We present the preliminary results of the treatment of teeth with a deep, non-contained periodontal residual defect, vestibular bone dehiscence, and soft tissue recession, by combining an apical non-incised papilla surgical approach (NIPSA) to the defect and leukocyte platelet-rich fibrin (L-PRF) in the vestibular aspect. Four patients (upper left first premolar, upper left central incisor, upper right central incisor and upper right lateral incisor) have been treated. At one year of follow up, all cases showed a considerable reduction in the periodontal pocket depth, a gain in clinical attachment and no bleeding on probing, as well as an improvement in the marginal soft tissue minimizing soft tissue contraction (recession and/or loss of papilla) and improving soft tissue architecture. NIPSA plus L-PRF seem to improve clinical outcomes in deep non-contained intrabony defects associated with soft tissue recession.

Abstract Background: The aim of the study was to clinically evaluate the healing of intrabony defects after treatment with a new generation of platelet-rich fibrin (A-PRF+) respect to enamel matrix derivatives (EMD). Methods: Thirty (30) intrabony defects of 18 patients (9 males, 9 females) were randomly treated with A-PRF+ (test, n=15) or EMD (control, n=15). The following clinical parameters were recorded at baseline and 6 months after surgery: pocket depth (PD), gingival recession (GR) and clinical attachment level (CAL). After debridement the intrabony defects were filled with A-PRF+ in the test group, respectively with EMD in the control group, and fixed with sutures to ensure wound closure and stability.Results: Both treatment methods resulted in statistically significant PD reductions, respectively CAL gains six months post-operatively. No statistically significant differences were found between the two groups as the mean CAL gain was 2.33±1.58 mm in the A-PRF+ group, respectively 2.60±1.18 mm in the EMD group (p < 0.001).Conclusion: Within the limits of this study the new-generation platelet-rich fibrin seems to be as clinically effective as EMD in the surgical treatment of intrabony defects. Treatment with A-PRF+ or EMD resulted in reliable clinical outcomes. The use of A-PRF+ as a human autologous product can give a positive impact on periodontal healing.Clinical Relevance: A-PRF+ may be suitable for the treatment of intrabony periodontal defects. Abstract Background Preservation rhinoplasty (PR) is a new chapter in rhinoplasty history. The term was coined by Daniel in 2018 and represents a fundamental change in philosophy. Objectives The aim of this study is to discuss a single-surgeon case series utilizing PR techniques. Methods One hundred fifty-three primary rhinoplasty cases were studied retrospectively between December 2016 and August 2017. One hundred cases had at least 1 year of follow-up. Technical details were recorded, including dissection plane, ligament preservation, tip support, lateral crural maneuvers, alar contour grafts, and preservation of the dorsum vs traditional reduction. These 100 cases can be categorized as either complete preservation rhinoplasty (PR-C) or partial preservation rhinoplasty (PR-P). Results All patients had open rhinoplasty and the average follow-up time was 13 months. All patients had preservation of the dorsal soft tissue envelope, and in 36 the entire soft tissue envelope and ligaments were preserved. Fifty-four had preservation of the alar cartilages. Thirty-one had dorsal preservation. The combinations include: PR-C (skin, dorsum, and alars): 24; PR-P (skin and dorsum): 2; PR-P (alars and dorsum): 2; and PR-P (skin and alars): 7. Conclusions In most patients, the dorsal soft tissue envelope and nasal ligaments can be preserved. When possible, the lateral crura should be preserved and tensioning chosen over excision. Dorsal preservation is a versatile technique when proper patient selection is undertaken, and long-term issues with the middle vault and keystone area can be avoided. Some patients will benefit from total preservation where nothing is removed/disrupted and underlying structures are reshaped. Level of Evidence: 4

Background: Porous hydroxyapatite and β-tricalcium phosphate (β-TCP + HA) bone grafting material has resulted in clinically acceptable responses when used to fill the periodontal intrabony defects. PRF is an autologous leukocyte and platelet preparation that concentrates various polypeptide growth factors which therefore holds potential to be used as regenerative treatment for periodontal defects. Aim: The purpose of this study was to evaluate clinical and radiographic outcomes in periodontal intrabony defects treated with platelet rich fibrin compared to alloplastic bone graft material. Materials and Methods: Twenty subjects (10 subject per group, one site/subject) were treated either with platelet rich fibrin or alloplastic bone graft (30% β-TCP+ 70% HA). Primary clinical parameters: PD, CAL were taken at baseline, three months, six months and nine months post-operatively. Standardised radiographic data were collected at baseline, six months and nine months postoperatively. Results: Preoperative parameters were similar for both groups. Postsurgical measurements revealed a greater reduction in pocket depth in bone graft group (2.5 mm), greater CAL gain (2.2 mm) and greater defect fill (1.30 mm) as compared to platelet rich fibrin group (1.50 mm, 1.6 mm and 0.80 mm respectively ) at nine months. Conclusion: Treatment of intrabony defects with alloplast (30% β TCP+ 70% HA) or platelet rich fibrin both resulted in a significant probing depth reduction, CAL gain and bone depth reduction, with significantly better improvement in bone graft group.

