Treatment of Lung Cancer Complicated with Infection of Talaromyces Marneffei Based on Chest Imaging Characteristics Monitoring (Preprint)

BACKGROUND Background: At present, although the infection of Talaromyces Marneffei has been known at home and abroad, there are few reports of Talaromyces Marneffei in lung cancer. OBJECTIVE Objective: The objective is to explore the diagnosis and treatment process of lung cancer patients with infection of Talaromyces Marneffei and its chest imaging characteristics, so as to improve the clinicians' realization of the disease. METHODS Method: The patients with lung cancer and infection of Talaromyces Marneffei (observation group) and the patients with infection of Talaromyces Marneffei (control group) are taken as the study objects, and the clinical characteristics and chest CT (computed tomography) imaging characteristics of the two groups are compared and summarized. RESULTS Results: The number of male patients infected with Talaromyces Marneffei is significantly higher than that of female patients (P < 0.05). The symptoms of cough and expectoration in the observation group are more than those in the control group (P < 0.05). The main imaging features of the observation group are obvious enhancement of focus enhancement scanning, strip shape and nodule, and the situation of obvious enhancement of focus enhancement scanning in the observation group is significantly higher than that in the control group (P < 0.05). CONCLUSIONS Conclusion: The clinical and imaging features of lung cancer and Talaromyces Marneffei infection overlap. When the lung lesions of patients with Talaromyces Marneffei have significant malignant signs, the possibility of lung cancer should be considered.

Abstract Objective The aim of this study was to explore the safe and effective method of expectoration in the preoperative period of patients with lung cancer resection and to promote the rehabilitation of patients. Methods A total of 100 cases of lung cancer patients undergoing elective surgery were divided into the observation group and the control group, with 50 cases in each group. The control group was treated with vibration expectoration vest for expectoration during the perioperative period, and the observation group was treated with respiratory function exerciser that has expectoration function in the perioperative period, three times a day, and the effect was evaluated after 5 days. Results The number of patients in the observation group after the first expectoration time was significantly less than that of the control group (P<0.001). Pain score, pulmonary atelectasis, and pulmonary infection rate of the observation group were significantly lower than those of the control group; the hospitalization time was significantly shorter than that of the control group; and the difference was statistically significant (P<0.05). Conclusions Lobectomy for lung cancer patients with perioperative respiratory training for respiratory function exercise, compared with conventional methods, is helpful for postoperative expectoration and to reduce the incidence of adverse events.

Objective: As for the lung cancer patients, their imaging characteristics of CT and DSA are evaluated, so as to analyze the relationship between the degree of angiogenesis and the expression of cytokines. Method: Firstly, the CT and DSA data of 40 lung cancer patients who are confirmed by pathology and received intravascular interventional therapy are collected. 5 mL venous blood of patients is extracted and serum samples are extracted. The levels of matrix metalloproteinase-2 (MMP-2) as well as basic fibroblast growth factor (bFGF) in 40 patients with lung cancer are detected by enzyme-linked immunosorbent assay (ELISA). Meanwhile, the contents of MMP-2 and bFGF in serum of 40 healthy people are collected as normal control group. Normal distribution, t-test and F-test are used to verify the correlation between the image features of lung cancer and the expression levels of MMP-2 and bFGF in serum. Results: Firstly, the levels of MMP-2 and bFGF in lung cancer group are obviously higher than those in the control group, and the difference was statistically significant. P values were 0.002 and 0, respectively (P < 0.01). In addition, a positive correlation exists between MMP-2 and bFGF in the group of lung cancer as well as control, and the correlation coefficients are 0.602 as well as 0.712, respectively. Secondly, DSA of lung cancer shows that MMP-2 and bFGF of different types of blood supply lung cancer are different. The P value of intra group and inter group comparison was 0, and the difference is statistically significant. Thirdly, on the image, the lung cancer has necrotic cavity. The P value of MMP-2 and bFGF are 0.046 and 0.005 respectively. The P value of MMP-2 and bFGF are 0.001 and 0.001 respectively with the burr sign on the image. The P value of serum MMP-2 and bFGF are 0 and 0.001 with the deep lobulated sign on the image, both of which are statistically significant. There is no correlation between the size and location of lung cancer focus, whether there is peripheral obstructive pneumonia and the level of MMP-2 and bFGF. Conclusion: The DSA blood supply of lung cancer is related to the expression of MMP-2 as well as bFGF in serum, which can reflect the angiogenic state of tumor and be used as the basis of anti-angiogenic therapy. Whether the CT image of lung cancer shows necrotic cavity, burr sign and deep lobulated sign has a correlation with the expression of MMP-2 as well as bFGF in the serum. The combination of the two is helpful for the accurate assessment of the disease.

