Subacute Thyroiditis Post-Covid-19 Infection: A Case Report

Background: Subacute thyroiditis (SAT) or granulomatous thyroiditis is usually a self-limiting inflammatory condition. The clinical presentation varies from person to person, but is usually associated with pain and systemic symptoms. Case Report: Herein, a case of a previously healthy 40 years old male who presented with unilateral right neck pain of gradual onset and progressive course has been described. The patient had recovered from a symptomatic Covid-19 infection 5 months back. The clinical examination, imaging and histopathological examination were consistent with the clinical diagnosis of subacute thyroiditis. The patient received medication in the form of pain killer, Celebrex and paracetamol, without corticosteroids, and completely recovered. Conclusion: The possibility of subacute thyroiditis post-COVID-19 infection should be considered.

Seborrheic keratosis is a benign tumour of skin, a common hyperkeratotic lesion of the epidermis,that usually occurs in the trunk and less frequently in the extremities, face and the scalp. A 65-year old farmer presented with a long standing, slowly growing, firm, redbrown, polypoidal mass about 2×2.5 cm in size, located at the skin mucosa interfare of the tip of nose. The lesion was excised under general anesthesia and histopathologic examination showed seborrheic keratosis. Diagnosis is made on the basis of clinical & histopathological examination. Here, we discuss the clinical presentation, differential diagnosis, pathological diagnosis and management of such a case. Background: Acute appendicitis is the most common surgical emergency. Obstruction of the lumen by fecolith is the usual cause of acute appendicitis.The aim of the study was to analyze clinical presentation of acute appendicitis and its histopathological correlation. Materials and Methods: A retrospective study of acute appendicitis was done in the Department of Surgery of Helping Hands Community Hospital from January 2009 to December 2010. Three hundred forty five patients out of 415 patients with clinical diagnosis of appendicitis underwent operative treatment. The histopathological reports were reviewed and correlated with clinical diagnosis. Results: Out of 345 patients who underwent operative procedure 98% (n= 338) came with chief complaint of pain in the periumbilical region migrating to the right iliac fossa. The mean age of presentation was 42 years. Increased leucocyte count was seen in only 65% cases. Acute appendicitis was more commonly seen in male patients (214 cases, 62%). The most common per operative finding was acutely inflammed appendix (84%) followed by perforated appendix (7.5%), gangrenous appendix (3.5%) and appendicular lump (1.5%). However, histopathological diagnoses were acute appendicitis (91.9%), resolving appendicitis (3.5%), lymphoid hyperplasia (2.6%), mucocele (0.3%) and carcinoid (0.3%). Normal histology was seen in 1.4% cases. Conclusion: Though there are other causes of acute abdomen, acute appendicitis still stands first amongst all the emergencies. Histopathological examination of appendectomy specimen should not be omitted in order to see the incidence negative appendectomy rate and to avoid complications relating to malignant conditions. ABSTRACTOne week after swimming in a man-made lake, a 9-year-old boy presented to the emergency department with headache, vomiting and lethargy. He had neck pain upon flexion and was unable to stand or walk. Cerebrospinal fluid examination revealed trophozoite and diflagellate forms consistent withNaegleria fowleri, an ameba species known to cause primary amebic meningoencephalitis. Despite aggressive management with amphotericin B and rifampin, he died 2 days later. This case report describes the clinical presentation, diagnostic findings and management of this uncommon but lethal entity.

Abstract Background Subacute thyroiditis (SAT) is rarely diagnosed in pregnant women, and only 7 cases have been reported to date. Thyroid dysfunction, especially hyperthyroidism, during pregnancy has been associated with both maternal and neonatal complications. Thus, the early diagnosis and treatment of SAT during pregnancy may be beneficial. We present a case report and literature review to complement the diagnostic evaluation and management of SAT during pregnancy. Case presentation A 27-year-old woman presented in gestational week 17 of her first pregnancy and had a negative prior medical history. She presented to the Endocrinology Department complaining of neck pain for one month that had intensified in the last five days. Physical examination revealed a diffusely enlarged thyroid gland that was firm and tender on palpation. The patient also had an elevated temperature and heart rate. The increasing and long-lasting pain coupled with a decreased level of thyroid-stimulating hormone indicated hyperthyroidism. Ultrasound findings were indicative of SAT. Importantly, the pain was so severe that 10 mg of oral prednisone per day was administered in gestational week 18, which was increased to 15 mg/d after 10 days that was discontinued in week 28. Levothyroxine was started in gestational week 24 and administered throughout the pregnancy. The patient responded well to the treatments, and her neck pain disappeared in gestational week 21. She gave birth to a healthy male in gestational week 41. Conclusion SAT can be diagnosed and effectively managed during pregnancy, thus benefiting mothers and infants.

