In 1993 the Massachusetts Department of Public Health (MDPH) began defining essential components of school health service programs, consistent with the public health model. The MDPH designed and funded the Enhanced School Health Service Programs to develop 4 core components of local school health services: (a) strengthening the administrative infrastructure; (b) promoting health education, including tobacco control activities; (c) linking school health services with health care providers; and (d) implementing management information systems. Funds were appropriated in 1992 from the tobacco excise tax. With additional funding appropriated in 1999 and 2000 from the Tobacco Settlement Fund, these school nurse–managed programs have increased in number. The goal is to develop a statewide system of high-quality school health service programs responsive to the specific needs of students in each community. To be effective, these programs must be recognized as essential components of the primary health care delivery system serving children.

IN DECEMBER 1951 a general discussion of the significance of organized health services for school children was presented in this column, using certain specific health problems for illustration. Two recent reports, "Priorities in Health Services for Children of School Age" and "Report on the First Session of the Expert Committee on School Health Services, World Health Organization," were cited at that time. Both of these have material of considerable interest to practicing pediatricians and child health workers. The "Priorities" statement, published by a joint committee of the Children's Bureau, Public Health Service and Office of Education, presents the results of the deliberations of a group of consultants who met in Washington in the summer of 1949. The group included physicians, nurses, teachers and health educators. They addressed themelves particularly to the matter of assessing the factors which would determine what services should take precedence as a program gets under way, or when limited funds and facilities are available. At the very beginning this report points out that the phrase in the title—Health Services for Children of School Age—was chosen to emphasize the fact that such services stem from the community as a whole. Furthermore, "Parents have primary responsibility for the health of their children. Health service programs should be designed to assist parents in discharging this responsibility but not to assume it for them.

Background: Irrespective of the provision of an integrated school health policy, the school health nurses continue to experience multiple challenges regarding the provisioning of school health service delivery.Aim: The aim of this paper was to explore and describe the experiences of school health nurses regarding school health service delivery in the Tshwane district.Setting: Schools in the Tshwane district in Pretoria were used in the study. Methods: A qualitative and descriptive phenomenological design was used to conduct the study. Purposive sampling techniques were used to select a sample from the population of school health nurses employed in the Tshwane district and conduct the enquiry because of their knowledge and experience of school health services. The researchers collected data by means of unstructured, one-on-one in-depth interviews. The Tesch data analysis method was used by the researcher and co-coder. The researcher identified categories, subcategories and themes and these were reduced into grouping topics that were related to one another. Results: Positive and negative experiences of school health nurses emerged. It was evident from the findings of the study that the factors affecting the quality of the integrated school health programme (ISHP) provided were interrelated. Most of these factors negatively affected service delivery. Conclusion: It was recommended that the partnership between the National Department of Health and National Department of Basic Education as the main role players should be sustained at all times to ensure the successful implementation of the ISHP.
