Dental aspects of hearing impairment in children – aetiology, classifications, diagnostic programmes, rehabilitation. A literature review

A literature review was performed to present the epidemiology of hearing impairment both worldwide and in Poland. Pre-and postnatal risk factors were assessed. The paper presents classifications of hearing impairment considering different factors, as well as the method of neonatal hearing screening in Poland. Diagnostic, therapeutic and rehabilitation methods for patients with hearing loss were described. The paper further describes oral health in deaf and hardly hearing children in Poland and worldwide, as well as implications of hearing impairment on the masticatory organ. Several factors, such as insufficient hygiene, contribute to high caries frequency and gingivitis in children with hearing impairment. Missing or malformed teeth and malocclusions pose a great challenge for dentists. Furthermore, the work draws attention to the psychological aspect of the development of children with hearing impairment. The communication barrier during a dental visit attended by a hearing-impaired child, as well as the need for appropriate training for medical personnel, which could contribute to improved oral health, are emphasised.

<p><strong>Objective:</strong> The aim of this study was to review the literature about the relationship between gingivitis and oral health-related quality of life (OHRQoL). <strong>Material and methods:</strong> Relevant databases were searched for articles in English, which had been published from October 1990 to February 2014. Two independent examiners selected relevant papers, by initially assessing the abstracts and subsequently the full-text articles. Selected studies were grouped based on clinical and OHRQoL instruments and submitted to qualitative analyses. <strong>Results:</strong> Out of 184 references, twelve were eligible for synthesis. All studies were cross-sectional and reported data from the following five different countries: Chile (n=2), India (n=1), Brazil (n=3), Thailand (n=5) and Sudan (n=1). The number and age of subjects included ranged from 53 to 9133 and from eight to 106 years, respectively. The following six OHRQoL instruments were used: Oral Health Impact Profile (OHIP), OHIP-14, Child Perceptions Questionnaire (CPQ), Oral Impacts on Daily Performance (OIDP), Child-OIDP and Geriatric Oral Health Assessment Index (GOHAI). The different methods to evaluate the presence of gingivitis were: necrotizing ulcerative gingival lesions (n=2), Gingival Index (n=2), Community Periodontal Index (n=7) and gingival bleeding after tooth brushing (n=1). <strong>Conclusion:</strong> This literature review suggests that gingivitis is associated with impairment of OHRQoL. </p>

Objective: The authors conducted an integrative literature review of recent studies that explored the impact of interventions implemented in the U.S. that focused on improving access to dental care for low-income and vulnerable populations. Methods: The authors conducted an integrative literature review of studies published between 2012-2018 that addressed six oral health policy spheres. 1) Community-based dental access programs; (2) Medicaid reimbursement and expansion; (3) Student loan support; (4) Oral health services in non-traditional settings and dental residency programs; (5) Programs to improve oral health literacy; and (6) Use of dental therapists. Results: The authors included 39 articles for qualitative synthesis. Numerous public health initiatives and programs exist in the US aimed at increasing access to quality oral health care. Medicaid expansion, increased Medicaid fee-for-service reimbursement rates, and state loan repayment programs have demonstrated some success in improving access among underserved populations. A diversified dental workforce, with community dental health workers and mid-level providers like dental therapists, as well as interprofessional training of nurses and primary-care physicians in oral health have also shown positive impacts in advancing health equity. Further studies are needed to understand how oral health literacy programs can affect access and utilization of dental services. Conclusions: Improvements to the oral health care safety net will require a holistic and multifaceted approach in order to reduce oral health disparities. Policy levers should work, not in isolation, but rather in complementary fashion to one another.

Introduction: For the assessment of child development in the deaf is effective protocols are needed for this population, as in Brazil, standardized tests for these children are still scarce 4.5. Thus, emphasis is placed on studying child development in deaf so that therapeutic and educational approaches are contemplated in accordance with the needs of each child. Objective:  Analyze the performance of children with hearing defi cit in different areas: Communications-Issue, Communication-reception, aspects Motors and Cognitive Aspects of Language. Methods: We have carried out a comparative study in an institutional clinic with 109 children, divided into 60 hearing individuals with typical development and 49 with hearing loss, severe to profound. The performance index was calculated for each child in these four domains. The index was analysed according to the equation: (number of responses in each area / number of assessed behaviours) x100. Results: In all age groups, hearing-impaired children had a smaller performance when compared to hearing children in the Communication – Emission domain. The Communication-Reception domain showed signifi cant differences (p < 0.05) from the 12-to-23-months to the 60-to-71-months age groups. The Cognitive Aspects domain demonstrated a signifi cant difference (p < 0.01) between hearing-impaired and hearing children aged 24 to 35 months and onwards. Motor Aspects only had a signifi cant difference (p < 0.01) in the last two age groups, 48-59 months and 60-71 months. The signifi cance level was 5%. Conclusion:  In all age groups, hearing-impaired children’s performance was worse when compared to hearing children. The intervention made by health professionals ought to take place as soon as possible in order to develop the cognitive, motor and language skills of the hearing-impaired child.
