The results of congenital malformations monitoring in 23 regions of the Russian Federation for the period from 2011 to 2018 are presented. There is an increasing tendency of the overall frequency of congenital malformations from 84.18 per 10000 births in 2011 to 102.18 in 2018. Estimates of the baseline of selected defects are obtained, which should be used as the expected frequency during planning the volume of medical care for newborns with congenital malformations.

Objective. To analyze legislation regulating medical care for children with rheumatic diseases in the Russian Federation. Materials and methods. We analyzed regulatory documents of the Ministry of Health of Russia, including the rules of medical care for children with rheumatic diseases, the resolution regulating the distribution of medicines at a discount or for free, and regional legal acts regulating routing of children with rheumatic diseases. Results. The legislation regulating medical care for children with rheumatic diseases is imperfect and needs to be updated. We identified shortcomings in the system regulating preferential provision of certain groups of patients with medicines. The results of our analysis can be used as a rationale for changes and amendments to existing regulatory legal acts and for the development of new regulatory documents. Conclusion. This analysis of legal documents allows us to suggest amendments to the existing legal framework in order to reduce tensions (including legal tensions) associated with distribution of preferential medicines, reconsider establishment of disability in patients with juvenile idiopathic arthritis, expand opportunities to prescribe biologicals for non-disabled patients with systemic scleroderma, juvenile dermatomyositis, and systemic vasculitis in regions. We justified detailed description of regional legal documents regulating distribution of preferential medicines among patients with rheumatic diseases. Key words: rheumatic diseases in children, regulatory framework, health care organization, drug provision

According to the State program of the Russian Federation «Health care development» till 2020 approved by the decree of the Russian Federation government dated 24.12.2012 N 2511-p, the principle of the cluster approach became the one of the directions of informatization of healthcare. Subsystem perinatal cluster is the continuous monitoring of pregnant women. Its main advantage is the formation of a unified information space of the hospitals in the region that allows you to reliably group out pregnant females according to the risk of complications of pregnancy and childbirth, to determine the routing for all the patients. The goal of the system is a significant improvement of the assistance within a cluster (group), the formation of a standardized approach to the medical care, management of emergency situations in the occurrence of life-threatening conditions, the reduction of maternal and perinatal mortality rate. Computerization of medical care in obstetrics was implemented in the automated system «Program monitoring for pregnant females».

Organizational issues of the interdepartmental application of telemedicine technologies in the provision of medical care to patients are considered. The most significant provisions of the concept of interdepartmental application of telemedicine technologies in the organization and provision of medical care to patients in critical conditions are presented. In addition, data on telemedicine consultations on federal districts of the Russian Federation performed by the federal level of health care in 2017 are shown, the frequency of emergency telemedicine consultations is revealed depending on the disease profile of patients or the specialty of a consulting physician. Prospects for the further introduction of telemedicine technologies into daily practical activities and the possibility of their adaptation to crisis situations and military conflicts are substantiated. It has been established that in various natural disasters, accidents, catastrophes, terrorist acts and during military conflicts with a large number of victims, successful organization of medical care and medical evacuation is possible only when combined with the efforts of the Ministry of Health, the Ministry of Defense, the Ministry of internal affairs and the Ministry of the Russian Federation for Civil Defence, Emergencies and Elimination of Consequences of Natural Disasters. Thus, the adequate development of telemedicine systems will increase the level of their rational and effective interdepartmental use for the benefit of saving people as Russia’s human capital.

It is shown that one of the priority areas of the social policy of the Russian Federation is the consideration of improving the drug supply, the rationale for the national drug policy, as well as the introduction of the drug reimbursement (insurance) system, as one of the key mechanisms to increase the affordability of drugs by partially or fully reimbursing their cost citizens when providing medical care on an outpatient basis. It was revealed that in the context of reforming the healthcare system, the specifics of the work of power ministries and departments are not sufficiently taken into account. In this regard, the features have been studied and an assessment has been given of the prospects for introducing drug insurance into military health care as part of project activities, as one of the mechanisms for implementing interagency cooperation in modern socio- economic conditions when reforming drug supply. It is reflected that the introduction of drug insurance mechanisms can increase the availability of drugs, have a positive effect on the health of the assigned contingents, and reduce the frequency and duration of hospitalizations. At the same time, the following were identified as the main prospects and systemic prerequisites for implementation: the need to create a reliable legal basis for the formation and subsequent development of a unified state system of drug supply, taking into account the specifics of military health; transition to a rational system of financing drug supply for the provision of medical care on an outpatient and inpatient basis as part of a project to develop a new model for the provision of primary health care and specialized medical care based on a unified medical and technological hospital base; prospective reduction of expenses for the supply of medical care in stationary conditions; optimal distribution of rights and responsibilities, ensuring coherence between health authorities; prevention of duplication, ensuring equal and guaranteed drug supply for citizens as part of the project to ensure the quality and accessibility of medical care to privileged contingents of the Ministry of Defense of the Russian Federation. A possible model of drug supply for contingents attached to military medical organizations is substantiated when providing medical care on an outpatient basis when introducing a drug insurance system at the state level. The following key elements of a grounded model are described: adjustment of drug supply management; clarification of the categories of assigned contingents; optimization of the order of dispensing of drugs; the formation of restrictive lists of drugs; optimization of financing.

Relevance. The problem of organizing specialized medical care in emergency forms for children is caused by three groups of reasons: the presence at the junction of several adult and pediatric medical specialties, the medical and geographical features of the country and the lack of a unified regulatory framework.Intention. To increase the availability of medical services and reduce regional inequality in their distribution. A model of organizing a single operational service of specialized medical care for children and the basic principles of its functioning on the territory of the Russian Federation are proposed.Methodology. Statistical data, features of legislation, ways of solving the problem in certain regions and the achievements of initiative groups of specialists are analyzed. Results and Discussion. Since 2000, the number of hospital beds for children, the number of emergency medical stations has decreased, and the number of diseases and conditions requiring specialized emergency care for children has increased. Infant mortality rates in remote and hard-to-reach regions exceed the national average by 1.5–2.6 times. At the same time, the current statistics of medical evacuations of children are not accurately reflected in any of the official sources. The regulatory framework for medical evacuation of children includes three “ Procedures for providing medical care “in the profile “ambulance”, “neonatology” and “obstetrics and gynecology” and Federal Law N 323. The law implies the presence of mobile teams in hospitals and departments of different profiles that are not related to each other, but there is no structure that unites them and analyzes the results of work at the federal level.Conclusion. To solve the problem, it is necessary to create a federal operational service and a “hotline” that can be contacted by any resident of the country, to form a federal medical register of critical conditions in children, to develop and implement common principles of diagnosis and treatment. The presented model and principles contribute to the development of a three-level system of emergency medical care and correspond to the goals and objectives of the Ministry of Health of the Russian Federation set out in Letter N 14-3/n/2-2339 of 20.03.2019.
