ABSTRACT Introduction: The anterior cruciate ligament (ACL) is one of the important structures of knee joint stability. Objective: To investigate the application of medical training therapy (MTT) in the functional recovery of patients after anterior cruciate ligament reconstruction. Methods: Eighty patients who were treated in a secondary hospital were selected as the study subjects and were divided into a control group and an observation group according to a random number meter, with 40 cases per group. We compared Lysholm knee function scores, the range of motion (ROM), daily life capability (ADL scale), pain degree (VAS rating), knee stretch muscle score, and standing balance ability score of the two groups 3 months after intervention. Results: After intervention, the first time getting out of bed, the doctor-patient satisfaction score, and the average length of stay were better in the observation group than in the control group (P<0.05). The excellent rate of knee function in the observation group was 92.50%, which was significantly higher than 70.00% in the control group (P<0.05). Conclusions: MTT therapy can significantly improve knee the function and pain level of patients after anterior cruciate ligament reconstruction, shorten the length of hospital stay, promote rehabilitation, improve the therapeutic effect, and improve the quality of life of patients. Level of evidence II; Therapeutic studies - investigation of treatment results.

To construct the tissue engineering urethral material that is closest to the normal urethral structure in the true sense in vitro. Abdominal ADSC from a 2-month-old New Zealand white rabbit was extracted and directly compounded with non-woven polyglycolic acid (PGA) (control group) to induce the differentiation of myoblasts and epithelial-like cells in vitro and shaped into urethral structure lumen Observation group); After Gd chelating protein nano-labeling and VEGF-loaded sustained release, the rabbit model of a long urethral defect was replanted and cultured for 4 weeks, 8 weeks and 12 weeks, respectively. There was no difference in urinary tract patency rate, urinary tract infection, and renal dysfunction rate between the two groups (P > 0.05). The urine flow rate in the observation group was significantly higher than that in the control group, and the residual volume decreased (P < 0.05). The blood vessel density and CD31 percentage in the observation group increased (P < 0.05). Compared with the conventional ADSC directly in contact with the composite material to construct the urethra, in vitro induction of ADSC to myoblasts and epithelial-like cells respectively, and then use the cell membrane technology to build a tissue engineering urethral material that is closest to the normal urethral structure in the true sense, and loaded with VEGF Loop release technology can significantly improve urodynamic functions, optimize tissue engineering urethral structure and vascularization, and is expected to become a new technology for constructing new tissue engineering urethral materials.

