The ideal marker of bacterial infection should not only allow for early diagnosis, but also provide information about the course and prognosis of the disease. Nowadays the most well-studied and widely used in clinical practice is procalcitonin (PCT), but its value in surgical forms of erysipelas is insufficiently defined. The aim of the article is to determine the diagnostic and prognostic significance of procalcitonin in patients with surgical forms of erysipelas.114 case histories of patients who were hospitalized in the center of purulent-septic surgery of the Non-profit municipal enterprise "City Hospital №3" in Zaporozhye for 2019-2020 period were analyzed retrospectively and prospectively. It was found that most often the focus was localized on the lower extremities. In the complex treatment of patients with erysipelas, the main place belongs to antibiotic therapy (ABT) and early surgical treatment of the area of the pathological process. Determination of the level of procalcitonin in the serum makes it possible to assess the progression and generalization of the process and is a sensitive test for the effectiveness of treatment.Radical surgery and timely targeted ABT are key elements of success in the treatment of surgical forms of erysipelas and are not subject to revision. Procalcitonin is a diagnostic marker that can predict the development of sepsis.

Materials and methods. 114 case histories of patients who were hospitalized in the center of purulent-septic surgery of the Non-profit municipal enterprise «City Hospital № 3» in Zaporozhye for the period 2019–2020 were analyzed retrospectively and prospectively. Results. It was found that most often the focus was localized on the upper and lower extremities. In the complex treatment of patients with surgical forms of erysipelas, the main place belongs to antibiotic therapy (ABT) and early surgical treatment of the area of the pathological process. Determination of the level of procalcitonin in blood serum makes it possible to assess the progression of the inflammatory process and is a sensitive test for the effectiveness of treatment. Conclusions. The incidence of complicated forms of erysipelas requiring surgery in the general population was 78,9%. Patients with circular erysipelas need the most extensive surgery, when it is necessary to perform an autopsy within the area of inflammation with longitudinal and transverse wavy autopsies throughout the pathological focus. Determination of the level of procalcitonin in blood serum makes it possible to assess the progress of the process and is a sensitive test for the effectiveness of treatment. Aim. The aim of the study was a clinical assessment of the integrated determination of predictors of life-threatening ventricular arrhythmias (LVA) in patients with ventricular extrasystole (VE) without structural changes in the heart to predict the development of cardiovascular disease in a prospective study. Material and methods. From 1997 to 2014 199 patients with neurocirculatory dystonia (NCD), complicated with VE II-V class EE at the age of 18-35 years (mean 27.3-2.6 years) were observed in District Military Clinical Hospital of the Ministry of Defense of the Russian Federation, City Hospital named after St. Martyr Elizabeth. All patients, in addition to a general clinical examination, determined late potentials of the ventricles (LPV), QT interval variability (QTd), heart rate turbulence (HRT), and also indicators reflecting the possible mechanism of VE development. Results. The results of the study showed that in patients with NCD complicated by VE, the detection of indicators reflecting the presence of VE caused by the mechanisms of early post-depolarization and re-entry is a predictor of the development of coronary heart disease (CHD) and hypertension (HB). The positive prognostic significance of the indicators showing the presence of VE with these development mechanisms for the development of CHD and HB averaged 74% and 44% respectively, and with the appointment of сlass III drugs for elimination of VE and (or) development of unstable ventricular tachycardia, the risk of developing CHD increased to an average of 93%. Identification of NDC patients with indicators reflecting the presence of VE due to delayed post-depolarization is a predictor of the development of gastrointestinal tract (GT) diseases with positive prognostic significance, averaging 61%. Positive prognostic significance with respect to the development of CHD, HB, GT diseases for the detected LPV, QTd > 80 ms, pathological values of HRT VE did not exceed 30%. Indicators and predictors, which characterize the mechanism of development of VE and the risk of LVA occurrence in patients with NCD complicated by VE, were highly correlated (r > 0.50) with risk factors for cardiovascular and gastrointestinal system diseases.

