré syndrome has symptom variability, especially according to its pathophysiology and clinical and electrophysiological variant, thus avoiding that conditions such as this one are underdiagnosed.

Guillain Barre Syndrome (GBS) is an autoimmune disorder which affects the peripheral nervous system. It is a rare disorder affects in 1 per million people in year. It is characterized by symmetrical, progressive limb weakness and tingling. Case Report: A 53 year old male patient was presented with insidious onset of difficulty in moving right upper and lower limbs as well as gradual weakness of left limbs, and breathing difficulty, known case of diabetics’ mellitus and hypertension. Nerve conduction study shows suggest axonopathy; Acute Inflammatory Demyelinating Polyneuropathy (AIDP) is identified, which is a subtype of Guillain Barre Syndrome. Patient gradually develops areflexia, bifacial weakness, and quadriparesis. Patient was treated with IV immunoglobulin and intranasal oxygen therapy. Patient shows slight improvement in his medical condition, shows improvement in the power of lower limbs after one week of therapy. Physiotherapy was suggested.
