Kesehatan masyarakat merupakan suatu usaha untuk mencegah penyakit, memperpanjanghidup, dan meningkatkan kesehatan melalui usaha-usaha pengorganisasisan masyarakat(Winslow, 1920).Pengalaman belajar lapangan (PBL) adalah salah satu bentuk pengalaman belajar mengajar dimasyarakat yang merupakan siklus pemecahan masalah dengan materi substansinya adalah“Community Health Diagnosis” dan Pengembangan Proyek Intervensi. Dengan adanyakegiatan PBL diharapkan dapat meningkatkan pemahaman dan keterampilan mahasiswatentang Ilmu Kesehatan Masyarakat dan aplikasinya di tengah-tengah masyarakat.

Introduction: There are different hidden health related problems in community. These needs to be identify and adhered by effective, efficient tools to bring the better outcomes. One of the effective tools for the identification and quantification of the health problems in a community is Community Health Diagnosis. The aim of this study was to find out the common health problems in community, describe the related demographic, education, socio –economic and environmental factors and to convey health message to improve health profile of community. Methods: Four weeks Community Health Diagnosis was conducted in four selected VDCs after feasibility studies in 16 VDCs. Sample size households (HH) were calculated based on morbidity status of each VDCs. Students covered allocated number of Households in each VDCs by selecting household with convenience sampling method. Results: In all studied VDCs, nuclear family was the commonest, CBR and literacy rate were good compared to national figure. Population pyramid showed that active age group was more in these VDCs. There was lack of government tap for drinking water and latrine. Maternal parameters were found to be better than national fi gure. Conclusions: Though there is more illiteracy rate in all studied VDCs compare to national figure, maternal parameters, child health indicators and fertility rates are better which may be the impact of effective health education.  Medical Journal of Shree Birendra Hospital; January-June 2013/vol.12/Issue1/32-36DOI: http://dx.doi.org/10.3126/mjsbh.v12i1.9090

Community health diagnosis is a comprehensive assessment of health status of the community in relation to its social, physical and biological environment. The purpose of community health diagnosis is to define existing problems, determine available resources and set priorities for planning, implementing and evaluating health action, by and for the community. The community health diagnosis program began on 4th September 2015 and continued till 13th September 2015 in ward no 1 and 5 Rupakot VDC, Kaski, Nepal. The program was organized in following phases: data collection, data analysis, first community presentation, prioritization of need and planning of micro health project (MHP), implementation and evaluation of MHP, and final community presentation. On the basis of the observed and the felt needs of the community, we found the real needs and prioritized them as follows. For community: Proper water purification, information about common diseases, KAP on diseases, knowledge on TB and DOTS. For school-going children: Education on environmental sanitation, education on personal hygiene - teeth brushing and hand washing, adolescent health education. Purpose: to identify features of diagnosing the health of local communities, to examine the current state and changes in the approaches to identifying gaps and needs for action planning. Materials and Methods. The study makes use of the current data on approaches to identifying gaps and needs health diagnostics of local communities in Ukraine and the world. Results. There are five types of models for health diagnosis and needs assessment: epidemiological diagnosis, public health diagnosis, social diagnosis, asset diagnosis, and rapid diagnosis. Each model has its own vision, as well as advantages and disadvantages. In practice, the selected model can be supplemented with elements of other models in accordance with the resources and purpose of the assessment. Determining the population to be assessed is an important early stage in assessing community health. It can be determined geographically, by a specific area, place of work, residence, or study. The state health department can target the entire population, while a small local non-profit agency is likely to focus only on potential customers. The use of very specific parameters to determine the population makes the assessment more focused and detailed, allows very specific adaptation of health measures. Conclusions. When assessing community health, the boundaries of the target audience may change during data collection and analysis. Analysis and interpretation of epidemiological data may reveal that only working mothers are at high risk for health problems that the organization can address. This refinement of the target audience can occur because of a community health assessment.

Community health diagnosis is a comprehensive assessment of health status of the community in relation to its social, physical and biological environment. The purpose of community health diagnosis is to define existing problems, determine available resources and set priorities for planning, implementing and evaluation health action, by and for the community. A cross sectional descriptive study was done at Dhampus VDC from 1 Sept 2016 AD (16th Bhadra, 2073 BS) to 16 Sept 2016 AD (31st Bhadra, 2073). The data was collected by questionnaires, anthropometric measurements, interviews and secondary data. The community health diagnosis field visit gave us the opportunity to build our team spirit and taught us the arts of problem solving. It taught us the importance of correct conduct and building a good rapport with the people who were unfamiliar to us. J-GMC-N | Volume 11 | Issue 01 | January-June 2018, Page: 71-76
