Objective. To study the incidence and to determine the main trends of treatment of the erysipelas surgical forms. Materials and methods. There were retrospectively and prospectively analyzed 114 histories of the patients’ cases, who were treated in stationary in the centre of purulent-septic surgery of the Municipal Hospital No 3 of Zaporizhzhya in 2019 - 2020 yrs. Results. Mostly often the affection site was localized on the lower extremities. In bullous and phlegmonous forms of erysipelas the most frequent clinical signs of intoxication were general weakness, hyperthermia, muscular pain. In necrotic form of erysipelas the signs of severe intoxication with nausea, regurgitation and confusion were observed. In complex treatment of erysipelas the main principles are antibioticotherapy and early surgical processing of the pathological process zone. Determination of indices of the procalcitonin content in the blood serum gives possibility to estimate progression and generalization of the process and constitutes a sensitive test for efficacy of the treatment conducted. Conclusion. Radical surgical intervention and timely targeted antibioticotherapy constitutes basic elements of successful treatment for surgical forms of erysipelas, and this position must not be revised. Procalcitonin content in the blood serum constitutes a diagnostic marker, permitting to prognosticate the sepsis development timely.

The goal of the article is to find out the frequency of complicated forms of erysipelas among the general population of the disease and to determine the main directions for their surgical treatment. Materials and methods. 114 case histories of patients who were hospitalized in the center of purulent-septic surgery of the Non-profit municipal enterprise «City Hospital № 3» in Zaporozhye for the period 2019–2020 were analyzed retrospectively and prospectively. Results. It was found that most often the focus was localized on the upper and lower extremities. In the complex treatment of patients with surgical forms of erysipelas, the main place belongs to antibiotic therapy (ABT) and early surgical treatment of the area of the pathological process. Determination of the level of procalcitonin in blood serum makes it possible to assess the progression of the inflammatory process and is a sensitive test for the effectiveness of treatment. Conclusions. The incidence of complicated forms of erysipelas requiring surgery in the general population was 78,9%. Patients with circular erysipelas need the most extensive surgery, when it is necessary to perform an autopsy within the area of inflammation with longitudinal and transverse wavy autopsies throughout the pathological focus. Determination of the level of procalcitonin in blood serum makes it possible to assess the progress of the process and is a sensitive test for the effectiveness of treatment. In the study of blood serum in 118 patients with different forms of otitis media, higher titers of anti-brain antibodies were found than in healthy ones. There was a correlation between the titer value and the intensity of the pathological process in the middle ear. A prognostically unfavorable sign in the occurrence of otogenic intracranial complications is the disappearance or a sharp drop in the titer of anti-brain antibodies during an exacerbation of chronic suppurative otitis media.

The ideal marker of bacterial infection should not only allow for early diagnosis, but also provide information about the course and prognosis of the disease. Nowadays the most well-studied and widely used in clinical practice is procalcitonin (PCT), but its value in surgical forms of erysipelas is insufficiently defined. A polarographic study of the hydrolysis of [8-lysine]vasopressin and some hormonogens of the vasopressin series with the blood serum of women in the last week of pregnancy was studied. The dependence of hydrolysis on pH (pH optimum: 7.4-7.50, substrate concentration (Km 1.2 . 10-5M), pH stability and thermal stability were determined. The rate of hydrolysis of individual vasopressin analogues decreases in the order: [8-lysine]vasopressin > Nα-glycyl-prolyl[8-lysine]-vasopressin > Nα-leucyl-[8-lysine]vasopressin > Nα-alanyl-[8-lysine]vasopressin > Nα-phenyl alanyl-[8-lysine]vasopressin > Nα-diglycyl-[8-lysine]vasopressin > Nα-prolyl-[8-lysine]vasopressin > Nα-triglycyl-[8-lysine]vasopressin > Nα-sarcosyl-glycyl-[8-lysine]vasopressin. The degree of hydrolysis gradually increases to a multiple with the length of the pregnancy in consequence of the presence of oxytocine. However, vasopressin is also hydrolysed to a small extent with the enzymes from the blood sera of non-pregnant women. Under similar analytical conditions oxytocin was not hydrolysed with the sera of non-pregnant women and therefore oxytocin is a more suitable substrate than vasopressin for polarographic determination of serum oxytocinase.

Abstract Background Naso-ethmoidal meningoencephalocele is usually a congenital anomaly consisting of a protrusion of cerebral tissue and meninges into the ethmoidal labyrinth. The condition is a rare cause of structural epilepsy in dogs. We report the clinical presentation, surgical intervention, postoperative complications and outcome in a dog with drug resistant epilepsy secondary to a meningoencephalocele. Case presentation A 3.3-year-old male neutered Tamaskan Dog was referred for assessment of epileptic seizures secondary to a previously diagnosed left-sided naso-ethmoidal meningoencephalocele. The dog was drug resistant to medical management with phenobarbital, potassium bromide and levetiracetam. Surgical intervention was performed by a transfrontal craniotomy with resection of the meningoencephalocele and closure of the dural defect. Twenty-four hours after surgery the dog demonstrated progressive cervical hyperaesthesia caused by tension pneumocephalus and pneumorrhachis. Replacement of the fascial graft resulted in immediate resolution of the dog’s neurological signs. Within 5 months after surgery the dog progressively developed sneezing and haemorrhagic nasal discharge, caused by sinonasal aspergillosis. Systemic medical management with oral itraconazole (7 mg/kg orally q12h) was well-tolerated and resulted in resolution of the clinical signs. The itraconazole was tapered with no relapsing upper airway signs. The dog’s frequency of epileptic seizures was not affected by surgical resection of the meningoencephalocele. No treatment adjustments of the anti-epileptic medication have been necessary during the follow-up period of 15 months. Conclusions Surgical resection of the meningoencephalocele did not affect the seizure frequency of the dog. Further research on prognostic factors associated with surgical treatment of meningoencephaloceles in dogs is necessary. Careful monitoring for postsurgical complications allows prompt initiation of appropriate treatment.