<p class="Abstract">It was aimed to explore the expression level of miRNA-486 and miRNA-499 in the plasma of lung cancer patients and analysis their differences in expre-ssion. The expression level of both miRNA-486 and miRNA-499 in the plasma of non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC) were lower than that of the control group (p&lt;0.05) and the decrease was more obvious in NSCLC. Compare with the miRNA-499,expression quantity in NSCLC patients plasma. There was statistical significance difference (p&lt;0.05) between III~Ⅳstage and I~II stage. The expression quantity of miRNA in plasma of patients with extensive-stage SCLC was lower than that of patients with limited-stage SCLC (p&lt;0.05). The sensitivity and specificity of plasms miRNA-486 respectively were 88.5% and 83.3%. The expression of miRNA-499 and miRNA-486 in lung cancer patients were up-regulated, and might be closely related to the occurrence and prognosis of lung cancer, and might be used as potential screening and prognosis index for lung cancer.</p><p> </p>

In developed countries, lung cancer is the leading cause of cancer-related death in both sexes. Although cigarette smoking represents the principal risk factor for lung cancer in females, the higher proportion of this neoplasm among non-smoking women as compared with non-smoking men implies distinctive biological aspects between the two sexes. Gender differences depend not only on genetic, environmental, and hormonal factors but also on the immune system, and all these aspects are closely interconnected. In the last few years, it has been confirmed that the immune system plays a fundamental role in cancer evolution and response to oncological treatments, specifically immunotherapy, with documented distinctions between men and women. Consequently, in order to correctly assess cancer responses and disease control, considering only age and reproductive status, the results of studies conducted in female patients would probably not categorically apply to male patients and vice versa. The aim of this article is to review recent data about gender disparities in both healthy subjects’ immune system and lung cancer patients; furthermore, studies concerning gender differences in response to lung cancer immunotherapy are examined.

Rearranged during transfection proto-oncogene (RET) fusions represent a potentially targetable oncogenic driver in non-small cell lung cancer (NSCLC). Imaging features and metastatic patterns of advanced RET fusion-positive (RET+) NSCLC are not well established. Our goal was to compare the imaging features and patterns of metastases in RET+, ALK+ and ROS1+ NSCLC. Patients with RET+, ALK+, or ROS1+ NSCLC seen at our institution between January 2014 and December 2018 with available pre-treatment imaging were identified. The clinicopathologic features, imaging characteristics, and the distribution of metastases were reviewed and compared. We identified 215 patients with NSCLC harboring RET, ALK, or ROS1 gene fusion (RET = 32; ALK = 116; ROS1 = 67). Patients with RET+ NSCLC were older at presentation compared to ALK+ and ROS1+ patients (median age: RET = 64 years; ALK = 51 years, p < 0.001; ROS = 54 years, p = 0.042) and had a higher frequency of neuroendocrine histology (RET = 12%; ALK = 2%, p = 0.025; ROS1 = 0%, p = 0.010). Primary tumors in RET+ patients were more likely to be peripheral (RET = 69%; ALK = 47%, p = 0.029; ROS1 = 36%, p = 0.003), whereas lobar location, size, and density were comparable across the three groups. RET+ NSCLC was associated with a higher frequency of brain metastases at diagnosis compared to ROS1+ NSCLC (RET = 32%, ROS1 = 10%; p = 0.039. Metastatic patterns were otherwise similar across the three molecular subgroups, with high incidences of lymphangitic carcinomatosis, pleural metastases, and sclerotic bone metastases. RET+ NSCLC shares several distinct radiologic features and metastatic spread with ALK+ and ROS1+ NSCLC. These features may suggest the presence of RET fusions and help identify patients who may benefit from further molecular genotyping.

In this prospective, interventional case-control study, 58 patients with unilateral acute central serous chorioretinopathy (CSCR) were recruited. Patients ≥ 18 years age, presenting with first episodes of acute CSCR, were included. Acute CSCR was defined by the presence of subretinal fluid (SRF) and symptoms for <12 weeks duration with no clinical or imaging features of chronicity. Patients were alternately divided into treatment (Table Eplerenone 50 mg/day for minimum 1 month) and observation groups. Vision, SRF height and subfoveal choroidal thickness (SFCT) were checked at 1-, 2- and 3-months in both eyes of each group. Each group had 29 eyes. Mean age was 40.4 ± 7.1 and 43.3 ± 8.34 years in treatment and observation group, respectively. Mean symptom duration was 6.46 ± 1.45 and 5.87 ± 2.09 weeks, respectively. Vision improvement to 6/6 was seen in 92%, 100% and 100% cases in treatment group and 74%, 86% and 100% in control group at each visit, respectively. Complete SRF resolution in the treatment group was noted in 45%, 55% and 62% cases at each respective monthly visit. In the observation group, complete SRF resolution was noted in 10%, 21% and 31% at 1-, 2- and 3-month visits, respectively. SRF (p < 0.001) and SFCT (p < 0.001) reduction was noted in the affected eye of both groups. SFCT was reduced in the fellow eye after treatment (p = 0.005) compared to the observation group (p = 0.276). In conclusion, oral eplerenone achieves faster SRF resolution and vision improvement in acute CSCR. Additionally, it shows beneficial effects on the fellow eye.
