Implementation Strategies to Improve Access to and Use of MAT for Defendants in Opioid Courts

In 2013, the National Institute of Mental Health funded five trials of unique, multicomponent, systems-based innovations designed to improve access to early screening, diagnosis, and treatment of autism spectrum disorder—collectively known as the Autism Spectrum Disorder Pediatric, Early Detection, Engagement, and Services Network. As part of an ongoing effort to pool data and learn from shared experience, we collected information across all studies about innovation components and implementation strategies. First, each study group completed standardized checklists based on the Template for Intervention Description and Replication and the Expert Recommendation for Implementing Change. Then, we interviewed principal and co-investigators of each study (n = 9) to further explore innovation components and assess barriers and facilitators to implementation. Innovation strategies were diverse (five different autism spectrum disorder screeners were used, 40% included early intervention trainings, 60% involved new technology). Common implementation strategies included developing stakeholder relationships and provider trainings. Barriers included inefficient systems of care, difficulty engaging families in the innovations, provider attitudes, and organizational culture (e.g. difficulty changing clinic processes). These findings suggest that—despite diverse settings and a variety of innovation content—common facilitators and challenges exist in implementing innovations to enhance access to early autism spectrum disorder screening, diagnosis, and treatment.

Background The best approach to provide comprehensive care for individuals with co-occurring disorders (CODs) related to substance use and mental health is to address both disorders through an integrated treatment approach. However, only 25% of behavioral health agencies offer integrated care and <7% of individuals who need integrated treatment receive it. A project used a cluster-randomized waitlist control group design to evaluate the effectiveness of network for the improvement of addiction treatment (NIATx) implementation strategies to improve access to addiction and psychotropic medications. Methods This study represents a secondary analysis of data from the NIATx project. Forty-nine agencies were randomized to Cohort1 (active implementation group, receiving the NIATx strategy [ n = 25]) or Cohort2 (waitlist control group [ n = 24]). Data were collected at three time points (Baseline, Year1, and Year2). A two-level (patient within an agency) multinomial logistic regression model investigated the effects of implementation strategy condition on one of four medication outcomes: both medication types, only psychotropic medication, only addiction medication, or neither medication type. A per-protocol analysis included time, NIATx fidelity, and agency focus as predictors. Results The intent-to-treat analysis found a statistically significant change in access to addiction versus neither medication, but Cohort1 compared to Cohort2 at Year 1 showed no differences. Changes were associated with the experimental intervention and occurred in the transition from Year1 to Year2, where greater increases were seen for agencies in Cohort2 versus Cohort1. The per-protocol analysis showed increased access to both medications and addiction medications from pre- to post-intervention for agencies in both cohorts; however, differences in change between high- and low-implementation agencies were not significant. Conclusions Access to integrated services for people with CODs is a long-standing problem. NIATx implementation strategies had limited effectiveness in improving medication access for individuals with CODs. Implementation strategy adherence is associated with increased medication access. Plain language summary Individuals with co-occurring substance use and psychiatric disorders face significant challenges in receiving care. With only 25% of behavioral health organizations offering integrated substance use and mental health services, it is not surprising that only 7% of individuals with a co-occurring disorder (COD) receive integrated care. The use of implementation strategies including feedback reports, external coaching, and in-person learning can provide staff with the necessary skills to implement changes in their organization. In this study, staff in addiction treatment agencies provided comprehensive assessments and treatment plans that targeted patients’ CODs, using proven implementation strategies. Results found that the application of these strategies increased patient access to addiction medications and their access to both addiction and psychotropic medications for their CODs. Higher implementation strategy adherence appears to be associated with improved access to addiction medications or both addiction and psychotropic medications but not psychotropic medications exclusively. Implementation researchers and addiction treatment agencies have evidence that implementation strategies can be utilized to improve access to medications as part of integrated treatment for individuals with CODs. However, additional research is needed to understand how adherence and fidelity to the implementation strategies are associated with more significant improvements. In addition, the results from this study raise questions as to whether efforts to improve access to psychotropic medications or addiction and psychotropic medications take longer to implement in addiction treatment agencies that do not typically provide integrated treatment for individuals with CODs.

