Stress and Burnout Among Mental Health Staff During the COVID-19 Pandemic

Purpose: The coronavirus disease 2019 (COVID-19) pandemic has deeply altered social and working environments among health care workers. These health care workers are therefore at risk of additional psychosocial strain and ensuing metal health symptoms, which indirectly affects patient care. In this study, we aimed to assess the psychosocial and psychopathological impact of COVID-19 among acute care mental health and addictions staff. Methods: This study is a cross-sectional survey and contains a sample size of 60 mental health and addiction acute care workers recruited from within Nova Scotia Health Authority. The survey was constructed using the online survey system, Opinio, and consisted of three sections: demographic variables (gender, age group and profession); the DASS-21 Questionnaire (which provides dimensional measures of stress, anxiety and depression); and the MBI-HSS (MP) Questionnaire (which measures three dimensions of burnout—emotional exhaustion, depersonalization and personal achievement). Results: The majority of participants had at least one pathologic score on the DASS-21 and MBI-HSS (MP) sections (75.5% and 93.5%, respectively). The median severity on the DASS-21 and MBI-HSS (MP) were both moderate, with the younger age group (20-35 years) having more significant burnout scores (p = 0.0494). Simple logistic regression showed a significant relationship between burnout severity and pathologic distress, and simple linear regression showed significant correlation between DASS-21 and MBI-HSS (MP) scores, with a R2 value of 0.4633. Conclusion: More planning, programs, resources and further research are needed to support wellness and recovery of all health care professionals who work at the mental health and addictions acute care unit.

Introduction: The mental health of the health care professionals is more likely to be affected by the coronavirus disease-19 compared to the general population. Accordingly, the current study aimed to summarize the magnitudes and determinants of the psychological impact of coronavirus among health care professionals. Methods: The studies from Medline via PubMed, Science Direct, and Google Scholar were searched from 5 September to 19 October 2020. The review was conducted as per PRISMA-2009 (Preferred Reporting Items for Systematic Reviews and Meta-analyses). Result: Initially, 6316 articles were searched from three databases (PubMed, Science Direct, and Google Scholar). Finally, 20 articles were filtered to be included in our review. Among different types of psychological impact, stress was reported that lies within the range from 5.2% to 100%, anxiety was reported from 11.1% to 100%, depression was from 10.6% to 58%, and insomnia was from 28.75% to 34%. Several factors were related to the negative psychological state of health care workers due to coronavirus disease-19 like educational level, occupation, gender, age, working environment, work experience, legal status, ethnicity, psychological comorbidity, social support, personal/family exposure, and feeling of health care providers. Conclusion: The most common psychological impacts among health care workers were anxiety and stress. Besides different sociodemographic factors that determine the psychological impacts, front-line health care workers and participants having psychological comorbidity have a high risk of negative psychological state impacts as compared to their counterparts. Therefore, special attention should tend to health care workers directly involved in the prevention and management of coronavirus disease-19 and having a different risk of mental health condition.

Abstract Background Women represent the majority of the healthcare workforce and many of these women are probably pregnant and working at the front-line during COVID-19 outbreak. In COVID-19 crisis healthcare workers experienced excessive issues and challenges that made them vulnerable for getting various mental health disorders. We aimed to evaluate the mental health state among pregnant health care workers in COVID-19 era. MethodsThis cross-sectional study included 64 pregnant clinical residents, general physicians, nurses and other medical staff who worked in the hospitals in Iran, from August 2020 to December 2020 using an anonymous online survey available on Google Form platform. The online survey consisted of socio demographic questions, pregnancy-related questions, and some questions about medical/drug history, The Persian versions of General Health Questionnaire – 28 (GHQ-28), and Corona Disease Anxiety Scale (CDAS) were also fulfilled by the participants.ResultsThe mean age and gestational age of participants were 31.3±3.9 years, and 24.5±10.1 weeks. Psychological problems were moderate (total score GHQ-28> 40) in 9.4% (n:6) of the pregnant health care workers (PHCW). The frequency of moderate/severe problems in the four subclasses, including somatic symptoms, anxiety/insomnia, social dysfunction, and depression were 9(14.1%), 25(39.0%), 7(10.9%), and 10(15.6%), respectively. According to CDAS, 26.5% of participants had moderate to severe COVID-19 related anxiety. COVID-19 related anxiety and its subclasses, physical and psychological, were significantly correlated with GHQ-28 total score and its subclasses, including somatic symptoms, anxiety/insomnia, social dysfunction, and depression among pregnant health care workers (P<0.01).ConclusionAccording to the results of the current study, over 25% of the pregnant health care workers have moderate to severe COVID-19 related anxiety; so it is crucial for policymakers to focus on the mental health of the pregnant health care workers during COVID-19 pandemic. This study has major implications on health care practice. Further studies are needed to draw a clear picture of mental health problems among PHCW.

