Background. The clinical course of chronic pancreatitis (CP) is characterized by symptomatic polymorphism, which is often associated with comorbidity of CP with other lesions of the gastrointestinal tract. Destructive-inflammatory processes of the upper gastrointestinal tract are very common. Such comorbidity is especially typical for young patients with CP (up to 45 years). The aim is to study the effectiveness of bioregulatory anti-inflammatory and metabolic therapy based on the dynamics of clinical indices and parameters of exocrine pancreatic function and the condition of the mucous membrane of the gastroduodenal zone of patients with CP. Materials and methods. We examined 50 patients with CP with exocrine insufficiency in the phase of unstable remission in combination with inflammatory diseases of the mucous membranes of the gastroduodenal zone. Group 1 (28 patients, age 40.25±1.34 years) received conventional treatment (CT) — pure pancreatin enzymes, proton pump inhibitors, antispasmodics or prokinetics (if necessary), enhanced by a step-by-step course of bioregulatory therapy, including parenteral course of anti-inflammatory drug Traumel C 2.2 ml intramuscularly twice a week (e.g., Monday and Thursday) and metabolic Ubiquinone Compositum 2.2 ml intramuscularly twice a week (e.g., Tuesday and Friday) № 5 each drug with the transition for oral administration of Traumel C per 1 tablet 1 hour after meals three times a day for 30 days. 22 patients with CP and concomitant exacerbation in gastroduodenal zone group 2 (age 41.34±1.21 years) received only complex CT. Results and discussion. A comparative analysis of the dynamics of treatment outcomes in the two study groups showed that almost all clinical signs of the disease, endoscopy and exocrine pancreatic function after treatment in the group using anti-inflammatory and metabolic bioregulatory therapy results were significantly better than in CT group. The mean time to normalization of mucous membranes of gastroduodenal zone in the group with an additional course of bioregulation was 14.4±0.86 days, while in the group of CT — 18.1±0.46 days. Conclusion. It was proved that the additional inclusion of step-by-step bioregulatory antiinflammatory (Traumel C) and metabolic (Ubiquinone Compositum) therapy according to the proposed scheme to the CT significantly more effectively (p˂0.05–0.001) improved the clinical course of the disease, contributed to the normalization of exocrine pancreatic function, as well as more quickly normalized the condition of the mucous membrane of the gastroduodenal zone — during 14.4±0.86 days, while in the group of CT — during 18.1±0.46 days.

Аim of research is to study the functional state of the pancreas in patients with various forms of chronic pancreatitis. Materials and methods. Complex examination of 210 patients with various forms of chronic pancreatitis (І, n=26 — obstructive, ІІ, n=56 — calcifying, ІІІ, n=78 — fibrous-parenchymal, ІV, n=50 — chronic pancreatitis complicated by pseudocyst) was carried out. Activity of enzymes (amylase, lipase, trypsin, phospholipase A) in serum, duodenal and ductal pancreatic contents was determined by common clinical techniques. In addition, content of bicarbonates was determined in duodenal and ductal contents. Exocrine pancreatic function was assessed by fecal elastase-1 level, endocrine function — by glycosylated hemoglobin level. Results of study. Increased activity of enzymes was determined (amylase, lipase, trypsin, phospholipase A) in the serum of patients of all groups: amylase activity (61.6%), trypsin (85.9%) by 3.5 times and phospholipase A (84, 6%) — by 2.3 times in comparison with the control group (p <0,001). Most often, α-amylase activity increased in patients of I and IV groups (with the same frequency of 70.0%) due to the process exacerbation or obstacle for the pancreatic juice outflow; increased trypsin activity was more frequent in III group of patients (91,0%). According to the fecal elastase test, the highest degree of pancreatic parenchymal involvement was detected in II and III groups of patients. Fecal elastase test showed increased level of enzymes in the duodenal and ductal pancreatic contents, increased level of bicarbonates, and decrease in the exocrine pancreatic function. Inhibition of basal pancreatic secretion and increase of debit-hour of pancreatic enzymes in duodenal contents after stimulation of the pancreas was states, which corresponds to the intraoperatively obtained data. Conclusions. It was found out that functional activity of the pancreas was preserved in 83.3% of patients, which coincides with literature data on changes in the indices of pancreatic functional activity upon 90.0% lesion of its parenchyma.

Relevance. Multiform exudative erythema is a polyetiological disease, which is characterized by damage not only to the skin, but also to the mucous membranes, mainly the mucous membrane of the oral cavity. The article reviews the literature on the clinical course of multiform exudative erythema of the oral cavity.Purpose. Study the clinical course of erythema multiforme.Materials and methods. We conducted a review of studies of the etiology, pathogenesis, prevalence of erythema multiforme according to domestic and foreign authors, and also reflected the identification of various forms and clinical manifestations of this disease.Results. The analysis of clinical manifestations of erythema multiforme exudative in the oral cavity, the most common symptoms and manifestations on the oral mucosa.Conclusion. The clinical picture of lesions of the oral mucosa in multiform exudative erythema is very diverse, which is important for the timely diagnosis of the disease.