Purpose The purpose of this article was to propose the use of a translational model as a tool for identifying limitations of current interprofessional education (IPE) research. Translational models allow researchers to clearly define next-step research needed to translate IPE to interprofessional practice (IPP). Method Key principles, goals, and limitations of current IPE research are reviewed. A popular IPE evaluation model is examined through the lens of implementation research. The authors propose a new translational model that more clearly illustrates translational gaps that can be used to direct future research. Next steps for translating IPE to IPP are discussed. Conclusion Comprehensive reviews of the literature show that the implementation strategies adopted to date have fostered improved buy-in from key stakeholders, as evidenced by improved attitudes and perceptions toward interprofessional collaboration/practice. However, there is little evidence regarding successful implementation outcomes, such as changed clinician behaviors, changed organizational practices, or improved patient outcomes. The authors propose the use of an IPE to IPP translational model to facilitate clear identification of research gaps and to better identify future research targets.

Important initiatives in health care include (a) improving access to services for disadvantaged populations, (b) providing equal access for individuals with limited or non-English proficiency, and (c) ensuring cultural competence of health-care providers to facilitate effective services for individuals from diverse racial and ethnic backgrounds (U.S. Department of Health and Human Services, Office of Minority Health, 2001). This article provides a brief overview of the use of technology by speech-language pathologists and audiologists to extend their services to underserved populations who live in remote geographic areas, or when cultural and linguistic differences impact service delivery.

Abstract. Background: The German Guideline on Screening, Diagnosis and Treatment of Alcohol Use Disorders aims to increase the uptake of evidence-based interventions for the early identification, diagnosis, prevention and treatment of alcohol-related disorders in relevant healthcare settings. To date, dissemination has not been accompanied by a guideline implementation strategy. The aim of this study is to develop tailored guideline implementation strategies and to field-test these in relevant medical and psycho-social settings in the city of Bremen, Germany. Methods: The study will conduct an impact and needs assessment of healthcare provision for alcohol use orders in Bremen, drawing on a range of secondary and primary data to: evaluate existing healthcare services; model the potential impact of improved care on public health outcomes; and identify potential barriers and facilitators to implementing evidence-based guidelines. Community advisory boards will be established for the selection of single-component or multi-faceted guideline implementation strategies. The tailoring approach considers guideline, provider and organizational factors shaping implementation. In field tests quality outcome indicators of the delivery of evidence-based interventions will be evaluated accompanied by a process evaluation to examine patient, provider and organizational factors. Outlook: This project will support the translation of guideline recommendations for the identification, prevention and treatment of AUD in routine practice and therefore contributes to the reduction of alcohol-related burden in Germany. The project is running since October 2017 and will provide its main outcomes by end of 2020. Project results will be published in scientific journals and presented at national and international conferences.

The maldistribution of and lack of access to health professionals continues to be a major issue for policymakers at all levels of government. Additionally, the basis by which Health Human Resource (HHR) policy is determined is unclear. Publications found in independent reports, peer-reviewed journals and most importantly, grey literature, can significantly influence or inform major policy decisions for “hot button” HHR issues (1) . We propose a framework that can be used to classify, rank and evaluate HHR policy/planning documents. Our framework creates six major criteria that are used to evaluate policy documents. These criteria are: 1) literature review, 2) source of primary information, 3) nature of recommendations, 4) implementation strategies, 5) credibility of authors and 6) credibility of publisher. Within each category, a score from zero to three (for criteria 1-4) or zero to two (criteria 5-6) is assigned, depending on the caliber of the document. Summing the scores from each section yields a document’s overall score. The intent of this measure is two-fold. Firstly, we want to create a tool that can be widely utilized by policymakers to help inform their decisions. Secondly, it can be used as a springboard to stimulate discussion and debate around HHR planning and policy formulation.
