Acute bacterial meningitis is a medical emergency that warrants an early diagnosis and aggressive therapy. Despite the availability of potent newer antibiotics, the mortality caused by acute meningitis and its complication remains high in India, ranging from 16% to 32%. The aim of this case report is to present the isolation of Acinetobacter baumannii from cerebrospinal fluid(CSF) of an elderly male following trauma without any neurosurgical intervention and without any evident cerebrospinal fluid otorrhea or rhinorrhea. Objective: The aim of this report is to demonstrate that acute rhino-sinusitis may result in orbital cellulitis and even life threatening complications especially intracranial abscesses in children and young adults. Rare complications such as subperiosteal abscess seen in this patient may also occur Morbidity and mortality can be prevented through early diagnosis and treatment by relevant specialists. Method: We present a case report and literature review on unilateral pansinusitis complicated with orbital cellulitis and subperiosteal abscess of the frontal bone. Results: Following a diagnosis of orbital cellulitis and subperiosteal abscess of the frontal bone from a unilateral pansinusitis, an initial intravenous antibiotic was given for 72 hours, followed by a surgical drainage with subsequent rapid improvement. Conclusion: Acute rhinosinusitis may be complicated by orbital cellulitis and abscess formation. Prompt referral to a tertiary health facility as well as ooperation between the Ophthalmologists and Otorhinolaryngologists is very important to prevent life threatening complications. Herein, we describe a case report of carbapenem-resistant Acinetobacter baumannii and Klebsiella pneumoniae isolates that were identified from the same patient at a Tertiary University Hospital Centre in Portugal. Antimicrobial susceptibility and the molecular characterization of resistance and virulence determinants were performed. PCR screening identified the presence of the resistance genes blaKPC-3, blaTEM-1 and blaSHV-1 in both isolates. The KPC-3 K. pneumoniae isolate belonged to the ST-14 high risk clone and accumulated an uncommon resistance and virulence profile additional to a horizontal dissemination capacity. In conclusion, the molecular screening led to the first identification of the A. baumannii KPC-3 producer in Portugal with a full antimicrobial resistance profile including tigecycline and colistin.
