Chronic pancreatitis is defined as continuing inflammatory disease of pancreas, characterised  by irreversible morphological changes leading to pancreatic insufficiency. Chronic pancreatic  insufficiency without pancreatitis is also seen in children suffering from cystic fibrosis which is  a disease with congenital enzyme deficiency. Mean age of chronic pancreatitis is about 40  years. We present a case of a 9-year-old boy who was diagnosed as a case of chronic calcific  pancreatitis. He was admitted with the complaints of paraumbilical pain and vomiting and  was found diabetic. Diagnosis was established by X-ray, USG and CT scan of abdomen and  ERCP. He was treated surgically in the form of lateral pancreaticojejunostomy (Puestow’s  procedure). DOI: http://dx.doi.org/10.3329/jemc.v2i1.11929 J Enam Med Col 2012; 2(1): 43-45

Chronic pancreatitis is a fairly common condition with pain being the major symptom and longitudinal pancreaticojejunostomy (LPJ) is performed for symptomatic relief. The aim of the study is to assess the outcome of LPJ for chronic calcific pancreatitis and to evaluate the factors influencing relief of symptoms. A prospective observational study enrolling 28 patients was conducted in the department of surgery of Shaheed Suhrawardy Medical College Hospital from January 2018 to December 2018. This study involved identification of various risk factors related to chronic pancreatitis, pancreaticojejunostomy, early post-operative outcome and postoperative assessment of pain relief at 1 and 6 months from surgery. Pain was assessed using Visual analogue scale (VAS). In chronic pancreatitis, there is a significant relief of pain in post-LPJ; the degree of relief was less in the alcoholics than non-alcoholics (p=0.09) and in smokers. There was also reduction in analgesic requirement and frequency of acute attacks of pain. In chronic pancreatitis, there is a significant relief of pain in post-LPJ, although the degree of relief is less in the alcoholics and smokers. The work is devoted to the study of the clinical and paraclinical peculiarities in children with CF (including respiratory tract microbiocenosis) in Kharkiv region. Also it is about the correlation of microbiological status of CF patients with the disease morbidity.Under the supervision were 30 children with cystic fibrosis. They conducted clinical, paraclinical (bacteriological examination of sputum and epithelial lining fluid, chest X-ray, CT scan of lungs) were performed.Clinical and paraclinic (bacteriological examination of sputum and epithelial lining fluid, chest X-ray, CT scan of lungs) were performed.During observations revealed that the condition severity of CF patients is associated with chronic P. aeruginosa infection, and B. cepacia. And also, that none of patients in Kharkiv region has determined  any of pathognomonic respiratory causative microorganisms – M. Tuberculosis and non-tuberculous micobacteria, H. influenza, Ralstonia picketi, condition of infecting by P. Aeruginosa is not identified, and this is the evidence of insufficient laboratory diagnostics.Key words: cystiс fibrosis, children, microflora В.А. В.А.  Клименко, Е.А. A patient with cystic fibrosis (CF) with pancreatic insufficiency presented with jaundice due to an ampullary tumour. CF is known for a higher incidence of gastrointestinal malignancies. The patient suffered from pancreatic insufficiency. At computed tomography (CT), pancreatic lipomatosis with absence of the pancreatic duct was seen. This is uncommon, also in patients with CF. During surgery, a total pancreatectomy was performed, because there was no possibility to construct a duct to mucosa anastomosis due to the absence of the pancreatic duct and more importantly the pancreas was already afunctional. The presence of lipomatosis increases the risk of leakage at the pancreaticojejunal anastomosis. Therefore, it is important to take this phenomenon, in this case already visible on the preoperative CT scan, into account during the workup for surgery.

The article provides an overview of the European recommendations on the diagnosis and treatment of chronic pancreatitis. The results of evidence-based research related to the epidemiology, course of chronic pancreatitis, assessment of the informative value of various imaging techniques, reatment of pancreatic insufficiency upon chronic pancreatitis, cystic fibrosis, pancreatic cancer are analyzed. A conclusion is drawn about the need for further carrying out of evidence-based studies.