Purpose: To describe a new surgical approach for the tectonic reconstruction of the anterior ocular segment and the management of scleral and conjuctival melting. Methods: Case series of patients demonstrating anterior segment anomalies, such as scleral and conjunctival melting. The anterior stromal part of a pre-cut corneal graft for Descemet’s stripping automated endothelial keratoplasty (DSAEK) was cross-linked with riboflavin and ultraviolet A irradiation and was used to cover scleral (scleral melting in a patient with necrotizing scleritis, one case) and conjuctival (dehiscence of the conjunctiva in patients with anti-glaucomatous valve exposure, two cases) areas. The endothelial part of all corneal grafts was used for DSAEK in patients with need of corneal endothelial transplantation. Results: Repair of scleral and conjuctival melting was successful in all cases. No intra- or post- operative complications occured. Visual acuity of all patients remained stable during the follow up period. One year postoperatively the corneal graft remained in place and no signs of inflammation were evident, while all grafts were epithelialized. Conclusions: The use of cross-linked corneal graft for the tectonic reconstruction of the anterior ocular segment represents an adequate surgical approach for the management of scleral and conjuctival melting; while, at the same time it offers the advantage of using one corneal graft for two recipients.

Reasons for performing the study. Equine cheek teeth periodontal disease is a common, often painful dental condition that may lead to premature tooth loss if left untreated. All cases of periodontal disease in this study were associated with diastemata, with the majority of cases being secondary or senile diastemata. At present limited objective information is available comparing different treatment methods for the condition to assist clinicians in making evidence-based treatment decisions. Objectives. To compare the effectiveness of four commonly used treatments for equine periodontal disease (additional to routine dental treatment and equilibration) to assess their merit in reducing periodontal pocket depth. Study design. Retrospective case series. Methods. Four commonly used equine periodontal treatments were compared: Removing feed material and lavaging the periodontal pocket with dilute chlorhexidine and rinsing the mouth with chlorhexidine based mouthwash (‘Hexarinse’), ‘clean and antiseptic lavage’ (CL); CL plus the addition of metronidazole antibiotics into the periodontal pockets (M); M plus the addition of a polyvinyl siloxane temporary filling over the diastema (PVS); and diastema widening to open the diastema and increase the interdental space between adjacent teeth, then PVS (DW). Periodontal pocket depth measurements were compared before and after treatment at treatment intervals between two and six months. Results. Treatment groups CL, M and PVS showed statistically significant reductions in pocket depth following treatment. Mean pocket depth reduction was greatest in the DW group (and this was the only group with no cases involving an increase in pocket depth), but this was based on a small sample size and was not statistically significant. Additional analysis to compare effectiveness revealed a strong confounding effect of initial pocket depth. After taking this into account, there was some evidence that DW was associated with smaller improvements than the other treatments, however this result was also based on a small sample size, and influenced by a small number of cases with particularly large improvements. Among the other treatments, no statistically significant differences in effectiveness were found. Conclusion. This study has shown that treatment methods CL, M and PVS are associated with statistically significant reductions in pocket depth. Due to the confounding effect of initial pocket depth, no clear differences in effectiveness were found between treatment methods.

ABSTRACT Objective: The primary objective of this study was to compare clinically and radiographically the efficacy of autologous platelet rich fibrin (PRF) and autogenous bone graft (ABG) obtained using bone scrapper in the treatment of intrabony periodontal defects. Materials and Methods: Thirty-eight intrabony defects (IBDs) were treated with either open flap debridement (OFD) with PRF or OFD with ABG. Clinical parameters were recorded at baseline and 6 months postoperatively. The defect-fill and defect resolution at baseline and 6 months were calculated radiographically (intraoral periapical radiographs [IOPA] and orthopantomogram [OPG]). Results: Significant probing pocket depth (PPD) reduction, clinical attachment level (CAL) gain, defect fill and defect resolution at both PRF and ABG treated sites with OFD was observed. However, inter-group comparison was non-significant (P > 0.05). The bivariate correlation results revealed that any of the two radiographic techniques (IOPA and OPG) can be used for analysis of the regenerative therapy in IBDs. Conclusion: The use of either PRF or ABG were effective in the treatment of three wall IBDs with an uneventful healing of the sites.

This study prospectively evaluated the clinical prognoses of short implants (7-mm long) in the mandible. We investigated the clinical prognosis of short implants in 20 patients (46 implants) according to the installation technique (submerged or nonsubmerged), installation depth (5.5-mm depth, 7-mm depth), and crown/implant ratio. We investigated the marginal bone loss and peri-implant soft-tissue index 12 months after the final prosthetic delivery. Twelve months after prosthetic delivery, no statistically significant differences were observed in bone loss in relation to the type of installation technique, installation depth, or crown/implant ratio. The plaque index and pocket depth indexes were not influenced by the installation technique, installation depth, or crown/implant ratio. We observed marginal bone loss of 3.3 mm in 1 implant from the nonsubmerged group. The total 1-year success rate was 97.83%. Based on this 1-year success rate, short implants had a good clinical prognosis, regardless of the installation technique, installation depth, or crown/implant ratio.
