The article is focused on the main parameters hemostasis and platelet count of newborns with congenital heart diseases who could require an early cardiac surgery during the first 12 hours of their life. Some known disorders of hemostasis in children with congenital heart diseases are described. A clinical case of the thrombotic complications in the child after cardiac surgery are presented, the tactic of drug therapy is described. The study was approved by the Independent Ethics Committee of the V.I. Kulakov Obstetrics, Gynecology and Perinatology Research Centre.

Background: Acute kidney injury (AKI) is one of the most significant postoperative complications of pediatric cardiac surgery. Because serum creatinine has limitations as a diagnostic marker of AKI, new biomarkers including neutrophil gelatinase-associated lipocalin (NGAL), kidney injury molecule-1 (KIM-1), and interleukin-18 (IL-18) are being evaluated to overcome these limitations and detect AKI at an early stage after cardiac surgery.Purpose: This study aimed to investigate the clinical usefulness of these biomarkers in young children.Methods: Thirty patients with congenital heart diseases who underwent cardiac surgery using cardiopulmonary bypass (CPB) were selected, and their urine and blood samples were collected at baseline and 6, 24, and 48 hours after surgery. Serum creatinine and blood urea nitrogen levels as well as NGAL, KIM-1, and IL-18 levels in urine samples were measured, and clinical parameters were evaluated.Results: Of the 30 patients, 12 developed AKI within 48 hours after cardiac surgery. In the AKI group, 8 of 12 (66.6%) met AKI criteria after 24 hours, and urine KIM-1/creatinine (Cr) level (with adjustment of urine creatinine) peaked at 24 hours with significant difference from baseline level. Additionally, urine KIM-1/Cr level in the AKI group was significantly higher than in the non-AKI group at 6 hours. However, urine NGAL/Cr and IL-18/Cr levels showed no specific trend with time for 48 hours after cardiac surgery.Conclusion: It is suggested that urine KIM-1/Cr concentration could be considered a good biomarker for early AKI prediction after open cardiac surgery using CPB in young children with congenital heart diseases.

Abstract Background: Postoperative thrombocytopenia with congenital heart disease (CHD) after percutaneous transcatheter closure (PTC) has seldom been reported. We sought to investigate the treatments, severity and causes of postoperative thrombocytopenia among patients with CHD undergoing PTC.Methods: Six PTC patients undergoing postoperative thrombocytopenia in our department were included. Platelet count was measured at admission and daily after PTC at least for day 9.Results: Three patients had platelet counts below 30×10^9 /L while in the other two patients the counts were below 50×10^9 /L. All patients achieved a stable platelet count, ranging from 99×10^9 /L to 241×10^9 /L after day 9. The platelet counts recovered after being given intravenously the methylprednisolone (two patients), immunoglobulin (one patient) and platelet transfusions (two patients). During the whole course, the face and limbs of one patients were visible ecchymosis but no major bleeding events were recorded.Conclusion: In this report, the clinical outcome of our patients were satisfactory; they complaints regressed after above treatment.

The organization of cardiologic and cardio surgical care for children and adolescents in Republican Heart Center of Republic of Bashkortostan is characterized. The hospital has a united block for providing specialized medical care. Departments of cardio surgery, pediatric cardiology and intensive care unit №1 are based on the same floor. The advantages of staged approach and continuity of care for children and adolescents with cardiovascular diseases is emphasized. Registers for prenatal diagnosis of congenital heart diseases (includes 7894 children and adolescents by December 31, 2012), for pulmonary hypertension are developed and used in the Republican Heart Center for timely cardio surgery in patients with congenital heart diseases. The main directions of medical care for children with cardiovascular diseases in Republic heart center are cardiac surgery of congenital heart diseases, pediatric cardiology, radiosurgery of congenital heart diseases, emergency cardiology and cardiac surgery, and neonatal cardiac surgery. 4500 cardiovascular surgeries are performed annually, including 398 on children aged 0-17 years, among them 224 on assisted circulation. Department of cardio surgery annually performs about 100 surgeries on infant patients with «critical» congenital heart diseases, contributing up to 35% of all children who underwent surgery, including 30 surgeries per year on newborns. The outpatient clinics examines over 15 thousand children per year. The basis of the successful maintaining the public health is good teamwork of highly specialized professionals in the atmosphere of a clear motivation, enthusiasm and high morals.
