BackgroundLittle is known about factors affecting implementation of patient safety programmes in low and middle-income countries. The goal of our study was to evaluate the implementation of a patient safety programme for paediatric care in Guatemala.MethodsWe used a mixed methods design to examine the implementation of a patient safety programme across 11 paediatric units at the Roosevelt Hospital in Guatemala. The safety programme included: (1) tools to measure and foster safety culture, (2) education of patient safety, (3) local leadership engagement, (4) safety event reporting systems, and (5) quality improvement interventions. Key informant staff (n=82) participated in qualitative interviews and quantitative surveys to identify implementation challenges early during programme deployment from May to July 2018, with follow-up focus group discussions in two units 1 year later to identify opportunities for programme modification. Data were analysed using thematic analysis, and integrated using triangulation, complementarity and expansion to identify emerging themes using the Consolidated Framework for Implementation Research. Salience levels were reported according to coding frequency, with valence levels measured to characterise the degree to which each construct impacted implementation.ResultsWe found several facilitators to safety programme implementation, including high staff receptivity, orientation towards patient-centredness and a desire for protocols. Key barriers included competing clinical demands, lack of knowledge about patient safety, limited governance, human factors and poor organisational incentives. Modifications included use of tools for staff recognition, integration of education into error reporting mechanisms and designation of trained champions to lead unit-based safety interventions.ConclusionImplementation of safety programmes in low-resource settings requires recognition of facilitators such as staff receptivity and patient-centredness as well as barriers such as lack of training in patient safety and poor organisational incentives. Embedding an implementation analysis during programme deployment allows for programme modification to enhance successful implementation.

In this study, the two key factors affecting the thermal performance of the insert rubber and stress distribution on the tire sidewall were analyzed extensively through various performance tests and simulations to promote the development of run-flat tires. Four compounds and two structures of insert rubber were designed to investigate the effects of heat accumulation and stress distribution on durability testing at zero pressure. It was concluded that the rigidity and tensile strength of the compound were negatively correlated with temperature. The deformation was a key factor that affects energy loss, which could not be judged solely by the loss factor. The stress distribution, however, should be considered in order to avoid early damage of the tire caused by stress concentration. On the whole, the careful balance of mechanical strength, energy loss, and structural rigidity was the key to the optimal development of run-flat tires. More importantly, the successful implementation of the simulations in the study provided important and useful guidance for run-flat tire development.

Although the electronic health (e-health) cloud computing system is a promising innovation, its adoption in the healthcare industry has been slow. This study investigated the adoption of e-health cloud computing systems in the healthcare industry and considered security functions, management, cloud service delivery, and cloud software for e-health cloud computing systems. Although numerous studies have determined factors affecting e-health cloud computing systems, few comprehensive reviews of factors and their relations have been conducted. Therefore, this study investigated the relations between the factors affecting e-health cloud computing systems by using a multiple criteria decision-making technique, in which decision-making trial and evaluation laboratory (DEMATEL), DANP (DEMATEL-based Analytic Network Process), and modified VIKOR (VlseKriterijumska Optimizacija I Kompromisno Resenje) approaches were combined. The intended level of adoption of an e-health cloud computing system could be determined by using the proposed approach. The results of a case study performed on the Taiwanese healthcare industry indicated that the cloud management function must be primarily enhanced and that cost effectiveness is the most significant factor in the adoption of e-health cloud computing. This result is valuable for allocating resources to decrease performance gaps in the Taiwanese healthcare industry.

AbstractObjectiveIndia has proposed legislating an upper limit oftransfat in partially hydrogenated vegetable oils and mandatingtransfat labelling in an effort to reduce intakes. The objective of the present study was to examine the complexities of regulatingtransfat in India by examining the policy processes involved and the perceived implementation challenges.DesignSemi-structured interviews (n18) were conducted with key informants from various sectors. Interviewees were asked about sources oftransfat in the food supply, existing policies that may influencetransfats and perceived challenges related to the proposedtransfat regulation, in addition to questions tailored to their area of expertise. Interview data were organised based on common themes.SettingInterviews were conducted in India.SubjectsInterviewees were key informants from various sectors including agriculture, trade, industry and health.ResultsSeveral themes were identified related to the complexity of regulatingtransfat in India. A lack oftransfat awareness, the large unorganised retail sector, a need for suitable alternative products that are both acceptable to consumers and affordable, and a need to build capacity were crucial factors affecting India's ability to successfully regulatetransfat. The limited number of food inspectors will create an additional challenge in terms of enforcement oftransfat regulation.ConclusionsAlthough India will face challenges in regulatingtransfat, legislating an upper limit oftransfat in partially hydrogenated vegetable oils will likely be the most effective approach to reducing it in the food supply. Ongoing engagement with industry, agriculture, trade and processing sectors will prove essential in terms of product reformulation.
