AbstractIntroduction. Nurses’ entitlement to write prescriptions and to provide nursing advice in primary health care can improve the quality of these services. In the process of ensuring the quality of healthcare, the desirable element of its assessment is the patient’s perspective, taking into account his expectations.Aim. Determining the scope of expected nursing advice and learning the opinions of patients regarding the writing of prescriptions by family nurses.Material and methods. A qualitative study was conducted using a semi-structured interview with 37 patients under the care of eight primary health care outpatient clinics from the Podlaskie Voivodeship.Results. The scope of advice expected from a family nurse concerns: dealing with a health problem, lifestyle and prevention, the scope of family nurse care and the operation of the healthcare system. Writing prescriptions by family nurses can improve the functioning of primary health care.Conclusions. Nursing advice perceived by patients is in line with the competences of the family nurse. Opinions on the entitlement of the family nurse to write prescriptions and prescribing medicines vary.

The aim of the study was to evaluate patients’ satisfaction with the quality of provided services in private primary health care institutions in Kaunas. Material and Methods. A questionnaire-based inquiry of 280 persons registered to family physicians at primary health care settings was performed. The study was carried out using 20-item anonymous questionnaires with questions about the quality of services provided in primary health care settings. Results. More than 50.0% of the respondents stated that they waited for more than 15 minutes at the physician’s office, while 17.0% of the respondents stated that the waiting time exceeded 30 minutes. More than 25.0% of the respondents positively evaluated the possibility to consult their family physician by phone. In 67.0% of patients, the family physician determined the cause of the disorder and administered treatment; in 32.0% of patients, the family physician referred them to a specialist, and 1.0% of patients were urgently sent to hospital. More than 90.0% of the respondents were satisfied with the services provided by their family physicians. Those who were dissatisfied with these services indicated that the provided treatment failed to eliminate the disorder, that they wanted to be referred to a specialist, and that they expected more diagnostic tests to be performed for more effective treatment. Conclusions. A greater part of the patients indicated that the main reason for long waiting at the physician’s office was physicians’ wish to serve too many patients. More than two-thirds (67.0%) of the patients stated that their family physicians determined the cause of the disorder and prescribed treatment. The overwhelming majority (more than 90.0%) of the patients were satisfied with the services provided by their family physicians.

Resumo: Objetivou-se identificar que serviços as Unidades Básicas de Saúde da Família ofereciam aos idosos e qual o perfil dos profissionais em Campina Grande-PB, tendo como método de coleta a entrevista. Obtivemos um perfil sociodemográfico dos profissionais de acordo com o gênero: 88% feminino, 12% masculino; como estratégia mais utilizada por esses profissionais, observou-se a utilização de ações em grupo em 76% da amostra, porém, a caderneta do idoso era utilizada em apenas 12% dos casos. Abstract Background: Tanzania is among the countries that adopted task sharing as a strategy of addressing health workforce shortage. Although the strategy has existed for over five decades, concerns are upon the quality of the training of the mid-level cadres amidst the growing number of medical universities. This study sought to explore the challenges facing the training of the Assistant Medical Offices, AMOs (a mid-level cadre) in Tanzania.Methods: An exploratory qualitative case study was carried out in four regions to include one rural district in each of the selected regions and two AMOs Training Colleges in Tanzania. A semi-structured interview guide was used to interview 29 Key informants from the district hospitals, district management, regional management, AMOs training college and one retired AMO. In addition, four focus group discussions were conducted with 35 AMO trainees.Results: Training of AMOs in Tanzania faces many challenges. The challenges include; use of outdated and static curriculum, inadequate tutors (lack of teaching skills and experience of teaching adults), Inadequate teaching infrastructure in existence of many other trainees to include interns, and limited or lack of scholarships and sponsorship for the AMO trainees. Conclusions: The challenges facing AMO training not only affect the quality of the graduates but also affect the realization of task sharing strategy that requires that task sharing should not compromise the quality of services produced by the task-shared cadre. It is high time for revising the AMOs curricula and train the tutor through continued medical education programmes to reflect the dynamics of medical education. The government in collaboration with other stakeholders should work together to address the challenges on teaching infrastructure and scholarships to this cadre that has continued to be the backbone of the primary health care in Tanzania. The aim of this work was to characterize the views of nurses about factors modulating smoking cessation. Results of this study will allow us to design helping interventions with the maximum specificity for nurses. A qualitative study through a semi-structured interview of 15 Primary Health Care nurses who were smokers was performed. In contrast with other studies in which nurses were not aware of any particular social pressure to give up smoking, 18 months after the application of the Anti-Smoking Spanish Law, this feeling was expressed. Therefore, the main reasons for giving up smoking include that smoking in public is every day worse seen, together with a sense of shame and guilt in front of their social and family environment, especially for being a professional group dedicated to health.

