ABSTRACTThis study aims at displaying the results of reviewing research-based articles about gamification in the context of learning English. The concept of games has changed from a self-entertainment into an edutainment through adjustments and modifications. This notion is adapted by teachers in big countries such as Poland, China, and the US to enhance their English class with surprisingly ensuing positive results (Rothwell & Shaffer, 2019). These results have triggered the writer to find out more about the benefits of gamification and to conduct qualitative research through the review process. The writer collected and analyzed thirteen relevant articles from journals and websites. The results indicated that gamification indeed brought benefits to students. The foremost results included motivation improvement, promoting positive attitudes and better performances, fostering 21st-century skills and better cognitive achievements, encouraging social interaction and independencies, and improving competitiveness among students during the learning process.ABSTRAKPenelitian ini bertujuan untuk memaparkan hasil dari telaah artikel berbasis riset tentang gamifikasi pada konteks pembelajaran Bahasa Inggris. Purpose Sentinel node (SN) status is the most important prognostic factor for overall survival (OS) for patients with stage I/II melanoma, and the role of the SN procedure as a staging procedure has long been established. However, a less invasive procedure, such as ultrasound (US) -guided fine-needle aspiration cytology (FNAC), would be preferred. The aim of this study was to evaluate the accuracy of US-guided FNAC and compare the results with histology after SN surgery was performed in all patients. Patients and Methods Four hundred consecutive patients who underwent lymphoscintigraphy subsequently underwent a US examination before the SN procedure. When the US examination showed a suspicious or malignant pattern, patients underwent an FNAC. Median Breslow thickness was 1.8 mm; mean follow-up was 42 months (range, 4 to 82 months). We considered the US-guided FNAC positive if either US and/or FNAC were positive. If US was suggestive of abnormality, but FNAC was negative, the US-guided FNAC was considered negative. Results US-guided FNAC identified 51 (65%) of 79 SN metastases. Specificity was 99% (317 of 321), with a positive predictive value of 93% and negative predictive value of 92%. SN-positive identification rate by US-guided FNAC increased from 40% in stage pT1a/b disease to 79% in stage pT4a/b disease. US-guided FNAC detected SN tumors more than 1.0 mm in 86% of cases, SN tumors of 0.1 to 1.0 mm in 46% of cases, and SN tumors less than 0.1 mm in 23% of cases. Estimated 5-year OS rates were 92% for patients with negative US-guided FNAC results and 51% for patients with positive results. Conclusion US-guided FNAC of SNs is highly accurate. Up to 65% of the patients with SN-positive results in our institution could have been spared an SN procedure.

Tuberculosis (TB) is a cause of ill health and death worldwide. Since 2010, the diagnostic process has strongly relied on GeneXpert assays on biological specimens. Xpert MTB/RIF is an automated nucleic acid amplification test (NAAT) for Mycobacterium tuberculosis and rifampicin resistance, endorsed by the World Health Organization and the US Food and Drug Administration. Xpert is used in many countries as the initial diagnostic test for tuberculosis. Nevertheless, the reliability of GeneXpert positive tests in patients with a history of TB is largely unknown, due to possible false-positive results (i.e., GeneXpert-positive but culture-negative patients). We present a case report of a patient with a history of pulmonary TB, who was GeneXpert positive but culture negative on bronchoalveolar lavage 22 months after completion of appropriate antitubercular therapy.

The US-based authors argue that the practice of what we currently call “international service-learning” does not generally achieve its most important goals in the context of the global South, especially those based on the development of mutually beneficial relationships with host communities. The primary impediments to achieving this goal in any interaction between students and vulnerable peoples are: (1) the belief that a student’s reflection on their experience is sufficient, both to evaluate community impact and to promote student learning; (2) the assumption that service always has positive results; and most critically; (3) the concept of service itself. While something new and positive can be created to replace international service-learning, the changes required to do so are so fundamental that the result will be something completely different from the way that international service-learning is conducted today. We argue for the need to reframe international service-learning with a focus on reciprocal learning or risk continuing to function as a neocolonial practice with likely harm to communities and to our students as we engage in practices that reify global inequality.

