Hyperglycemia is a major cause of complications in patients with diabetes mellitus (DM). Since the optimal level of glycemic control is achieved only in a small number of patients with diabetes mellitus, additional methods of prevention and treatment of complications are strongly recommended. The focus of healthcare specialists is increasing on substances that can affect intracellular pathological processes associated with hyperglycemia. Such drugs or agents include certain vitamins and minerals.

The most common form of peripheral nervous system damage in diabetes mellitus is distal symmetric sensorimotor polyneuropathy (DSSMPN). Chronic hyperglycemia, dyslipidemia, and impaired microcirculation are considered to be the key mechanisms for the development of DSSMPN, but its pathogenesis is still unclear and continues to be studied. The paper analyzes the issues of diagnosis of DSSMPN and the effective principles of patient treatment. It also discusses the use of alpha-lipoic acid (ALA) as a drug for the pathogenetic treatment of DSSMPN and describes the results of clinical trials of its treatment with ALA preparations.

Obesity type 2 diabetes mellitus is a common metabolic disease in clinical practice, and its prevalence is increasing rapidly with the aging of the population and changes in lifestyle. Acupuncture, as a distinctive therapy, has its unique advantages in the treatment of obesity type 2 diabetes and has an irreplaceable role in a variety of treatment methods. The author organized the literature on acupuncture and its related therapies to prevent and treat obesity type 2 diabetes in recent years and found that acupuncture and its associated therapies to prevent and treat obesity type 2 diabetes mainly include: simple acupuncture, electroacupuncture, acupoint catgut embedding therapy, auricular-plaster therapy and other treatments, all of which can safely and effectively improve clinical symptoms, acupuncture and its related therapies to treat obesity type 2 diabetes has a broad prospect, worthy of further clinical promotion.

SUMMARY Although long ago described, there is no established consensus regarding the real existence of Diabetic Cardiomyopathy (CMPDM). Due to its complex pathophysiology, it has been difficult for clinical and experimental research to establish clear connections between diabetes mellitus (DM) and heart failure (HF), as well as to solve the mechanisms of the underlying myocardial disease. However, the epidemiological evidence of the relationship of these conditions is undisputed. The interest in understanding this disease has intensified due to the recent results of clinical trials evaluating new glucose-lowering drugs, such as sodium-glucose transporter inhibitors 2, which demonstrated favorable responses considering the prevention and treatment of HF in patients with DM. In this review we cover aspects of the epidemiology of CMPDM and its possible pathogenic mechanisms, as well as, present the main cardiac phenotypes of CMPDM (HF with preserved and reduced ejection fraction) and implications of the therapeutic management of this disease.