Different mix of clients visit primary health care (PHC) facilities, and the quality of services is critical even in rural communities. The study objective was to determine the relationship between socio-demographic characteristics and client satisfaction with the quality of PHC services in Calabar Municipality, Cross River State, Nigeria. Specifically to describe aspects of the health facilities that affect client satisfaction; determine the health-care providers’ attitude that influences client satisfaction; and determine the socio-demographic characteristics that influence client satisfaction with PHC services. A cross-sectional survey was adopted. Ten PHCs and 500 clients utilizing services in PHC centers in Calabar Municipality were randomly selected. Clients overall satisfaction with PHC services was high (80.8%). Divorced clients were less (75.0%) satisfied than the singles and the married counterparts (81%), respectively. Clients that were more literate as well as those with higher income were less satisfied, 68.0% and 50.0%, respectively, compared to the less educated and lower-income clients, 92.0% and 85.0% respectively. These differences in satisfaction were statistically significant (P = 0.001). Hence, it was shown that client characteristics such as income and literacy level show a significant negative relationship with the clients satisfaction with the quality of PHC services in Calabar Municipality.

BACKGROUND: Diabetes mellitus (DM) is a lifelong metabolic disease with a high rate of mortality and morbidity. Uncontrolled and untreated diabetes results in serious complications that subsequently cause patients’ quality of life (QoL) to deteriorate. Adherence to Mediterranean diet (MD) may relieve the complications of diabetes, thereby improving the quality of life for these patients. OBJECTIVE: The aim of this study was to assess the QoL of DM patients who adhered to MD. METHODS: In this cross-sectional study, we examined the QoL and MD data of 106 DM II patients being treated at a primary health care clinic in Hebron. We used the SF-36 questionnaire to measure the patients’ QoL and the MEDAS tool to assess their MD adherence. We also recorded their anthropometric measurements, abdominal obesity, lifestyle habits and blood biochemical results. RESULTS: The sample comprised male and female DM II patients between the ages of 35 and 72, with their mean age being 55.8±10.24. Patients’ QoL scores showed a significant relationship with three BMI categories, i.e., total QoL score, physical function, and pain domains (p <  0.05). In terms of diet, high adherence to MD had a positive impact on all domains and on patients’ total QoL with significant differences in physical functioning, emotional well-being, social functioning and pain domains. CONCLUSION: Patients’ QoL domains were relatively low and highly affected by DM II. Patients with greater MD adherence reported higher scores in all QoL domains. Significantly higher scores were noted for the physical, social and pain domains. Hence, MD is a recommended dietary pattern for DM II patients to achieve a better QoL.

Introduction. Sudden cardiac arrest (SCA) in out-of-hospital conditions is a frequent condition of immediate life-emergency of cardiological background. The survival and discharge rate in SCA patients treated by medical services is still at a low level. Aim. The aim of the study was to analyze selected parameters of the quality of chest compressions and relaxation performed by nursing staff as part of out-of-hospital SCA. Material and methods. The study covered a group of 48 nurses working in primary health care in branches of conservative (PRE/C = 24 people, mean age 33.04 ± 9.20) and surgery profile (PRE/I = 24 people, average age 31.88 ± 9.97), participating in the research project of the Center for Medical and Natural Sciences Research and Innovation, University of Rzeszów in Rzeszów. Two months after the initial examination and intervention, the test was repeated. The assessment of selected compression and relaxation parameters was performed using the True CPR Resuscitation Assistant. The statistical analysis of the collected material was carried out with the SPSS Statistics 20 software. The statistical analysis used the analysis of variables using parametric and non-parametric tests. The statistical significance was assumed at p < 0.05. Results. Initially, a high percentage of the effectiveness of the most important compression and relaxation parameters of the chest was obtained during 2-minute out-of-hospital cardiopulmonary resuscitation with no statistically significant differences between the two groups (PRE/C: 51 mm depth, 83.92% relaxation, frequency 115.46 vs. PRE/I: depth: 47.58 mm, relaxation: 92.54, frequency: 109.96, p < 0.05). After two months, similar parametric values were observed with no statistical significance (p < 0.05). Conclusions. The initial high quality of compressions and relaxation was found in nurses participating in the simulated out-of-hospital SCA, while there were no significant differences between primary health care nurses working at conservative departments and nurses from surgical ones. After a two-month interval from the initial examination and training, high CPR effectiveness was observed based on the guidelines of the European Resuscitation Council from 2015.
