ABSTRACT Objective:  To compare the outcome of mini implant placement by four different methods: blind placement, a single periapical radiograph (PA), a single panoramic radiograph, and a small-volume cone-beam computed tomography (SV-CBCT). Our hypothesis was that SV-CBCT, with its high resolution, low radiation dose, and three-dimensional depiction of area of interest would yield superior diagnostic information in assessing the potential anchorage site compared to currently used methods that often result in undesired root perforations. Materials and Methods:  Potential mini implant sites of 20 dentate or partially dentate human skulls were imaged using three different imaging modalities: PA, panoramic radiograph, and SV-CBCT. Mini implants were placed in 10 maxillary and 10 mandibular randomized sites blindly and using each of the three imaging modalities. Large-volume CBCT scans done postoperatively were used to detect root perforation. Two oral radiologists analyzed the images for perforation of root structures at each site. Results:  There was significantly (P < .05) less root perforation with SV-CBCT when compared with other imaging modalities. Fifty-five percent of mini implants placed blindly, 60% of mini implants placed using PA, and 50% of mini implants placed using a panoramic radiograph perforated a root structure, whereas only 5% of mini implants placed using SV-CBCT perforated a root structure. Conclusions:  Preoperative evaluation of potential mini implant insertion sites using SV-CBCT aids in predictable placement and results in the least amount of root perforation.

Abstract Objective: To report the occurrence of taurodontism in a clinical sample of Van der Woude syndrome (VWS) and describe its association with hypodontia and cleft type. Materials and Methods: This retrospective, cross-sectional study was carried out on chart reviews and radiographs of 13 persons with VWS. Mean age was 10 years 11 months ± 1 year 5 months. Panoramic radiographs were used to confirm the presence or absence of teeth and to measure crown body and root lengths of mandibular first molars. Three-dimensional cone beam computed tomography (CT) scans were available on two persons with VWS. Both volumetric and linear measurements were obtained. Results: The occurrence of taurodontism of the mandibular first molar was 35%: 27% hypodont and 8% mesodont. Of the 13 subjects with VWS, 6 (4 males and 2 females) had at least one tooth identified with taurodontism. Half of the cases were unilateral and half were bilateral, and all of the unilateral cases were on the left side. Five of the six subjects with taurodontism had missing incisors and premolars. Taurodontism was two times more frequent in those who were missing their second premolars than in those who had their second premolars. There was no correlation between cleft type and presence of taurodontism. The cone beam CT pilot study on two persons showed very abnormal morphology of both crown and roots, which was not apparent on the standard panoramic radiograph. Both the volumetric and linear measurements of the ratio of crown body to root were highly indicative of taurodontism. Further genetic studies are needed. Conclusion: There is a likely association between VWS and taurodontism.

Background: Clinicians should assess and monitor mandibular canal (MC) to achieve an uneventful and successful final result. Two dimensional techniques like panoramic radiography, intra oral periapical radiographs are commonly used for assessment of MC, along with three dimensional techniques like cone beam computed tomography (CBCT).Objectives: To assess the radiologic modalities and assessment criteria used by clinically practicing dental surgeons when determining the proximity of MC to third molars.Method: A questionnaire based study was carried out among national and international oral maxillofacial surgeons and general dental practitioners. Data were fed to SPSS software version 21 for descriptive statistics.Result: Most of the participants (54.7%) in the study preferred combination of panoramic radiograph and CBCT for determining the relationship of mandibular canal and third molars measures.Conclusion: Initial screening can be carried out using panoramic radiograph which is a popular method in determining the relation between the third molar and mandibular canal in spite of some limitations. Cone Beam Computed Tomography is an important three-dimensional imaging technique which is a preferred method amongst most dental surgeons.Journal of BP Koirala Institute of Health Sciences, Vol. 1, No. 1, 2018, page: 44-50

The middle third of the facial skeleton is one of the most complex areas of the human body. A critical factor determining the successful treatment of midfacial fractures is an early and correct diagnosis. Assessment and management of the traumatized patient requires rapid evaluation and decision making. Imaging for the evaluation of patients with midfacial fractures has contributed to achieving an accurate diagnosis and a subsequent successful management. It requires a knowledge of those imaging modalities and projections that provide the most information with the least exposure of radiation to the patient. Conventional radiography has been used successfully for many years although advanced imaging methods such as—three-dimensional computed tomography, magnetic resonance imaging, and cone beam computed tomography have been applied more recently. In this article, we will take a relook at various modalities to image midfacial fractures, and discuss the rationale for selection of those procedures that offer the greatest diagnostic information.

Background: A 25-year-old man presented to the Department of Oral Medicine and Radiology with a painful and persistent discharge under the tongue on right side of the mouth. The patient complained of repeated episodes of pain before, during and after food intake with a recent episode of fever. Panoramic radiograph showed a radio-opaque mass superimposing over right mandibular angle region (Figure 1). Cone Beam Computed Tomography (CBCT) was obtained for the description and location of the radio-opaque mass. Multiple planar images were constructed which showed a round sialolith measuring around 22 mm x 18mm, in coronal section (Figure 2) and Three-Dimensional (3D) reconstruction (Figure 3). This work compared the assessment of clinically relevant parameters by two-dimensional, that is, full-mouth intraoral radiograph (I-O) and panoramic radiograph (OPT), and three-dimensional, that is, cone beam computed tomography (CBCT), imaging methods. Different radiographic images (CBCT, I-O and OPT) were available for a 53-year-old female patient with dental and periodontal problems. A total of 14 dental and periodontal parameters were assessed by two independent examiners and compared among the three radiographic imaging modalities. For 10 parameters (71%), the CBCT images were superior to both I-O and OPT images. In contrast, CBCT demonstrated an inferior performance compared to I-O and OPT in the assessment of caries and dental restorations. Compared to OPT, I-O provided more clinically relevant findings for 10 out of 14 parameters (71%). Agreement between I-O and OPT was found with respect to dehiscence, fenestration, the number of bone walls and the root canal cross-section. Differences between the radiographic images were more likely to be detected when maxillary teeth rather than mandibular teeth were assessed with regard to furcation involvement, root proximity and root fusion.

The use of X-ray radiation is the main additional method for complete visualization of hard and soft tissues in dentistry and maxillofacial surgery. Cone-beam computed tomography (CBCT) is of leading importance among other methods. Three-dimensional reformation of CBCT data into a multiplanar or three-dimensional image of anatomical structures allows for a qualitative assessment of the area of interest and to choose the right treatment tactics if necessary. Nevertheless, there are a number of factors or artifacts that lead to distortion, disrupt the image or lead to loss of information during CBCT, in addition, the formation of overdiagnosis is not excluded. Currently, an active search is underway for ways to improve the quality of CBCT images and eliminate artifacts to improve the quality of diagnostics. This review is devoted to the description of the main causes of artifacts and modern methods of their elimination.
