SummaryAims. The recent publication of two large trials of secondary prevention of coronary artery disease with oral anticoagulants (WARIS and ASPECT) has caused a revival of the interest for this antithrombotic therapy in a clinical setting where the use of aspirin is common medical practice. Despite this, the preferential use of aspirin has been supported by an American cost-effectiveness analysis (JAMA 1995; 273: 965). Methods and Results. Using the same parameters used in that analysis and incidence of events from the Antiplatelet Trialists Collaboration and the ASPECT study, we re-evaluated the economic odds in favor of aspirin or oral anticoagulants in the Italian Health System, which differs significantly in cost allocation from the United States system and is, conversely, similar to other European settings. Recalculated costs associated with each therapy were 2,150 ECU/ patient/year for oral anticoagulants and 2,187 ECU/patient/year for aspirin. In our analysis, the higher costs of oral anticoagulants versus aspirin due to a moderate excess of bleeding (about 10 ECU/ patient/year) and the monitoring of therapy (168 ECU/ patient/year) are more than offset by an alleged savings for recurrent ischemic syndromes and interventional procedures (249 ECU/ patient/year). Conclusions. Preference of aspirin vs. oral anticoagulants in a pharmaco-economical perspective is highly dependent on the geographical situation whereupon calculations are based. On a pure cost-effectiveness basis, and in the absence of data of direct comparisons between aspirin alone versus I.N.R.-adjusted oral anticoagulants, the latter are not more expensive than aspirin in Italy and, by cost comparisons, in other European countries in the setting of post-myocardial infarction.

Subject. The article focuses on the advanced time-driven tools for allocating overhead expenses, which are based on process-based budgeting. Objectives. We articulate a technique for cost allocation so as to assess the cost of each process with reference to the common time driver. Methods. The study relies upon methods of systematization, classification, analogy and comparison, and summarizes the scientific literature on the subject. Results. The article presents our own suggestions on implementing TD-ABC and TD-ABB into the strategic management accounting process of developer companies. The principles were proved to help more effectively allocate overhead expenses and assess the capacity load of each process performed by functions, departments and employees. Carrying out a comparative analysis, we found certain reserves for utilizing resources more effectively. Conclusions and Relevance. The findings are of scientific and practical significance and can be used by developer and construction businesses. The conclusions can prove helpful for scientific papers, student books, and further research.


The research is actual, has a theoretical and applied nature. Theoretical developments at cost management branch of economic organization were illustrated by practical examples. In the work the follow methods are used: abstract-logical, methods of induction and deduction, system and situate approaches, methods of comparative analysis, analysis of breakeven, monographic analysis. The follow sources of information were used: literature, the results of the experimental investigations, carried out in Verkh-nevolzhsky Federal Agrarian Research Centre, observations, carried out at an agricultural organization. Theoretical structural model of cost management was created in a functional way, relationship between elements were designated in it, as well as driving forces of its realization were determined. Such terms as “cost management ” and «management accounting» were specified. The paradigm of a relation to cost process was described. Approaches to realization of cost management system of organization on the basis offlexibility principle were developed. The most important of them are: application of applied programs packages and special program means for computer, organization of feedback, account of functional relationship of cost with production results, cost accounting on elements, places of origin, carriers and centers of responsibility, as well as interactive approach. Some examples which show possibilities of using some instruments of cost management and management accounting for generation of management solutions were described. These instruments are: analysis and planning on the basis of standards of constant and variable costs and flexible cost management in interrelations to other subsystems in management system of organization (in this case it is with technology management. The work is of theoretical and practical significance.
