Transcatheter Therapy for Coarctation of Aorta: Analysis of 150 Cases

Background: This study was conducted to analyze the outcome of transcatheter intervention of Coarctation of aorta carried out on 150 patients.Methods: This was a retrospective study, conducted in Pediatric Cardiology Unit, Combined Military Hospital Dhaka and Lab Aid Cardiac hospital from January 2007 to December 2015. Data were collected by chart review.Results: Age of the patient varies from 7 days to 28 years. Sixty six (44%) patients were female and rest were male. Native coarctation was 138 (92%) and post surgical re-coarctation was 12 (8%). Neonatal critical Coarctation was 60 (40%). Thirty six (24%) cases were detected when they were investigated for hypertension. Abdominal coarctation was found in 18 cases (12%). Multiple coarctation was found in 9 (6%) cases. Six cases (4%) had balloon angioplasty twice and 12(8%) cases had stenting after recoarctation as they had body weight above 15 kg. Aneurysm and aortic dissection was observed in one (0.66%) case. Thrombophlebitis observed in 3(2%) cases and temporary pulse loss in 9 (6%) cases. No mortality was noticed.Conclusion: Balloon angioplasty for coarctation of aorta and stenting in suitable cases is a safe and effective measure to treat coarctation of aorta.Cardiovasc. j. 2017; 10(1): 56-62

A retrospective study was carried out on 50 patients at Paediatric cardiology unit of Combined MilitaryHospital, Dhaka to analyze the outcome of balloon angioplasty of coarctation of aorta. The study periodextended over two years starting from January 2007. Age of the patient varies from 7 days to 28 years. Twentytwo (44%) patients were female and rests were male. Native coarctation was in 44 cases (88%) and postsurgical re-coarctation was in 5 cases (10%). Neonatal critical coarctation was 19 (38%). Twelve (24%) caseswere detected when they were investigated for hypertension. Abdominal coarctation was found in 6 cases(12%). Multiple coarctations were found in 3 (6%) cases. Two cases (4%) had balloon angioplasty twice asthey developed re-coarctation. No intra-operative or post-operative complication was observed in any cases ofthis series. Data were collected by chart review. No test was performed on any of the subjects as part of studyprotocol. Ehrlichia canis is a tick-borne rickettsia. It can cause canine monocytic ehrlichiosis (CME). Infected dogs are often reported to have changes in their blood values, such as anemia, thrombocytopenia, increased liver enzymes, and increased kidney function values. This study aimed to collect data that may be related to infected dogs, including age, gender, breed, weight, close-open housing system, the use of ectoparasiticides products. The sample comprised 57 infected dogs. Collecting hematology and serum biochemistry changes in comparison with the reference values of dogs detected with Ehrlichia canis from 2017-2019, Thonburi District, Bangkok, Thailand was also carried out. In summary, dogs infected with Ehrlichia canis mostly included mixed-breed dogs aged between 1 and 10 years. There were no differences in body weight or housing systems. Dogs that had never used ectoparasiticide products or used them intermittently were infected more often (by 7.14 times) than protected. Clinical hematology and serum biochemistry found anemia, thrombocytopenia, and increased liver enzymes.

Objective: To determine the effect of subcutaneous recombinant human erythropoietin on blood pressure in predialysis chronic kidney disease (CKD) patients. Study Design: Case-series descriptive study. Place and Duration of Study: Combined Military Hospital Peshawar, from Mar 2016 to Sep 2016. Methodology: A total of 100 cases were enrolled. Inclusion criteria was patients of 18 to 60 years of both gender & estimated glomerular filtration rate (eGFR) below 30 mL/min/1.73 m2 having Hb <10g/dL and pre-dialysis while Exclusion Criteria was pregnancy or lactation, BP more than 140/90 mmHg, patients on Haemodialysis and worsening renal function. Baseline BP, body weight and eGFR of anaemic chronic kidney disease patients were recorded prior to EPO Alpha therapy. Erythropoiesis-stimulating agents (ESAs) i.e. EPO Alpha (50-100 Units/kg thrice or once weekly) was administered subcutaneously. Subsequent blood pressure, body weight and eGFR monitoring was done after 2 and 4 weeks post EPO Alpha injection. Results: Mean age range was 46.71 years with range of 20-60 years, 73 (73%) were male while 27 (27%) werefemales. Mean ± SD for other quantitative variables like eGFR was 23.12 ± 5.28, Hb levels (g/dL) was 8.62 ± 0.85,Weight (kg) was 56.66 ± 6.62 and duration of CKD was 9.87 ± 4.02. Frequency of Hypertension (post EPO) was 2(2%) and p-value was 0.453. Conclusion: We concluded that the frequency of hypertension in pre-dialysis patients with chronic kidney disease (CKD) receiving recombinant human erythropoietin (rhEPo) subcutaneously (SC) in low doses, is very low, so rhEPo can be used subcutaneously......................

Background: Orbital cellulitis is a diagnostic and therapeutic emergency, jeopardizing the  vital and functional prognosis. This study aimed to analyze the epidemiological, therapeutic and evolutional aspects of orbital cellulitis cases treated at the pediatric service at Mohamed V military hospital. Patients and methods: retrospective study including all the children aged between 1 month and 15 years treated for orbital cellulitis at the pediatric service of the Mohamed V Hospital over a period of 3 years (1st January 2016-31st December 2019). Results: 24 cases of orbital cellulitis were gathered. Age varied between 1 month and 15 years with a median of age of 6. years. Feminine predominance (58%) was noted . The disease mainly involved the sinus (32%).Clinically, fever was present in 10 patients (41%),palpebral edema was universal, proptosis was noted in 5 cases(20.8%),chemosis and ptosis were noted in 4 cases(16.6%),bacteriological testing identified micro-organisms in 3 cases. An orbital CT scan was performed in all cases of our study, showing preseptal cellulitis in 14 cases (58.3%), orbital cellulitis in 3 cases(12.5%),and orbital abscess in 7 cases. The medical treatment consisted of ceftriaxone, metronidazole and aminoside or amoxicilline clavulanic acid, corticosteroid therapy prescribed in 5 cases. Surgical treatment was indicated in 3 patients. The outcome of All cases was favorable. Conclusion: The majority of our cases had a positive evolution highlighting the advantage of an early diagnosis, and adapted antibiotic and a multidisciplinary patient care making the need for surgery rarely necessary.

ABSTRACT This retrospective study included pediatric intensive care unit patients receiving continuous veno-venous hemodiafiltration (CVVHDF) being treated with cefepime. The free drug concentration above one time the MIC (fT>1×MIC) and four times a presumed MIC (fT>4×MIC) of 8 μg/ml were calculated. Four patients received doses ranging from 48 to 64 mg/kg of body weight every 6 to 12 h. Three patients achieved 100% fT>1×MIC, with the fourth patient achieving 98% fT>1×MIC. Therapeutic drug monitoring should be considered for critically ill patients receiving cefepime on CVVHDF.