Summary. A retrospective analysis of case histories and radiographs of 138 patients with hip-lumbar syndrome who underwent hip joint replacement in the clinic of orthopedics and adult traumatology at the ITO NAMS of Ukraine was carried out. It has been determined that the variant of vertical posture, acting on the morphogenesis of coxarthrosis, determines the rate of its progression. It was determined that in the presence of hip-lumbar syndrome, a moderate form of CA progression most often occurs, regardless of the type of lordosis. The slow rate of progression of coxarthrosis is characteristic of hyper- and normolordosis. Hypolordosis is statistically significant (p < 0,05) characterized by a faster course of the pathological process in the hip joint than other types of lordosis. The studies will contribute to the further development of the clinical diagnostic and therapeutic algorithm for patients with hip-lumbar syndrome.

Objective. To study the incidence and to determine the main trends of treatment of the erysipelas surgical forms. Materials and methods. There were retrospectively and prospectively analyzed 114 histories of the patients’ cases, who were treated in stationary in the centre of purulent-septic surgery of the Municipal Hospital No 3 of Zaporizhzhya in 2019 - 2020 yrs. Results. Mostly often the affection site was localized on the lower extremities. In bullous and phlegmonous forms of erysipelas the most frequent clinical signs of intoxication were general weakness, hyperthermia, muscular pain. In necrotic form of erysipelas the signs of severe intoxication with nausea, regurgitation and confusion were observed. In complex treatment of erysipelas the main principles are antibioticotherapy and early surgical processing of the pathological process zone. Determination of indices of the procalcitonin content in the blood serum gives possibility to estimate progression and generalization of the process and constitutes a sensitive test for efficacy of the treatment conducted. Conclusion. Radical surgical intervention and timely targeted antibioticotherapy constitutes basic elements of successful treatment for surgical forms of erysipelas, and this position must not be revised. Procalcitonin content in the blood serum constitutes a diagnostic marker, permitting to prognosticate the sepsis development timely.

Abstract Freshwater crustaceans are distributed throughout the montane and lowland areas of Colombia, and are therefore a useful indicator group for how aquatic species will respond to climate change. As such, metabolic determination of physiological performance was evaluated for the Colombian pseudothelphusid crab, Neostrengeria macropa (H. Milne Edwards, 1853), over a temperature range inclusive of current temperatures and those predicted by future scenarios in the plateau around the city of Bogotá, namely from 8 °C to 30 °C. The performance results mostly aligned with previous exploratory behavioral determination of the ideal temperature range in the same species, although the metabolism increased at the highest temperature treatments, a point when exploratory behavior declined. These results indicate that this species of montane crab behaviorally compensates for increased thermal stress by decreasing its physical activity, which could have negative predator-prey consequences with changes to community structure as different species undergo climate-mediated geographic range shifts in the region. As this species is endemic to the plateau surrounding Bogotá, it also experiences a number of other stressors to its survival, including infrastructure development and invasive species.

Density functional theory is used to determine the possible crystal structure of the CaSiO3 perovskites and their evolution under pressure. The ideal cubic perovskite is considered as a starting point for studying several possible lower-symmetry distorted structures. The theoretical lattice parameters and the atomic coordinates for all the structures are determined, and the results are discussed with respect to experimental data.

THE PURPOSE OF THE STUDY is to carry out an analysis of the literature evaluating diabetic encephalopathy by determining neuromarkers. MATERIAL AND METHODS. In this article, the authors analyzed the literature on the role of neuromarkers in patients with type 2 diabetes mellitus undergoing program hemodialysis. RESEARCH RESULTS. Among biochemical markers, the determination of the level of neurospecific proteins is actively being investigated. The main part of them is autoantigens, entering the bloodstream, can cause the appearance of autoantibodies, which, when the blood-brain barrier is impaired, enter the brain from the blood vessel and cause morphological changes, destructive processes in neurons, as well as the development of nonspecific acute-phase reactions like edema or inflammation. Biomarker studies for the diagnosis of various brain lesions have been under way for more than 20 years, but at present no ideal biomarker has been found. Among biochemical markers, the determination of the level of neurospecific proteins is being actively studied. In patients with type 2 diabetes mellitus undergoing hemodialysis, this issue is also relevant in view of the frequent vascular cerebrovascular complications, but few studies have been conducted. CONCLUSIONS. All of the above emphasizes the need to identify the features of clinical and functional changes in the nervous system in patients with type 2 diabetes mellitus receiving program hemodialysis and to evaluate the prognostic value of neuromarkers in early detection of the degree of brain damage.