Background: The COVID-19 pandemic adversely hit India and its economy recently and is associated with the increasing uncertainty among the mental health status of health care workers. When the World Health Organization focus has mostly on testing and finding a vaccine, healthcare workers are passing with a myriad of mental health problems. The present study decided to conduct an online survey for assessing health status. Aims and Objective: To assess the health of healthcare workers engaged in SARS-CoV-2 duties. Methods: From 4th July 2020 to 4th October 2020 an online Google Form, a survey was conducted among healthcare workers. The survey collected data on socio-demographic and health variables especially during COVID-19 duties in the form of questionnaires. Results: There were a total of 196 responses from different provinces of India collected. The mean age of the respondents was around 41 years with 62.3% males and 37.7% females. The study showed that most HCWs showed a change in the sleeping habit. Conclusions: The present study concluded that there is a need for holistic interventions among health care workers and more systematic and longitudinal evaluations of mental health status further needed. AimsFrontline health care workers exposed to COVID-19 patients could be at increased risk of developing psychological issues. The study aimed to estimate the prevalence of mental health-related problems, specifically depression, anxiety, post-traumatic stress disorder (PTSD), and insomnia among health care professionals during the COVID-19 pandemic in Bangladesh and to compare these between medical and allied health care professionals.MethodThis cross-sectional survey was conducted using Google Form then subsequent telephone interview between June and August 2020. Using random sampling, a total of 479 health care professionals participated in the study. We collected data on demographics. Anxiety and depression were measured using 4 items Patient Health Questionnaire-4 (PHQ-4), PTSD was measured using 4 items Primary Care (PC)-PTSD-Screen, and insomnia was measured by using a 7-item Insomnia Severity Index (ISI). A multivariable logistic regression analysis was performed to assess risk factors associated with mental health symptoms.ResultOverall, 17.6% of frontline health workers had symptoms of anxiety, 15.5% had depression symptoms, 7.6% had PTSD symptoms and 5.9% had symptoms of insomnia. Compared to allied health professionals (n = 113, 24%), doctors (n = 366, 76%) had significantly higher prevalence of anxiety: 21.1% vs 06%, (OR = 4.19; 95% CI = 1.88–9.35; p-value <0.001); depression: 18% vs 6.8%, (OR = 2.99; 95% CI = 1.40–6.42; p-value 0.005); PTSD: 9.4% vs 1.7%, (OR = 5.96; 95% CI = 1.41–25.11; p-value 0.015) and insomnia: 7.4% vs 0.9%, (OR = 9.22; 95% CI = 1.24–68.4; p-value 0.03). Logistic regression analysis showed that pre-existing medical illness has significantly more risks of developing symptoms of anxiety (adjusted OR = 2.85; 95% CI = 1.71–4.76; p-value <0.001) and depression (OR = 2.29; 95% CI = 1.39–3.77; p-value 0.001). Having a postgraduate degree (adjusted OR = 6.13; 95% CI = 1.28–29.28; p-value 0.023) and working in secondary care setting (adjusted OR = 3.08; 95% CI = 1.18–8.02; p value 0.021) have significant predictors of developing anxiety symptoms among health workers. Those who had worked more than 6 weeks in COVID-19 dedicated hospitals had risk of developing symptoms of PSTD (OR = 2.83; 95% CI = 1.35–5.93; p value 0.006) and insomnia (OR = 2.63; 95% CI = 1.15–6.02; p value 0.022).ConclusionOur study demonstrated a high prevalence of symptoms of depression, anxiety, PTSD, and insomnia among Bangladeshi frontline health workers (particularly among doctors) during the COVID-19 pandemic. Abstract Background The coronavirus disease 2019 (COVID-19) may aggravate workplace conditions that impact health-care workers’ mental health. However, it can also place other stresses on workers outside of their work. This study determines the effect of COVID-19 on symptoms of negative and positive mental health and the workforce’s experience with various sources of support. Effect modification by demographic variables was also studied. Methods A cross-sectional survey study, conducted between 2 April and 4 May 2020 (two waves), led to a convenience sample of 4509 health-care workers in Flanders (Belgium), including paramedics (40.6%), nurses (33.4%), doctors (13.4%) and management staff (12.2%). About three in four were employed in university and acute hospitals (29.6%), primary care practices (25.7%), residential care centers (21.3%) or care sites for disabled and mental health care. In each of the two waves, participants were asked how frequently (on a scale of 0–10) they experienced positive and negative mental health symptoms during normal circumstances and during last week, referred to as before and during COVID-19, respectively. These symptoms were stress, hypervigilance, fatigue, difficulty sleeping, unable to relax, fear, irregular lifestyle, flashback, difficulty concentrating, feeling unhappy and dejected, failing to recognize their own emotional response, doubting knowledge and skills and feeling uncomfortable within the team. Associations between COVID-19 and mental health symptoms were estimated by cumulative logit models and reported as odds ratios. The needed support was our secondary outcome and was reported as the degree to which health-care workers relied on sources of support and how they experienced them. Results All symptoms were significantly more pronounced during versus before COVID-19. For hypervigilance, there was a 12-fold odds (odds ratio 12.24, 95% confidence interval 11.11–13.49) during versus before COVID-19. Positive professional symptoms such as the feeling that one can make a difference were less frequently experienced. The association between COVID-19 and mental health was generally strongest for the age group 30–49 years, females, nurses and residential care centers. Health-care workers reported to rely on support from relatives and peers. A considerable proportion, respectively, 18 and 27%, reported the need for professional guidance from psychologists and more support from their leadership. Conclusions The toll of the crisis has been heavy on health-care workers. Those who carry leadership positions at an organizational or system level should take this opportunity to develop targeted strategies to mitigate key stressors of health-care workers’ mental well-being.