Objectives: To compare the efficacy and safety of intravenous thrombolysis with alteplase and intravenous thrombolysis with urokinase for patients with acute cerebral infarction. Methods: This prospective study included 140 patients with acute cerebral infarction who were admitted to our hospital between June 2018 and June 2019. They were randomly divided into two groups. The control group (70 cases) was treated with urokinase intravenous thrombolysis, and the observation group (70 cases) was given alteplase intravenous thrombolytic therapy. The treatment efficacy and safety of the two groups were compared. Results: The total effective rate of the observation group was 95.7%, and that of the control group was 78.6%, i.e., the total effective rate of the observation group was significantly superior to the that of the control group (P < 0.05). After treatment, the observation group had significantly lower National Institutes of Health Stroke Scale (NIHSS) score and significantly higher mini-mental state examination (MMSE) score than the control group; the difference was statistically significant (P<0.05). After treatment, the levels of inflammatory factors of both groups significantly decreased compared to before treatment, and the decrease in the observation group was larger than that in the control group (P<0.05). The levels of serum homocysteine (Hcy) and monocyte chemoattractant protein-1 (MCP-1) in the observation group were significantly lower than those in the control group after treatment, and the differences were statistically significant (P<0.05). The incidence of hemorrhagic adverse reaction in the observation group was lower than that in the control group (P<0.05). Conclusion: In the treatment of acute cerebral infarction, ccompared with urokinase, alteplase can further relieve cognitive impairment and promote the recovery of nerve function through inhibiting levels of inflammatory factors and levels of serum Hcy and MCP-1. doi: https://doi.org/10.12669/pjms.38.3.4521 How to cite this:Gao L, Zhang S, Wo X, Shen X, Tian Q, Wang G. Objectives: To explore the curative effect of minimally invasive percutaneous plate osteosynthesis (MIPPO) technology combined with locking compression plates (LCP) guided by C-arm X-ray machine in the treatment of tibial metaphyseal fracture and its impact on the surgical indexes and postoperative complications. Methods: The present study was a retrospective analysis focusing on the clinical data of 104 cases of tibial metaphyseal fracture who were treated surgically in our hospital from February 2019 to February 2021. According to different surgical methods, patients who were treated by using MIPPO technology combined with LCP guided by C-arm X-ray machine were divided into the observation group (n=54), while those who underwent traditional open reduction and internal fixation were classified into the control group (n=50). Further comparison was made on the differences of the curative effect, surgical index (operation time, intraoperative blood loss, incision length, and healing time) and postoperative complications between the two groups. In addition, the differences in ankle function, knee function and quality of life [Medical Outcomes Study 36-item Short-Form Health Survey (MOS SF-36)] were evaluated between the two groups before treatment and 6 months after treatment. Results: The clinical curative effect and total efficacy of the observation group were better than those of the control group (All p<0.05). The operation time, intraoperative blood loss, incision length, and healing time were lower in the observation group when compared to those of the control group (All p<0.05). The total incidence of postoperative complications was also lower in the observation group than that in the control group (p<0.05). In addition, the scores of ankle function, knee function and MOS SF-36 in both groups were higher after 6 months of treatment than those before treatment; besides, the inter-group comparison showed that the scores of ankle function, knee function and MOS SF-36 in the observation group were higher than those in the control group (All p<0.05). Conclusion: MIPPO technology combined with LCP guided by C-arm X-ray machine has a significant curative effect on the treatment of tibial metaphyseal fracture. It can significantly improve the surgical index, reduce postoperative complications, and have an obvious effect on postoperative recovery of ankle function, knee joint function and quality of life. doi: https://doi.org/10.12669/pjms.38.3.4757 How to cite this:Zhang X, Liu B, Wang Q, Shang H. In this study, dictionary learning and expectation maximization reconstruction (DLEM) was combined to denoise 64-slice spiral CT images, and results of coronary angiography (CAG) were used as standard to evaluate its clinical value in diagnosing coronary artery diseases. 120 patients with coronary heart disease (CHD) confirmed by CAG examination were retrospectively selected as the research subjects. According to the random number table method, the patients were divided into two groups: the control group was diagnosed by conventional 64-slice spiral CT images, and the observation group was diagnosed by 64-slice spiral CT images based on the DLEM algorithm, with 60 cases in both groups. With CAG examination results as the standard, the diagnostic effects of the two CT examination methods were compared. The results showed that when the number of iterations of maximum likelihood expectation maximization (MLEM) algorithm reached 50, the root mean square error (RMSE) and peak signal to noise ratio (PSNR) values were similar to the results obtained by the DLEM algorithm under a number of iterations of 10 when the RMSE and PSNR values were 18.9121 dB and 74.9911 dB, respectively. In the observation group, 28.33% (17/60) images were of grade 4 or above before processing; after processing, it was 70% (42/60), significantly higher than the proportion of high image quality before processing. The overall diagnostic consistency, sensitivity, specificity, and accuracy (88.33%, 86.67%, 80%, and 85%) of the observation group were better than those in the control group (60.46%, 62.5%, 58.33%, and 61.66%). In conclusion, the DLEM algorithm has good denoising effect on 64-slice spiral CT images, which significantly improves the accuracy in the diagnosis of coronary artery stenosis and has good clinical diagnostic value and is worth promoting.

Objective: Levosimendan is a novel drug often used to treat heart failure. We aimed to explore the effects of levosimendan preconditioning on left ventricular remodeling (LVR) after myocardial reperfusion in acute myocardial infarction (AMI) patients receiving the percutaneous coronary intervention (PCI). Methods: A total of 258 AMI patients treated from January 2018 to September 2020 were randomly divided into control and observation groups. Based on conventional drug therapy, levosimendan was given 30 min before PCI for the observation group, and dobutamine was intravenously injected for the control group. Baseline data, thrombolysis in myocardial infarction (TIMI) blood flow grade, myocardial injury markers, and LVR indices were compared, and the influencing factors for LVR were analyzed. Results: After treatment, various degrees of blood perfusion were found, and the TIMI grade was better than that before treatment in both groups (P < .05). The levels of aspartate aminotransferase, creatine kinase-MB, cardiac troponin T, and brain natriuretic peptide (BNP) declined in both groups, more significantly in the observation group (P < .05). Left ventricular end-systolic diameter, left ventricular end-diastolic diameter and left ventricular end-diastolic volume declined, whereas left ventricular ejection fraction rose in both groups, more significantly in the observation group (P < .05). Age and BNP were risk factors for LVR, whereas levosimendan preconditioning was a protective factor (P < .05). Conclusion: Levosimendan preconditioning can protect cardiac function and promote the recovery of the left ventricular structure. Age and BNP are risk factors for LVR after myocardial reperfusion in AMI patients undergoing PCI, and levosimendan preconditioning is a protective factor.
