SummaryBackground/Aim: Molar incisor hypomineralization (MIH) is a qualitative defect of systemic origin, affecting permanent first molars and often permanent incisors. The treatment modalities can include, amongst others, fissure sealants for prevention of dental caries and composite restorations. Both require adhesion to tooth structure. The aim of this study was to review the literature on the adhesion to enamel affected by MIH.Material and Methods: A search of PupMed/Medline, ResearchGate and Google Scholar was performed and limited between 2003, when the judgement criteria for MIH were set, and 2016. Thirty-three papers were considered relevant to the subject including five in vivo and six in vitro studies. Studies involving less than ten teeth were excluded.Results: A four-year clinical trial showed that the application of a total-etch 2-step adhesive system prior to sealant placement is superior to the etch-seal technique. Despite the high success rate of composite restorations shown in three clinical longitudinal studies, there are conflicting results over self-etch being superior to total etch adhesive systems. Pretreating the enamel surface, prior to the adhesive system, with fluoride preventive solutions could reduce the mikroleakage under orthodontic brackets. Three in vitro studies provide inconsistent data about NaOCl pretreating potentials to improve adhesion of composite restorations. Resin infiltration, prior to resin restorations, could improve the microhardness of defected enamel, which may lead to increased bond strength, especially in combination with NaOCl pretreatment.Conclusions: Adhesion to enamel affected by molar incisor hypomeralization is inferior compared to normal enamel. Sealants applied with the etch-bond-seal technique have greater retention than with the etch-seal technique. Further research is required to provide evidence of the effectiveness of the adhesive system and pretreatment to achieve optimal bonding to MIH.

SUMMARY Introduction The aim of this study was to evaluate marginal sealing ability of newly introduced Dyad Flow flowable composite and compare to total-etch and self-etch adhesive system at the coronal and apical margins of class V resin composite restorations. Material and Methods A standard class V cavity (3 mm mesiodistal width, 3 mm occlusogingival height and 1.5 mm axial depth) was prepared on the buccal surface of freshly extracted sound human teeth. Occlusal and gingival margins of the cavities were located in enamel and cementum/dentin, respectively. Teeth were randomly assigned into the three groups (n=10) and restored with different composite materials following the manufacturer’s instructions: group I was restored with nanohybrid resin composite using total-etch bonding agent; group II was restored with nanohybrid resin composite using self-etch technique; group III was restored with flowable composite (Dyad Flow), respectively. After finishing and polishing, the teeth were coated with nail varnish and immersed in rhodamine B dye and sectioned longitudinally. Dye penetration was examined under stereomicroscope and scored separately for occlusal and gingival margins on a 0–3 ordinal scale. Data were analyzed with Kruskal–Wallis, Mann–Whitney and Wilcoxon tests (a=0.05). Result Statistical analysis showed that specimens restored with the total-etch and self-etch adhesive systems revealed reduced leakage at the coronal margin. At the apical margin, Dyad Flow showed greater leakage than other groups. Conclusion Newly introduced flowable composite Dyad Flow showed inferior adhesive bond with enamel and dentin compared to total-etch and self-etch techniques.

The aim of this study was to assess the effect of radiotherapy on bond efficiency of two different adhesive systems using tensile bond strength test. Twenty extracted teeth after radiotherapy and twenty nonirradiated extracted teeth were used. The irradiation was appliedin vivoto a minimal dose of 50 Gy. The specimens of each group were randomly assigned to two subgroups to test two different adhesive systems. A three-step/etch-and-rinse adhesive system (Optibond FL) and a two-steps/self-etch adhesive system (Optibond XTR) were used. Composite buildups were performed with a nanohybrid composite (Herculite XTR). All specimens were submitted to thermocycling ageing (10000 cycles). The specimens were sectioned in 1 mm2sticks. Microtensile bond strength tests were measured. Nonparametric statistical analyses were performed due to nonnormality of data. Optibond XTR on irradiated and nonirradiated teeth did not show any significant differences. However, Optibond FL bond strength was more effective on nonirradiated teeth than on irradiated teeth. Within the limitations of anin vitrostudy, it can be concluded that radiotherapy had a significant detrimental effect on bond strength to human dentin. However, it seems that adhesive choice could be adapted to the substrata. According to the present study, the two-steps/self-etch (Optibond XTR) adhesive system tested could be more effective on irradiated dentin compared to three-steps/etch-and-rinse adhesive system (Optibond FL).