Abstract Background This study aimed to compare prevalence and risk factors of somatization (SOM) between health care workers and non-health care workers during COVID-19 outbreak in China. Methods From 14 February to 29 March 2020, an online survey was performed in both 605 health care workers and 1151 non-health care workers. Based on the somatization dimension score of the Symptom Checklist-90, participants were divided into non-SOM group and SOM group. Results Health care workers had higher prevalence rate of SOM (p < 0.001) than non-health care workers, with an OR of 1.70 (95% CI, 1.22–2.36, p = 0.002). Multiple logistic regression analysis revealed that in non-health care workers, the risk factors of SOM included other ethnicities, insomnia, and suicide, while in health care workers, the risk factors included working 6–8 h per day, and working ≥10 h per day during COVID-19 outbreak. Conclusions Our research suggests that both non-health care workers and health care workers have a relatively high prevalence of somatization. However, the related factors for somatization in both groups are significantly different, showing that medical service-related factors are associated with somatization in health care workers, while demographic and clinical factors are associated with somatization in non-health care workers.

Perception of physician reimbursement for surgical procedures is not well studied. The few existing studies illustrate that patients believe compensation to be higher than in reality. These studies focus on patient perceptions and have not assessed health-care workers’ views. Our study examined health-care workers’ perception of reimbursement for complex surgical oncology procedures. An anonymous online survey was distributed to employees at our cancer center with descriptions and illustrations of three oncology procedures—hepatectomy, gastrectomy, and pancreaticoduodenectomy. Participants estimated the Medicare fee and gave their perceived value of each procedure. Participants recorded their perception of surgeon compensation overall, both before and after revealing the Medicare fee schedule. Most of the 113 participants were physicians (33.6%) and nurses (28.3%). When blinded to the Medicare fee schedules, most felt that reimbursements were too low for all procedures (60–64%) and that surgeons were overall undercompensated (57%). Value predictions for each procedure were discordant from actual Medicare fee schedules, with overestimates up to 374 per cent. After revealing the Medicare fee schedules, 55 per cent of respondents felt that surgeons were undercompensated. Even among health-care workers, a large discrepancy exists between perceived and actual reimbursement. Revealing actual reimbursements did not alter perception on overall surgeon compensation.