The article investigates the incentive tourism as one of the effective ways of non-monetary encouragement of staff. The author defines the incentive tourism as one of the components of business tourism and the considers the basic features of the organization of incentive tour. The article presents the statements of well-known motivation theories and based on them analyzes incentive tourism as a way of motivation. The author proves that incentive programs can meet the specific needs of an employee, encouraging to a more qualitative and productive work. The article also notes the high valence of incentive tourism, and explains why this type of tourism is not well developed in Russia in comparison with the US and European countries. The author concludes that the using by companies incentive tourism programs not only can positively affect the indoor climate in the team, but also have a positive impact on strengthening the company´s commitment, improving the quality of work, professional-level rise, and this, in turn, necessarily affect on the favorable development and prosperity of the organization. Thus, the use by organizations of incentive programs is regarded as an effective investment, bringing positive results over the long term.

This study describes audiologic methodology and results for evaluating potential ototoxicity in a phase I clinical trial of a new glycopeptide. This study was conducted under good clinical practices, which are regulated by the US Food and Drug Administration (FDA) (21 Code of Federal Regulations), and input from the FDA was sought prior to study implementation. Healthy, normal volunteers underwent extensive medical and audiologic assessments as part of this phase I dose- escalation study of dalbavancin, a new glycopeptide, to assess potential side effects. Audiologic monitoring included air-conduction thresholds in the conventional (0.25-8 kHz) and high-frequency (10-16 kHz) ranges. At baseline, subjects were also tested using word recognition, bone conduction testing if indicated, and tympanometry. Full testing was to be repeated if any subject met the American Speech-language-Hearing Association (ASHA) 1994 criteria for ototoxic change. However, no subjects demonstrated ototoxic change after receiving dalbavancin, nor were any false-positive results obtained.

The Nursing Practice Environment (NPE) influences the quality of nursing care. Studies show far better outcomes for clients, with positive results on nursing teams’ satisfaction and patients with improvements in the outcomes of patients and healthcare organizations. The Practice Environment Scale-Nursing Work Index (PES-NWI) is an instrument that measures the nursing practice environment and is currently the most used at a global scale. The analyzed article proves the instrumentsreliability in the assessment of the NPE, despite needing further investigation at a psychometric level; and its applicability in different care settings – Other countries besides the US. However, it is emphasized the need to carry out further experimental and longitudinal studies to potentially identify the mechanisms that influence the NPE.

Cerebrovascular thromboembolism is responsible annually for 510.000 ischaemic stroke in the united states alone. PFO mechanism as a paradoxical embolism transit from right to left-sided chambers to intracranial vessels has a tremendous impact in neurological deficits. The aggressive treatment started since 2016 when the US Food and Drug Administration (FDA) approved the Amplatzer PFO occluder for recurrent stroke prevention of cryptogenic stroke with PFO. The trials show positive results since 2017 and the collaboration and partnership between neurologist and cardiologist are more needed to build a holistic and comprehensive treatment for cryptogenic stroke patient with PFO.

Background: Progress in immunotherapy (IT) has shifted treatment paradigms for multiple malignancies. In March 2019, the combination of nab-paclitaxel and atezolizumab was approved by the US FDA for patients with PD-L1 positive metastatic triple-negative breast cancer based on positive results of the Impassion130 trial. Although numerous studies have examined the prognostic role of PD-L1, the value of this test remains controversial. Results: Here, we presented the cases of three heavily pretreated women with metastatic triple-negative breast cancer who exhibited remarkable responses to combined IT and chemotherapy despite undetectable PD-L1. Conclusion: In our opinion, the current FDA-approved assessment for PD-L1 expression is a reasonable tool for deciding whether to start IT. However, because this approach has many limitations, patients with undetectable PD-L1 expression should still be considered for IT.