Early childhood caries commonly affects the primary maxillary anterior teeth, in which extensive post- endodontic rehabilitation has always been a challenge. This is due to the small size of the crown, relatively large pulp chambers, and the age of the child. The remaining inadequate sound tooth structure renders restorations more prone to fractures. Among the available treatment modalities which include extraction of the grossly destructed tooth structure, use of semipermanent crowns and intracanal posts, the posts can be used effectively, to restore the lost function and anatomy. They also enhance the quality of the final coronal restoration by providing additional support for retention. The available methods for intracanal reinforcement for anterior teeth include the metal posts fabricated using orthodontic wires, short composite posts fabricated directed in the primary root canal, prefabricated glass fiber posts and posts utilising polyethylene fibers (Ribbond). Recently certain advanced techniques such as the biologic dentin and zirconia have also come into use. The various advantages and disadvantages for these have been summarised in this review along with the standardised clinical protocol for their placement in the primary root canals. Various in-vivo and in-vitro studies have concluded that the glass-fiber short intracanal posts provide the most suitable esthetics, strength, elastic modulus, and resilience. However, further qualitative research is required to certify it as a standard treatment modality.

SUMMARY This study evaluated the effect of 2% chlorhexidine (CHX) pretreatment of dentin on the immediate and aged microtensile bond strength (μTBS) of different adhesives to dentin in vivo and in vitro. Class I cavities were prepared in 80 caries-free human third molars of 40 patients in a split-mouth fashion. In each tooth pair, one tooth received 2% CHX pretreatment after which both teeth were randomly assigned to one of the following groups with respect to the type of adhesive system applied: Adper Single Bond 2 (etch-and-rinse), Clearfil SE Bond (two-step self-etch), Clearfil S3 Bond (one-step self-etch), and Adper Prompt-L-Pop (all-in-one self-etch). The teeth were restored with resin composite and extracted for μTBS testing either immediately or after six months in function. In vitro specimen pairs were prepared as with the clinical protocol in intact, freshly extracted human molars, and thereafter, subjected to testing immediately or after 5000× thermocycling. Data were analyzed with four-way analysis of variance (ANOVA). Bonferroni test was utilized for pair-wise comparisons. The immediate bond strength values were significantly higher than “aged” ones for all tested adhesives (p=0.00). The in vitro immediate bond strength values were statistically higher than in vivo bond strength values (p<0.05). While the bond strength of in vitro aged, CHX-treated samples were higher than their in vivo counterparts (p<0.05), no difference was observed in non-CHX treated groups (p>0.05). In the absence of CHX pretreatment, all adhesives showed significantly higher immediate bond strength values than CHX-treated groups, while all “aged”, non-pretreated adhesives exhibited significantly lower bond strength values (both p<0.05). By contrast, chlorhexidine pretreatment resulted in significantly higher aged bond strengths, regardless of the adhesive system and testing condition. Aging-associated decline in dentin bond strength of etch-and rinse and self-etch adhesives can be counteracted by chlorhexidine application.

SummaryAn account is given of the effects of boomslang venom in man. Evidence was found of a fibrinolytic state apparently secondary to the coagulant action of the venom. These features rapidly responded to the administration of specific antivenom. In vitro studies, using a homogenate of boomslang parotids, confirmed the coagulant properties of the venom and showed them to be of much greater potency than the proteolytic actions.