Abstract BACKGROUND AND IMPORTANCE Hypertrophic interstitial neuropathy (HIN) is an uncommon, non-neoplastic lesion typically affecting peripheral nerves. Cranial nerve (CN) involvement is exceedingly rare. We present a case of isolated trigeminal nerve HIN manifesting with V3 distribution neuralgia. CLINICAL PRESENTATION A 50-yr-old male presented with left sided trigeminal neuralgia refractory to medical management. The patient underwent retromastoid craniectomy for possible microvascular decompression. Intra-operatively, the trigeminal nerve appeared to be focally enlarged with a sausage-like configuration. We selectively resected 1 fascicle which was predominantly involved. Histopathological examination revealed onion bulb formations composed of Schwann cells around centrally placed axons. A diagnosis of HIN was made. Postoperatively, the patient experienced complete resolution of symptoms. CONCLUSION This is the third case of isolated trigeminal nerve HIN in the literature. We performed a selective resection in a patient presenting with trigeminal neuralgia, resulting in complete resolution of symptoms. It is reported here with intraoperative microscope images, along with a review and analysis of this topic as it related to CN.

AbstractPituitary tuberculomas are extremely rare with only few cases reported in the literature. Intracranial tuberculoma commonly presents with gradual onset of headache and visual disturbances with or without systemic symptoms. We reported such a case who presented with headache without any visual symptoms, occasional vomiting, and significant weight loss. Contrast magnetic resonance imaging scan was suggestive of pituitary macroadenoma. Transnasal transsphenoidal excision of the lesion (microscopic + endoscopic) was done. Histopathological examination revealed possibility of tuberculosis. Patient was put on standard antituberculosis treatment and discharged in a stable condition.

Primary melanoma of the testis is very rare.  Melanoma of testis usually occurs as metastatic tumor. Here we present a case of 54 year old man with right testicular swelling proven to be primary malignant melanoma. Patient had right scrotal swelling of six months duration. With raised LDH and unremarkable Serum β-HCG and Alpha Feto Protein level and with a suscipicion of primary testicular tumor on MRI, right inguinal orchidectomy was done.  Its clinical presentation and histopathological examination is discussed here.

ABSTRACT Very few cases of oncocytoma parotid have been reported in literature. Because of its rarity (less than 1% of salivary gland tumors) and clinical presentation of the tumor, there is a tendency among the clinicians to misdiagnose it as pleomorphic adenoma, hemangioma or other forms of oncocytosis. Only a histopathological examination can confirm it. We report here a case of oncocytoma of parotid gland in a 70-year-old male who had the tumor for over 9 years. Primary carcinoma of fallopian tube is rare and accounts for only 0.7-1.5% of all gynecological malignancies. Majority of the patients are postmenopausal with mean age of 61 years. Study present a case of 48-year-old lady, who underwent total abdominal hysterectomy with bilateral salpingo oophorectomy for a clinical diagnosis of fibroid uterus. Histopathological examination revealed, in addition to intramural fibroids, bilateral serous papillary adenocarcinoma of fallopian tube. We present this case due to its rarity.

For a painful period, I experienced a disease since breast mass to Cancinoma till breast adenophaphy diagnosis and recovery; as a doctor of mine in medicine of Neonatology crossing with gynecology and obstetrics I have reviewed the information during disease development that related with the lab analysis, clinical diagnosis and therapy advancement [1-2]. Breast Mass is a one clinical presentation for many breast diseases as varied infection, injury, breast fibrosis, adenosis of disease or cancer; some of the diseases have involved in extensive age and area for female healthy. As usual knowing, female healthy is related with generation, some discussion is valuable to write here for clinical consideration
